1 VACANCY POSITION GLOBAL FUND COORDINATING UNIT (GFCU) 1. POSITION TITLE : Technical Advisor, TB and HIV 2. PERIOD OF PERFORMANCE : Two (2) years, with the possibility of 3. PLACE OF PERFORMANCE : Maseru, Lesotho. STATEMENT OF WORK/POSITION DESCRIPTION : I. SCOPE OF WORK A. Introduction and Background: HIV/AIDS has had a devastating impact on Lesotho and its health care system. An estimated 22.9% of the adult population (15 49 years) of Lesotho is living with HIV (about 360,000 people, including 37,000 children under age 15) which is the second highest prevalence in the world. For the past decade, adult HIV incidence has remained high. Women are disproportionally affected by the epidemic: prevalence among pregnant women attending antenatal clinics is 25.9%, and among women aged reaches as high as 40%. Lesotho also has the world s second highest tuberculosis (TB) prevalence and TB co infection is a significant challenge: 75% of those with active TB are HIV positive. The estimated TB mortality rate in HIV co infected patients was 250 per 100,000 in The Government of Lesotho (GOL) s national strategic plans for HIV and PMTCT present a comprehensive HIV prevention, care, and treatment programs. In 2013, the 1
2 Ministry of Health (MOH) adopted a policy of lifelong antiretroviral therapy (ART) for all HIV positive pregnant women (Option B+). In July 2014, revised National ART Guidelines were launched which expand the definition of populations eligible for ART. Through leadership by the Government of Lesotho (GOL) and with the support of the President s Emergency Plan for AIDS Relief (PEPFAR), Lesotho has significantly scaled up HIV services from 17 sites in 2007 to 220 sites in AIDS related deaths have declined slowly since In 2015, the MOH took an aggressive approach to move from universal facility coverage (service access) to universal population coverage (service uptake). This shift is critical to reaching the epidemic s tipping point when the annual number of new infections falls below the annual net increase in people living with HIV (PLHIV) on ART. Reaching the tipping point is key to controlling the epidemic and reaching the UNAIDS 90/90/90 targets: 90% of PLHIV know their status, 90% of those diagnosed initiated on ART, and 90% of those on ART virally suppressed. The Ministry of Health is supported by partners to move the TB and HIV/AIDS agenda. Through the Support from the Global Fund, MOH will be implementation the integrated TB/HIV grant starting in July 2016 March 2018, with the focus on Sustainable Control of the Epidemic to support UNAIDS ambitious global goals. To achieve this ambitious goal, MOH will engage additional support to reach the ambitious targets. The New funding from The Global Fund calls for a new way of doing business pivoting to a data driven approach that strategically targets geographic areas and populations; with PEPFAR supporting 5 priority and highly populated districts, while the Support from the Global fund being earmarked to provide funding to the 5 mountainous and highlands districts. B. Basic Functions 2
3 The Technical Advisor for National TB Control Program (NTP) and HIV/AIDS will serve a critical management and technical advisory role in the Lesotho Ministry of Health with specific focus to the New Funding Model grant. This includes having a state of art knowledge and expert in HIV/AIDS and TB Program, advising the HIV and TB programs on all aspects of the program including PMTCT, VMMC, Test and Start/Treatment as prevention (TaSP); and specific technical and managerial support for the Ministry of Health s clinical programs across the entire continuum of care and including design, management, technical direction to ensure that Service Providers / consultants are providing quality services for interventions as stipulated in the Service Packages. The position will support and advance program quality, provide expert advice/support, regular monitoring and supervision at implementation sites ; and transfer skills through technical assistance and the development of MOH staff capacity and advise the Minister on Health Systems Strengthening innovations. Reporting to the Director General, the Technical Advisor will provide technical and programmatic oversight in the design and day to day management of implementation of Global Fund New funding Model. C. Duties and Responsibilities 1. Technical Assistance The Senior Clinical Advisor guides the technical development and oversight of the Lesotho s HIV prevention, care and treatment and TB program. The position will serve as a lead technical consultant HIV prevention (PMTCT, VMMC and other high impact biomedical prevention interventions including PEP, and the Family Planning HIV integration; Adult and Pediatric Care & Treatment, TB/HIV, Laboratory Services, and other relevant portfolios in support of high impact, efficient delivery of comprehensive HIV care services). Prepare MOH s high quality presentations for meetings, conferences and strategic engagements with partners, donors and Government counterparts. This responsibility includes: Maintaining state of the art familiarity with relevant research in HIV prevention, pediatric and adult treatment, TB/HIV, viral load monitoring and other critical HIV clinical services and interventions. 3
4 Providing technical assistance for the design, implementation and evaluation of HIV biomedical prevention, and comprehensive HIV clinical care programs to senior and technical level MOH counterparts, GFCU staff, implementing partners and other institutions in Lesotho. Assisting the Minister of Health and the Director General appropriate by participating in and providing technical assistance to HIV prevention and comprehensive HIV clinical care national, regional and international Technical Working Groups (TWG), task teams, workshops, meetings and campaigns. Providing strategic leadership and advice to the Global Fund (TGF) processes related to HIV biomedical prevention and comprehensive HIV clinical care activities. Identifying and documenting technically sound, cost effective, and practical approaches to HIV biomedical prevention and comprehensive HIV clinical care programming consistent with state of art policies and priorities. This includes assisting in the preparation of design and other pre obligation documentation; assisting in the preparation of the annual reports; ensuring the integration of health systems strengthening in the work of partners. Collecting, analyzing, synthesizing, and managing data and information related to HIV, and TB activities focused on program monitoring, evaluation, and improvement of performance in HIV biomedical prevention, comprehensive HIV clinical care and mobile HIV service portfolios. Promote information exchange between partners, regional institutions and networks regarding effective biomedical prevention, comprehensive HIV clinical services and mobile HIV interventions as well as the interface with community programs. Remaining informed and up to date on the most recent relevant research, technical and management issues affecting the design, implementation and evaluation of HIV biomedical prevention, comprehensive HIV care and mobile HIV services, including keeping senior and technical counterparts current on policies and procedures. Supporting efforts to ensure that state of the art gender and stigma reduction programming are effectively incorporated across biomedical prevention, comprehensive HIV clinical care and mobile HIV services and interventions and to ensure robust linkages between the community and HIV care and prevention services. 4
5 Soliciting and utilizing the input of technical advisors, USG staff and consultants, both local and international as needed. Ensuring that Global Fund supported biomedical prevention, comprehensive HIV care and mobile HIV services, interventions, programs and efforts address the organizational, technical and human dimensions of sustainability and the serious human resource crisis in Lesotho. 2. Project Management Oversight Advise the DG and designated activities awarded under the Ministry of Health grants. In this context, the incumbent will assist in aspects of project management for her/his assigned activities, including technical and budgetary oversight Interfacing directly with senior and technical level counterpart officials and partners regarding technical and program performance and based on critical analysis of the evidence and data, make appropriate recommendations for program improvement Carrying out routine monitoring and evaluation of project progress by reviewing technical documents and reports, completing site visits, and engaging in one on one reporting and meeting with key counterpart officials. Undertaking regular site visits of implementing partners and maintain regular communication with implementing partners to ensure that they have feasible, results oriented, gender sensitive work plans and sound management and financial plans in place; that activity implementation is proceeding in accordance with the approved TGF New Funding Model; that planned inputs and outputs are clearly identified and produced on schedule; that funding advances are requested, provided, expended and liquidated in accordance with financial plans and relevant TGF procedures; that completed activities are promptly closed out; and that Service Providers/Consultants comply with the terms and conditions of activity agreements. Coordinating and participating in the community quality assurance/quality improvement (QA/QI) initiative Site Improvement through Site Monitoring (SIMS) for comprehensive HIV clinical care and treatment and TB/HIV activities. 5
6 Assisting Service Providers and Consultants to troubleshoot implementation problems, and bring any major management or urgent issues to the attention of the Ministry of Health and the GFCU. 3. Program Planning and Reporting Support MOH to ensure that Service Providers and Consultants provide required and quality reporting documents. Ensuring that all program documentation meets both TGF requirements and standards. This includes analyzing and providing guidance to the MoH on what kinds of documentation are required based on regulations; on requests/guidance from donors. Support the MOH to prepare annual AJR adult and pediatric care and treatment and TB/HIV, and prepare annual reports and other necessary documents. 4. Assisting the Disease Control in Budget Tracking and Analysis Performing budget formulation duties for biomedical prevention and comprehensive HIV clinical care portfolios, including preparing detailed analyses and estimates of annual funding needs for one or more future budget years of Ministry of Health, Global Fund and PEPFAR funding. Monitoring obligations, commitments, expenditures, and results against targets. Ensuring that the budget system is used effectively as an evaluating tool in measuring program performance and effectiveness. Other duties as assigned. Travel Requirements Ability to travel up to 50% time (domestically) to support site supervision II. QUALIFICATIONS: 6
7 A) Required Minimum Qualifications and Experience Education: A minimum of Bachelors s in Medicine (MBChB) is required and other Master s degrees in Public Health, Epidemiology or any other related field Work Experience: i. A minimum of 10 years of increasingly responsible experience in HIV/AIDS Care and Treatment Programming and/or implementation. ii. Experience working in technical oversight, design and implementation of HIV biomedical and clinical programs in Africa. iii. Experience working with Global Fund, PEPFAR and U.S. government operations, rules, regulations, procedures, policies, and programs. Knowledge, Skills and Abilities: Strong knowledge of TGF procedures and experience with PEPFAR and USAID missions. Excellent management, interpersonal, oral/written communication and computer skills. Proven ability to work collaboratively in a team environment and to proactively build consensus. Ability in dealing effectively and diplomatically with multiple Partners and host country government agencies, non governmental organizations, faith based organizations, and the private sector. Excellent communication skills in English [Level IV written/verbal (fluent) required] and diplomacy. B) Desired Qualifications and Experience The following are highly desirable : Adept in public health, epidemiology and quantitative research methods. 7
