1 Re: REQUEST FOR SUPPORT TOWARDS SSESE ISLANDS AFRICAN AIDS PROJECT (SIAAP) SIAAP BACKGROUND Ssese islands African Aids Project (SIAAP) is an HIV/AIDS non-government organization (NGO) in Kalangala district, Mugoye sub-county in Bugoma village, Ssese Islands. SIAAP was established in December 2004 by United States Army Officer, Major Frank Musisi a native of Kalangala, together with 10 other volunteers, with a major goal of reducing incidence of HIV transmission and tackling, the adverse effects of AIDS among the people in Ssese islands of Lake Victoria through going for voluntary testing and counseling, so that people could know their HIV/AIDS status in time and take precautionary measures. SIAAP was registered by the NGO board in December 2004 (REG NO. S 5914/5404) in Uganda and with the NGO Forum in Kalangala, with its operation area covering the whole district. The project is a community response to the HIV/AIDs pandemic in Kalangala district where services for counseling, testing, treatment and care is inadequate. It is against the above reason that SIAAP initiated a holistic approach to address the HIV/AIDs problem in Kalangala by focusing on creating supportive environment that can cause behavioral change among people on the islands and embrace voluntary counseling, testing, treatment and care. Why SIAAP was formed? Kalangala district is an Islanded area with a profound evidence of HIV/AIDS which is a challenge within and outside Ssese Islands African Aids project (SIAAP) Program area. Like any other Island areas in Uganda, Kalangala is faced with a number of challenges that includes, poverty, poor health services, High rate of HIV /Aids, High rate of domestic violence and mistreatment of women / Ladies, and children (Child abuse), Unemployment, poor sanitation, Diseases and ignorance s among others. In this case, SIAAP aimed at achieving the following objectives. To ensure pregnant mothers on treatment give birth to virus free children (negative babies) To prolong the lives of people with HIV/AIDS and allow them die with dignity by offering them medical, palliative counseling and social support. To ensure health education and sensitization at all fish landing sites through community outreaches. To improve accessibility to counseling services for children who are vulnerable, affected or infected by HIV/ AIDS and have been traumatized by the life s events. To ensure effective family planning activities are carried out at all landing sites in Mugoye subcounty and Kalangala in general. To reduce the rate of HIV /AIDS illiteracy among the population especially women.
3 To reduce the rate of other health related problems like Malaria, poor feeding and sexually transmitted infections (STIs). TARGET GROUPS AND AREA OF OPERATIONS. SIAAP targets children, youth, women, men and elderly in Kalangala district. For now, due to inadequate space and shortage of resources our activities or services are limited to Mugoye sub-county. The program area is predominantly fish landing sites/ Fishing communities with poor people, with high rates of HIV / AIDS, with high rate of other health related problems like Malaria, poor feeding etc. who are the beneficiaries. The organization focus areas include provision of Antiretroviral drugs (ARVs), counseling, family planning, general treatment, community outreaches, safe male circumcision, supportive counseling and referrals, Antiretroviral (ART) therapy enrollment. In addition to HIV/ AIDs services, SIAAP provides general treatment for other diseases including but not limited to: Malaria, Tuberculosis (TB), sexually transmitted Infections (STIs). GOVERNNANCE SIAAP is a non-government organization (NGO) formed to cater mainly on HIV/ AIDS and provision of general treatment to people of Kalangala with a board of governors, on top and a strong management team headed by the project coordinator who manages the day to day operations of the project. It has a holistic approach of addressing needs of both infected and affected people with in their communities. On ground is a network of village of trainees (VHTS) who play a key role in ensuring that the organizations interventions reach the targeted beneficiaries. The organization s headquarter is located at Bugoma, Kagulube parish, Mugoye sub-county in Kalangala district. CORE ACTIVITES In order to achieve SIAAP s objectives and contribute to the global 2030 sustainable development goals. SIAAP embarks on the following core activities. Ensuring healthy lives and promotes wellbeing upon all at all ages. Achieve gender equity and empower all women and girls. End poverty in all its forms everywhere. End hunger, achieve food, security and improve agriculture.
4 Ensure inclusive and equitable quality education and promote lifelong learning opportunities for all. All the above mentioned activities are elaborated as below: 1. Ensuring healthy lives and promotes wellbeing upon all at all ages. One of the SIAAP s main emphases is to sensitize people on health promotion practices so that they can get empowered on how effectively participate in all renovations aiming at improving and increasing control over their health. On weekly basis, SIAAP organizes health outreaches on different landing sites to test, counsel and treat people. Every Thursday, SIAAP organizes a counseling meeting with women over domestic violence and child abuse. Every 1 st of every month, SIAAP organizes and meets its HIV /AIDs positive patient s at its headquarters. This is known as antiretroviral (ART) day. This is the day when all HIV /AIDs positive penitents receive drugs. Youth, women, Children and Men plus school drop outs get trainings. 6 SIAAP started in 2005 with its major aims of handling HIV/ AIDs related cases. But as time went on, SIAAP started carry out general treatment due to high rate of health related diseases/ problems. Kalangala s Medical services are very poor as compared to main land areas in Uganda and this is due to geographical set up of the district. Due to lack of big hospital, most of patients in critical conditions (Emergences) are referred to either Masaka Regional Referral Hospital or Mulago Kampala. However such referrals cannot be managed by people with no stable income who cannot afford hiring choppers, speed boats and ambulances. This has caused many people to lose their lives especially pregnant mothers due to lack of potential to access medical facilities either at Masaka or Kampala. Hence the urgent need for an ambulance both the boat and vehicle. In addition we need three motorcycles in order to enhance our outreach program activities need more work. Due to such difficulties, Uganda s ethics and integrity minister Reverend father Simon Lokodo was almost losing his life while on official duties at Kalangala thus he had to be rushed to Mulago referral hospital at Kampala in a military helicopter. Imagine if such an incidence had happened to one of Kalangala s poor natives! What could have been the results? Below are media publications quoted about the minister s incidence. Minister Simon Lokodo collapses while on duty in Kalangala, airlifted to Kampala Lokodo was on Wednesday afternoon airlifted from Kalangala to Kampala in a military helicopter.
7 Hence urgent needs to help SIAAP to put in place such services so that poor Kalangala people can save their lives and at a free cost. 2. Achieve gender equality and empower all women and girls (Elimination of domestic violence).
8 Here, SIAAP target both the victims of domestic violence, especially the women and girls as well as the perpetrators (usually the men). The intention is to empower both male and female to learn how to use their power in all forms very well. This is usually done in our free meeting to all people in Mugoye sub-county especially domestically violated victims. Most of the domestic violence cases are related to poor earning (poverty), poor nutrition, HIV positive results and hunger (famine) in families / communities. 3. End poverty in all its forms everywhere: SIAAP has a plan / target of opening out a tailoring vocational institute. The aim is to empower valuable children with vocational knowledge and to enable them earn a living. On a monthly basis, SIAAP offers financial Literacy trainings aiming at promoting saving cultures to its clients (Patients on care and treatment). This is mostly done through encouraging and counseling meetings between SIAAP and its clients. However, SIAAP has started a policy of encouraging its patients on care and treatment to start village savings and loan associations. 4. End hunger, achieve food security and improve nutrition and promote sustainable agriculture. SIAAP always encourages its patients to always maintain food security. SIAAP is planning to initiate an agricultural training institute and this shall be done after expanding its working space. All SIAAP medical staff / personnel s do carry out small scale forming which enable them to have food. One of SIAAP employees in his Cassava plantation as an emphasis by the organization to have food security. SIAAP as an organization has a plan of buying a piece of land to be used as a demonstration farm to its patients on care and treatment and it even has a plan (SIAAP) of starting up an agricultural vocation to its patients.

We appreciate efforts from Uganda Cares, Uganda health marketing group (UHMG), Marie Stops, Buganda government, Kalangala district local government and government of Uganda (Ministry of health). All the above mentioned partners helped us (SIAAP) in different way in order for SIAAP to extend health services to people of Kalangala district.
