2 Purpose Cortical thickness mapping is a computational analysis technique that uses clinical computed tomography (CT) imaging to plot the distribution of cortical bone thickness in 3D (Treece et al., 2010 and 2012). It is able to accurately estimate cortical thickness from clinical CT imaging data down to 0.3mm, far better than the inherent resolution of the CT imaging system. Given the understanding of the relationships of age, weight, and osteophytes with bone mineral density, we performed a study to investigate their effects of on cortical bone distribution in the proximal femur. Methods and Materials We analysed CT imaging data from a cohort of 230 women aged 66±17 years, selecting one hip from each randomly according to side. Demographics for these participants are shown in table 1. A DICOM analysis tool (Stradwin: was used to contour each hip semi-automatically and create a 3D surface mapped with individual cortical thickness values according to a colour scale (figure 1). Stages of the cortical thickness mapping process are shown in figure 2. and listed here: 1 First we define the approximate hip surface as a mesh with several thousand vertices; 2 Each vertex defines the location of a measurement with a sampling direction through the cortex; 3 CT data is sampled (in 3D) in this direction and thickness and mass derived from this data, presuming a constant density; 4 Measurements are mapped to the original surface - cortical thickness is shown as a colour wash; Page 2 of 12
3 5 In order to compare with other femoral surfaces, we deform a canonical femur (red) so that it matches this femur (green); 6 Then we map the data to a canonical femur, putting each subject in the same framework; 7 This process is repeated for all subjects in the experimental and control groups; 8 Significance is assessed using a fixed effects general linear model, correcting for multiple comparisons (using Worsley SurfStat); 9 Average results and significance can be shown simultaneously by showing insignificant results in grey. Statistical parametric mapping was performed to investigate the model effects of age, weight and osteophytes on cortical thickness (Friston et al., 1994). Results for cortical thickness are expressed either as an absolute thickness measure or as a percentage of the mean cortical thickness. 6 Results Mean cortical thickness maps generated from the 230 selected hips are shown in figure 3. Cortical thickness was generally thinner with advancing age, but was preserved in characteristic load-bearing regions at the medial femoral neck and lateral subtrochanteric cortex (figures 4a and 4b). Greater weight was also associated with significantly thicker cortex per kilogram at the same sites (figures 4c and 4d). Increasing osteophyte severity (as determined by the CT technque described in EPOS#3970) was detected as thicker cortical bone circumferentially around the articular margin at the femoral head-neck junction (figure 5) in a distribution reminiscent of 'ring' osteophytes (Jeffery, 1975). There was also a region of thicker bone with increasing osteophyte severity in the anterior femoral neck cortex, co-localising with a region preserved with advancing age and increased weight (figures 4 and 5). There is a patch of cortical bone in the anteromedial femoral neck that is not only preserved with advancing age, but also thicker with greater weight (white asterisk). 9 Conclusion The finding of generalised thinner cortical bone with advancing age supports the established notion that bone mineral density reduces throughout life after early adulthood (Looker et al., 2012). We also show a region of age-preserved and weight-thickened cortical bone that colocalises with peak compressive stress in the mid-gait cycle (Wagner et al., 2010), suggesting that cortex here may be responding to ground reaction forces. Although the relationship between osteoporosis and osteoarthritis is not necessarily inverse (Herrero-Beaumont et al., 2011), results from this cohort support this notion, at least in cortical bone. We have demonstrated for the first time the effects of age, weight and osteophytes on cortical bone thickness in the proximal femur. It will be essential for future analysis of bone in this region to take these effects into account. In relevance to the imaging of osteoarthritis (see EPOS#3971) This cortical thickness analysis technique also holds promise for automated measurement of joint space width by measuring the distance between opposing femoral and acetabular cortical bone peaks derived from the mapping process (figure 6).
