1 The 2014 Survey of Optometric Practice was conducted by the American Optometric Association (AOA) Research and Information Center in July The survey collected information about private and corporate practice, and other practice types in which optometrists practiced during Respondents answered questions using information from the previous year of operation, not The 2014 Survey was sent to a random stratified sample of professionally active optometrists who had a valid address on file with the AOA or the state boards of optometry. is derived from the 2014 Survey of Optometric Practice, the intent of which is to gather key information about optometric practice characteristics, such as number and frequency of patient visits, hours worked by optometrists and optometric staff, as well as gross billings, expenses and income. This report provides descriptive statistics on characteristics of optometrists in the United States, patients of practicing optometrists, and optometrists participation in third party plans and managed care services. Data is presented for all optometrists, owner optometrists, solo optometrists and non-solo optometrists, new owners, independent contractors and employed optometrists. The Executive Summary aims to provide an overview of survey results. RESPONDING OPTOMETRISTS The majority of respondents to the 2014 Survey were owner optometrists (53%). Of these, the majority (53%) were non-solo owners in group practices; 47% were non-solo owners in group practices. Non-owner optometrists, including employed optometrists and independent contractors/lessees, accounted for 47% of respondents. Figure 1: Distribution of Optometrists by Primary Practice Type, 2013 Nearly three out of five optometrists (59%) indicated their primary practice was a private practice; 20% were in corporate practices. Of these private practices, 79% were owners. Non-owner optometrists comprised the majority (72%) in corporate practices. Among all responding optometrists, 70% said their primary practice type was their only practice in Of those who reported a secondary occupation, 19% were self-employed in their secondary practice and 11% were employees. Fifty-eight percent of responding AOA member optometrists were an owner of a practice compared to 42% of non-aoa member optometrists. 1
2 OPTOMETRIST CHARACTERISTICS The majority of owner responding optometrists were male. Male optometrists accounted for 76% of owner optometrists in all practice types; female optometrists made up 24%. However, 36% of new owner optometrists (those who graduated from optometry within the last 10 years) were female. Women comprise the majority of responding non-owner optometrists (51%) across all practice types; men made up 49%. Female optometrists again make up the majority (58%) of new non-owner optometrists. The majority of all responding optometrists (69%) were age 40 or older; 31% were under the age of 40. Of owner optometrists, the average age reported was 53. New owner optometrists averaged 35 years of age, and non-owners averaged 45 years of age. Twenty-six percent of responding optometrists have been in practice for more than 30 years, and 29% have been in practice for less than 10 years. Two-thirds of owner optometrists (65%) have been in practice for more than 20 years; 16% of owners have been in practice for 10 years or less. Of non-owner optometrists, the majority (67%) have been in practice for less than 20 years. 2
3 PRACTICE CHARACTERISTICS Responding optometrists reported an average staff of 5 optometrists and 5 optometric staff in Owner optometrists reported an average of 3 optometrists and 3 optometric staff. Non-owner optometrists averaged 2 optometrists in the practice and 5 optometric staff. Non-owner optometrists reported the largest practices with an average of 6,101 square feet of office space and eight exam rooms. The average for all owner optometrists was three exam rooms and 2,670 square feet of office space. Owner optometrists responding to the 2014 Survey reported an average of 39 hours spent in the office per week. The majority of that time (85%) was spent treating patients. Non-owner optometrists reported an average of 36 hours spent in the office per week, with 86% of that time spent treating patients. 3
4 PATIENT CHARACTERISTICS Patients between the ages of made up the majority (24%) of patients seen by owner optometrists in Patients over the age of 65 and patients between the ages of made up respective majorities (22% each) of patients seen by non-owner optometrists. More than half of all patients seen by responding optometrists in all practice types were male. Non-owner optometrists reported seeing the largest number of patients in their primary practice with 3,004 patients seen per individual optometrists within the practice over the course of Owner optometrists reported a total of 2,818 patients seen per individual optometrists within the practice in Owner optometrists averaged a total of 59 patients treated per week, including seven walk-in/emergency visits and 16 new patient visits. Non-owner optometrists reported an average of 63 total patients treated per week and reported 20 new patient visits and eight walk-in/emergency visits per week. 4
5 THIRD PARTY PAYER AND MANAGED CARE SERVICES Nearly all responding optometrists provided comprehensive eye exams for health maintenance organizations (HMOs) and preferred provider organizations (PPOs) in 2013; this includes the majority of both owner optometrists (83%) and non-owner optometrists (84%). Nine out of ten (89%) of owner optometrists participated in medical insurance plans; 95% participated in vision insurance plans. Owner optometrists reported participation in an average of ten medical and seven vision insurance plans. Eighty-three percent of non-owners participated in medical insurance plans, and 84% participated in vision insurance plans. Responding non-owner optometrists participated in an average of 9 medical insurance plans and 7 vision insurance plans. a public assistance program. Overall percentages for all responding optometrists can be seen in Figure 1. On average in 2013, owner optometrists accepted a reduced examination fee for 60% of their patients; owner optometrists also reported accepting reduced fees for 53% of patients requiring frames and lenses and 38% of patients needing contact lens services. Non-owners reported accepting reduced examination fees for 39% of their patients. Non-owners, on average, accepted reduced fees for 26% of patients requiring frames and lenses and 22% of patients requiring contact lens services. Figure 2: Percentage of Optometry Patients Covered by Insurance Programs, 2013 Responding owner optometrists reported that 45% of their patients were covered by vision insurance or a private insurance plan; 19% of patients were not covered at all. Of those patients covered by insurance, 25% were covered under Vision Service Plan (VSP) insurance. Incorrect addresses or delays in delivering the survey instrument may have excluded doctors from participating in, or receiving, the survey. (2) Optometrists who chose to participate in this survey were self-selected, which may mean that the survey attracted responses from optometrists whose practices were doing exceptionally well.
