2 I. Introduction This document will provide guidelines for Fox C-6 parents and schools regarding food allergies in order to develop appropriate procedures to reduce the risk of accidental exposure to those foods which can be life-threatening or cause anaphylactic reactions for students in our buildings. The Fox C-6 School District seeks to set age-appropriate guidelines for students and schools that minimize the risk for children with life-threatening food allergies. These guidelines include building-based general medical emergency plans, sample Individual Health Care Plans for students diagnosed with a life-threatening food allergy, training of staff, availability of on site medical equipment for quick response to life-threatening allergic reactions, and such other guidelines to allow students with life-threatening allergies to participate fully in school activities. It is the expectation that specific building-based guidelines/actions will take into account the health needs and well-being of all children without discrimination or isolation of any child. Open and informative communication is vital for the creation of an environment with reduced risks for all students and their families. Recognizing that it is not possible to eliminate all possible exposures, these guidelines also encourage age appropriate student education and self-advocacy. In order to assist children with lifethreatening allergies to assume more individual responsibility for maintaining their safety, guidelines will shift as children advance through the elementary grades and through secondary school. II. Purpose The purpose of these guidelines is to minimize the risk of exposure to food allergens that pose a threat to Fox C-6 School District students and to educate the school community about life-threatening food allergies. In furtherance of this goal, these guidelines are provided to assist everyone to better understand food allergies and related concerns. Each Fox C-6 School District school will: 1. Maintain a building-based general Medical Emergency Plan that includes a Life-Threatening Allergy Medical Emergency Plan. 2. Develop and implement an Individual Health Care Plan (IHCP) for students with diagnosed life-threatening allergies, based on medical documentation as appropriate for individual student needs and circumstances. 3. Implement annual life-threatening allergy and epinephrine auto-injector training for appropriate staff. 4. Monitor the use of food during the school day, as appropriate at the individual school building level based on student needs. III. Parent/Guardian Responsibilities 1. Notify the school nurse and principal of your child s allergies prior to the opening of school each year (or immediately after enrollment or a diagnosis). 2. Provide the school nurse with medical documentation from your licensed health care provider with medication and dietary orders before your child enters school. 3. Deliver/provide approved medications in proper containers to the school nurse on the first day your child enters school and maintain a non-expired supply in the nurses office for the duration of the school year.
3 4. No later than the first day your child enters school, provide the school nurse with a list of foods and/or ingredients that could cause a life-threatening or other allergic reaction. 5. Meet with the school nurse and other school staff to develop an Individualized Health Care Plan (IHCP) and Allergy Action Plan (AAP) and provide annual updates on your child s health status. This plan can include a mechanism for ongoing communication with school staff. 6. Educate your child in the self-management of their allergy as age appropriate, including: safe and unsafe foods, strategies for avoiding exposure to unsafe foods, symptoms of an allergic reaction, how and when to tell an adult they are having an allergy-related problem, and how to read food labels. 7. Consider purchasing a medical alert bracelet/necklace and encourage your child to wear it at all times. Contact information: Medic Alert 2323 Colorado Avenue Turlock, CA Provide the school with safe snacks. 9. Provide the school administration and nurse with updated emergency contact information. IV. School Administrator/Designee Responsibilities 1. Establish a basic Medical Emergency Plan for the building for use in any medical Emergency. 2. Ensure that an IHCP for each child with a life-threatening allergy is created and implemented. 3. Monitor satisfaction of cleaning protocol for classrooms, cafeteria, and other areas in the building. 4. Establish a procedure for how and when school staff should communicate with the main office and school nurse in the event of an emergency. This procedure should include guidelines for all school staff, coverage plans for the teacher and the nurse, and specific equipment to facilitate communication. 5. Promote a no sharing/no trading food rule. 6. Promote proper hand washing techniques and encourage students to wash hands before and after eating. 7. Discourage consuming food on routine school bus routes. Food may be allowed on longer trips with appropriate supervision by school personnel and for students with special health needs requiring the consumption of food at non meal times. 8. Establish within cafeterias designated tables for the restriction of specific foods, if stipulated by IHCP. 9. Request that the school nurse provide an annual educational program for building staff on life-threatening allergies in the classroom if there are students with life-threatening allergies in the building that school year. These training sessions for all school staff should be conducted as soon as practical each school year. Training will include a review of the signs and symptoms of anaphylaxis and the proper use of the epinephrine auto injectors and will emphasize the importance of prevention, risk reduction and early recognition of an allergic reaction and timely use of epinephrine.
4 10. Classroom teachers will carry AAP and medication, as designated by school nurse, for field trips. V. School Nurse Responsibilities 1. Develop an IHCP, corresponding Allergy Action Plan (AAP) and Medication Administration Record with parents/guardians and a multidisciplinary school team (when appropriate) prior to school entry or at the first opportunity following a new diagnosis of a life-threatening food allergy. IHCP will be reviewed annually. 2. Maintain open and frequent communication between home and school. 3. Communicate these plans to school staff that have a need to know. 4. At the beginning of each school year, provide education and training of all school staff to review the signs and symptoms of anaphylaxis and epinephrine auto-injector administration. 5. Maintain a list of students who require epinephrine auto injectors for allergic reactions in the Health Office. IHCPs and AAPs for those students will be easily accessible to substitutes. VI. Food Services Manager Responsibilities 1. Attend training if required by the student s individual health care plan. 2. Follow safe food handling practices to avoid cross contamination with potential food allergens. 3. Follow cleaning and sanitation protocol and safe food handling practices to avoid cross-contamination. 4. Set up procedures for cafeteria regarding food allergic students, including entering student s allergy into computerized database. 5. After receiving the medical statement for a student requiring special meals and in accordance with USDA guidance, the Fox C-6 School District Food Service Department will make reasonable modifications, as feasible, for meals served to students with food allergies. 6. Respond appropriately to all concerns from any student with a life-threatening allergy, including allowing student to see school nurse if complaining of any potential symptoms. VII. Cafeteria Hostess/Custodial Responsibilities 1. Attend training according to the student s individual health care plan. 2. Follow cleaning and sanitation protocol to avoid cross-contamination and thoroughly clean all tables, chairs and floors after each meal. 3. Provide a clearly labeled allergen safe table for students if required by an IHCP. 4. Respond appropriately to all concerns from any student with a life-threatening allergy.
5 VIII. Student s Responsibilities 1. Do not trade or share food with others. 2. Wash hands before and after eating. 3. Do not eat anything with unknown ingredients or known to contain any allergen. 4. Be proactive in the care and management of their food allergies and reactions based on their developmental level. 5. Notify an adult immediately if they eat something they believe may contain the food to which they are allergic, and/or if they believe they are having any symptoms of an allergic reaction. IX. Transportation Staff Responsibilities 1. Provide annual training for school bus drivers on managing life-threatening allergies, if indicated in IHCP. 2. Provide functioning emergency communication device (e.g. cell phone, two-way radio, walkie-talkie or similar device). 3. Know how to activate Emergency Medical Services (EMS). 4. Maintain a procedure of discouraging the consumption of food on routine school bus routes. Food may be allowed on longer trips with appropriate supervision by school personnel and for students with special health needs requiring the consumption of food at non-meal times. X. Final Note It is impossible to create a peanut-free or allergen-free environment. To create the illusion that the school environment is free of allergens would be misleading and potentially harmful. Instead, this protocol has been designed to increase awareness and communication, to prevent possible exposure to identified allergens, and to create an emergency procedure for allergic reactions.
6 Individualized Health Care Plan ANAPHYLAXIS ALLERGEN: Name: Parent: DOB: Special Ed: IEP 504 Bus: yes no Effective Date: School: School Nurse: Teacher: Doctor: Please provide a brief history of your child s allergy: Anaphylaxis is a rare, life-threatening allergy to certain substances such as foods, bee stings, chemicals and medications. It occurs rapidly and can close off the breathing passages. If instant treatment does not occur, it can be fatal. Student wears a Medic Alert neck chain/wrist bracelet (yes no ). Exposure to the above substance(s) should be avoided by the student, including skin contact, at all times! NOTE: For milder allergic reactions refer to Food Allergy Action Plan. Problem: Breathing difficulty Goal: Maintain airway. Action: I. Student has a severe allergy to the above mentioned which can be life-threatening. II. Symptoms of anaphylaxis which your child has experienced include: Tingling sensation in the mouth Feelings of apprehension Swelling of the tongue and throat Difficulty breathing Hives Itching Vomiting and/or abdominal cramps Chest pain Diarrhea Drop in blood pressure Weakness or dizziness Wheezing Shallow respirations Loss of Consciousness. III. If the above symptoms are noted: A. Give injection as prescribed. It is important to note that there is little downside to giving epinephrine if it is not needed. However delaying treatment can result in tragedy. B. Immediately call for emergency medical assistance (911). C. Notify parents.
7 IV. School staff must be trained in the following emergency procedures. A. Nurse will keep student s EpiPen and have available in their office. 1. Medication must be readily accessible and available at school or on field trips at all times. Student s medication will be kept at school in nurse s office. 2. Check expiration date to insure safety. 3. A minimum of three people must be trained in medication use. At school,,, and are trained to give the emergency EpiPen. 4. Student may carry and administer EpiPen when appropriate paperwork has been completed by both parent and physician. B. Administration of an EpiPen: 1. Do not remove safety cap until ready to use. 2. Place black tip on thigh at right angle to leg. 3. Press hard into thigh until you hear the click. 4. Hold for 10 seconds against thigh. 5. Massage injection area for 10 seconds. 6. Send injector with parent/medical personnel to emergency room. 7. Call 911 immediately. One person stays with student at all times. 8. Be prepared to initiate CPR if breathing stops. 9. Notify parents Problem: Epinephrine injection Goal: To understand what is given in the EpiPen. Action: I. Epinephrine is a hormone produced by all of us in the adrenal glands and is released in response to stress. An injection of epinephrine is a way to give a higher dose of something our bodies are already making. II. Epinephrine has three major actions that are important in the treatment of anaphylaxis. A. It constricts blood vessels and helps reduce the swelling caused by the allergic reactions. B. It helps to open the breathing passages. C. It helps prevent the blood pressure from falling. III. Epinephrine is very safe, but can cause minor side effects, including rapid heart beat, shakiness, headache, and restlessness. The severity of symptoms can quickly change. DOSAGE Epinephrine: inject intramuscularly (circle one) EpiPen EpiPen Jr. Twinject 0.3 mg Twinject 0.15 mg (see reverse side for instructions) Antihistamine: give medication/dose/route Other: give medication/dose/route IMPORTANT: Asthma inhalers and/or antihistamines cannot be depended on to replace epinephrine in anaphylaxis. Twinject 0.3 mg and Twinject 0.15 mg Directions Hold black tip near outer thigh (always apply to thigh). Remove caps labeled 1 and 2. Place rounded tip against outer thigh, press down hard until needle penetrates. Hold for 10 seconds, then remove. Swing and jab firmly into outer thigh until Auto-Injector mechanism functions. Hold in place and count to 10. Remove the EpiPen unit and massage the injection area for 10 seconds. SECOND DOSE ADMINISTRATION: If symptoms don t improve after 10 minutes, administer second dose: Unscrew rounded tip. Pull syringe from barrel by holding blue collar at needle base. Slide yellow collar off plunger. Put needle into thigh through skin, push plunger down all the way, and remove. Once EpiPen or Twinject is used, call the Rescue Squad. Take the used unit with you to the Emergency Room. Plan to stay for observation at the Emergency Room for at least 4 hours.
