2 Positioning and site selection criteria for Xpert MTB/RIF 1. Ideally district or sub-district level, not central/reference lab level 2. Magnitude of the drug resistance or HIV associated TB problem 3. Current or estimated workload of the facility (taking into consideration 4 module system testing capacity, 15-20/day) 4. Infrastructure stable electricity supply, secure room for GeneXpert system, computer and cartridges, appropriate ambient temperature 5. Personnel who can be trained, perform testing and keep equipment in good order 6. Facility where transportation of sputum specimens or suspect referral is feasible 7. Sufficient capacity for appropriate treatment of all identified patients including those with rifampicin resistance 2
3 Selection of individuals to test based on risk assessment: primary considerations (1) A. Individuals known or suspected of having TB and at high risk of MDR-TB. This will include two categories: Persons who have been treated with anti-tb drugs and in whom pulmonary TB has again been diagnosed, i.e., all retreatment categories (failure, default, relapse); Persons suspected of having pulmonary TB and considered at risk of harbouring MDR-TB bacilli (risk groups as per national policies or as defined in WHO Guidelines for the Programmatic Management of DR-TB); These individuals should receive an Xpert MTB/RIF as primary diagnostic test. 3
4 Selection of individuals to test based on risk assessment: primary considerations (2) B. People living with HIV: All persons living with HIV who: have any one of: current cough, fever, weight loss or night sweats, those seriously ill and suspected of having TB regardless of HIV status, and those with unknown HIV status presenting with strong clinical evidence of HIV infection in HIV prevalent settings, should receive an Xpert MTB/RIF test as primary diagnostic test. 4
5 Where MDR-TB or HIV associated TB is of lesser concern Prioritization due to resource implications with pre-test screening strategies (including CXR where available) depending on available resources and the screening and diagnostic algorithms at country level. Facilities with quality chest X-ray (accessible, free of charge) Chest X-ray use as a screening tool Xpert MTB/RIF in those with abnormal chest X-ray Facilities with smear microscopy only Sputum smear microscopy initial diagnostic test Smear negatives tested with Xpert MTB/RIF if clinical suspicion of TB remains (should be referred or their sputum sent for further testing, preferably in a facility with Xpert MTB/RIF). 5

7 Management of patients with MDR-TB Rifampicin resistance is a reliable proxy for MDR-TB. Patients with confirmed resistance to rifampicin by Xpert MTB/RIF should therefore be started on appropriate MDR-TB treatment immediately and treatment adjusted based on the results of further DST results. TB patients identified by Xpert MTB/RIF without rifampicin resistance should be referred for appropriate first-line anti-tb treatment. If TB is not detected, the individual should be referred for further investigations and clinical management according to national guidelines. 7
8 Management of persons living with HIV and suspected of having TB HIV testing and counselling should be offered to all TB patients and TB suspects Among PLHIV, a TB suspect is defined as anyone who reports any one of current cough, fever, weight loss or night sweats. PLHIV who do not report any of these symptoms should be offered IPT The diagnosis process of TB among PLHIV should be expedited with minimum number of visits to prevent unnecessary deaths Empiric TB treatment should be given to seriously ill HIV positive patients who do not respond to 3-5 days of parenteral antibiotics even if investigations for TB have turned negative PLHIV diagnosed with TB should be started on ART as soon as possible within intensive phase of TB treatment. 8
9 Interim case definitions TB case MTB complex identified from a clinical specimen, either by conventional culture or by a newer method recommended by WHO such as LPA or Xpert MTB/RIF, or a case in whom a health worker has diagnosed TB and has decided to treat with a full course of TB treatment. In countries that lack the laboratory capacity to routinely identify MTB complex, a pulmonary patient with one or more initial sputum smear examinations positive for acid-fast bacilli (AFB) is also considered a case of TB. Case of MDR-TB A patient with MTB confirmed to have resistance to both rifampicin and isoniazid identified from a clinical specimen, either by conventional DST or by a newer method recommended by WHO such as line probe assay. Under consideration*: A patient with MTB confirmed to have rifampicin resistance (as a reliable proxy of MDR-TB) identified from a clinical specimen, either by conventional DST or by a newer method recommended by WHO such as line probe assay or Xpert MTB/RIF *WHO consultation April
10 Registration of TB cases diagnosed using Xpert MTB/RIF All TB cases diagnosed with Xpert MTB/RIF and rifampicin susceptible, irrespective of smear results, should be registered as Xpert MTB/RIF positive TB cases. Registration of TB cases diagnosed using conventional TB diagnostics remains unchanged if results of Xpert MTB/RIF are not available. All cases diagnosed with Xpert MTB/RIF and rifampicin resistant should be registered as Xpert MTB/RIF positive with rifampicin resistance. If isoniazid resistance is confirmed by conventional or molecular techniques, the case should be registered as MDR-TB. 10
11 Patient monitoring during treatment Molecular tests, including Xpert MTB/RIF, are not suitable for patient monitoring as these tests also detect DNA from nonviable bacilli. Patients whose diagnosis of TB is confirmed by Xpert MTB/RIF and who have rifampicin susceptible TB disease should be monitored during treatment with sputum smear microscopy. Patients with TB and rifampicin resistance confirmed by Xpert MTB/RIF and placed on MDR-TB treatment should be monitored by sputum smear and culture as per current WHO guidelines. Outcome definitions should follow current guidelines without any modifications, including patients with a baseline positive Xpert MTB/RIF result only. 11
12 Practical considerations: operational Adoption of Xpert MTB/RIF to be phased in considering that GeneXpert: Is a technology platform for other diagnostic services (MRSA, CD, in future HPV, viral load) Doesn t eliminate need for conventional smear, culture, DST Requires stable electricity supply Has range of ambient operating temperatures max. 30C (under revision) Requires storage space for cartridges (at 2-28C ), shelf life 18 months Testing capacity of 4 module system per working day is tests (depending on working hours, each test 100 min.) Requires annual calibration Xpert MTB/RIF testing require bio-safety conditions similar to the conventional sputum smear microscopy sample processing or testing 12
