4 Hong Kong Arthritis & Rheumatism Foundation A Decade of Service to Hong Kong INTRODUCTION The Hong Kong Arthritis & Rheumatism Foundation was incorporated in 2001 as a non-profit making charitable organization. It is devoted to the objectives of improving the public understanding of arthritis and rheumatic disorders commonly seen in Hong Kong, and providing professional, social and financial support to sufferers of these conditions with the aim of improving their quality of life. The Foundation holds regular public educational talks, seminars, radio and TV talks by doctors, produces videos and booklets on the various rheumatic diseases, and organizes various programmes to help better understanding of these diseases by both patients and the general public so that they become better aware of them and know how to seek early treatment, understand the possible development and prevent serious handicaps which may be caused by these diseases. The Foundation provides needy patients with financial assistance to purchase medicine and to make home adaptations. It also subsidizes Patient Self-Help Groups to organize activities for patients. PATIENT SUPPORT COMMITTEE 1) Financial Support Chronic rheumatic diseases, such as osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, systemic lupus erythematosus and other bone and joint diseases, represent some of the most common diseases encountered in public outpatient clinics. Although there is no cure for most of these conditions, recent advances in the knowledge of their cause have led to the development of many new medical and surgical therapies that are effective in containing these disorders and restore patients physical function. However, as a result of the budget constraint of the public health care system, an increasing number of drug items and assistive devices, particularly the newer ones, are not subsidised by the Hospital Authority (HA). In other words, patients have to pay for these non-subsidised items. Undoubtedly, therefore, this causes an enormous economical burden to some patients and their families. The Foundation has set up a number of schemes to provide financial support for patients to purchase expensive drugs and rehabilitation aids and to organize social and educational activities. These programmes have been launched at all Hospital Authority hospitals and patient support groups since Hong Kong Arthritis & Rheumatism Foundation Annual Report
5 Total Allocations to Support Patients in Need HK$ 500, , , , , , , , , , , , , , , , , , i) Patient Support Fund The Patient Support Fund of the HKARF was set up in mid This is one of the major programmes of the Foundation with the primary objective of providing direct financial assistance to needy arthritis and rheumatic disease patients to purchase non-government subsidized medications and devices for arthritis rehabilitation, as well as home modification. Patients with low income are eligible for this fund. Each patient could apply to the Foundation for a maximum funding of HK$9,000 per year to purchase needed medications. 6 Patient Support Fund - No. of Approved Applications ii) Patient Emergency Fund The Patient Emergency Fund was set up in mid The aim of this Emergency Fund is to provide a one-off financial assistance for patients with sudden and unforeseen financial hardship or unexpected change in their health condition, which requires immediate treatment. There were 8 cases and HK$37,200 approved up to March iii) Activity Fund Beside the Patient Emergency Fund, the Foundation also set up the Activity Fund to sponsor registered patient self-help groups or related organizations to organize social and educational activities that promote the well being of people with arthritis and rheumatism. There were 25 cases and HK$90,387 approved up to March iv) Expensive Drug Support Scheme The Expensive Drug Support Scheme of the HKARF was set up in mid The aim of the Scheme was to reduce the financial burden of patients who required treatment with very expensive self-financed medicine. The Foundation negotiated with pharmaceutical companies to provide new expensive drugs, particularly the expensive biological drugs, to needy patients at large discounted prices, or an attractive bonus scheme would be offered. Applications had to be vetted and approved through the Foundation, but no direct subsidies were provided by the Foundation. 4 Hong Kong Arthritis & Rheumatism Foundation Annual Report
7 However, the Scheme has been halted temporarily from March 2010, because the Hospital Authority would not endorse the procedure whereby patients could get the drugs directly from pharmaceutical companies, but not through any pharmacist. Because of this change of policy, the Foundation allowed needy patients to apply for assistance through the Patient Support Fund instead. However, applicants using Remicade (Infliximab) would still face huge financial pressure as the drug would cost around HK$60,000 a year. Thus, the Foundation increased the ceiling of the Patient Support Fund funding for only those applicants to HK$20,000. Such level of financial assistance to the patients would be similar to the existing EDSS in that the pharmaceutical company would help the patients one-third of the total drug cost a year. There had been a total of 3 applicants using Remicade (Infliximab) who successfully obtained funding from the Patient Support Fund. Since December 2010, the Expensive Drug Support Scheme (EDSS) was resumed and the arrangement to provide temporary increase allocations to these patients was cancelled. 50 EDSS - No. of Cases Approved Bosentan (Tracleer) Enteric coated Mycophenolate Sodium (Myfortic) Mycophenolate Mofetil (Cellcept) Etanercept (Enbrel) Infliximab (Remicade) 2) Other Services i) Hydrotherapy Programme Since 2008, the Foundation has organized and subsidize a hydrotherapy programme for arthritis patients to use the hydrotherapy pool at the Queen Elizabeth Hospital to attend hydrotherapy classes under the supervision and guidance of physiotherapists so that the patients can experience the therapeutic effect of water exercises for their diseases. The programme has been so successful and popular with patients that it has now been extended to 2 hospitals, the Queen Elizabeth Hospital and the Pamela Youde Nethersole Eastern Hospital, with ten classes five evenings a week, to enable more patients to also benefit. It is important for them to have someone fully understanding their situation and providing the related information and guidance. Recognizing the needs for psychological counselling and the service gap, the Foundation approached the Department of Psychology of the University of Hong Kong and arranged to organize some tailor-made training to patient volunteers. Focus Group meetings were conducted by Prof Peter Lee and his professional team of psychologists to find out the psychological and emotional needs of patients at different stages of their ailments. The team provided 6 sessions counselling training to the patient volunteers from February to April and June After the training, Prof Peter Lee and his professional team of psychologists also conducted a survey in March 2010 to understand more thoroughly the psychological need of patients. Following the training the Foundation set up an Arthritis Help Line and selected trained volunteers to answer telephone enquiries on problems about arthritis from members of the public through the Helpline. The support line service was launched in November Hong Kong Arthritis & Rheumatism Foundation Annual Report
9 The support line service would be divided into 2 parts. First, patients could contact the volunteers for any enquiry or any problems that they might have with arthritis diseases. The volunteers would try to answer their queries, and provide suitable advice or share their experiences in coping with the disease. If the need arose and the callers agreed, the volunteers would introduce a mentors programme to provide further follow up assistance to the enquirer. A selected volunteer who had attended the training sessions from Prof Peter Lee would be assigned as a specific Mentor to make constant contacts with the caller in the following three months to provide more information and to provide more help as the caller s friend. If the volunteers found that the patients might need professional counselling service, the volunteers would refer the patient to social workers of the Community Rehabilitation Network who would follow up to assess if there was a need to have private psychologists to provide counselling sessions to the patient, at pre-agreed concessionary rates. 10 EDUCATION COMMITTEE Because most arthritis and rheumatic diseases are chronic and not immediately life-threatening, the potential debilitating consequences of the diseases are not fully understood by both patients and the public. However, greater awareness and understanding of the diseases, and the diseases management, can be life-changing. Therefore the Foundation has been proactive in promoting the awareness of rheumatic problems among the general public and providing arthritis patients with correct and up-to-date information on dealing with their diseases. 1) International Arthritis Day Events The Foundation organizes large-scale public awareness events every year to support the International Arthritis Day, which falls on 12 October every year, to draw public attention and to promote better public understanding about arthritis diseases. 11 2) Educational Talks and Publications A number of educational pamphlets and video discs have been produced on different arthritis conditions for use by health care workers at various related institutes and local organizations. Besides, the Foundation organized regular education activities each year. A large number of talks have been organized free for the public as well as health care professionals in the last ten years to promote better understanding of the diseases as well as to improve the skills of health care workers in caring for patients with arthritis. 14 3) HKARF Ambassadors Election Arthritis is the most common cause of disability and those patients need long-term treatment. Although their health condition might be bad, many patients were still active in voluntary works to help others and share their experiences to encourage other patients. In order to recognize the contributions of those patient volunteers, the Foundation organized HKARF Ambassadors Election from time to time. The first batch of HKARF Ambassadors was selected in Five of the 6 Foundation ambassadors, Ms. Chow Lai-yee, Mr. Lam Ming-chung, Mr. Lam Wai-hung, Ms. Lau Lai-bing, Ms. Ng Siu-mei, visited the Singapore National Arthritis Foundation (NAF) between September The ambassadors had ample opportunities to exchange ideas with members of the Singapore NAF also had a chance to attend part of a scientific symposium and an Alternative Medicine for Rheumatic Disease symposium during the visit. 1st Batch of HKARF Ambassadors: Name Ms. Chan Yim-yim Ms. Chow Lai-yee Mr. Lam Ming-chung Mr. Lam Wai-hung Ms. Lau Lai-bing Ms. Ng Siu-mei Disease SLE RA AS AS RA RA The second batch of HKARF Ambassadors was selected in As an award for these Ambassadors and to widen their knowledge of the diseases, a Study Tour to Guangzhou and Shenzhen Hospitals for arthritis patients was organized for these Ambassadors and families from 27 to 29 March The tour provided these volunteers a very memorable experience as it not only gave them an exposure to the hospital facilities (the 4th People's Specialist Rheumatism Hospital in Shenzhen and the Third Hospital of Sun Yat-sen University in Guangzhou) and tourist attractions in Shenzhen and Guangzhou but also an experience and knowledge about how arthritis patients were being treated in China. These conferences were aimed at providing more detailed, in-depth, most up-to-date information to the arthritis patients. The first Arthritis Patient Conference was held in October 2004 and also was the event to commemorate the International Arthritis Day The conference lasted for 2 days and was held in the University of Hong Kong with around 300 participants. 8 talks and 14 workshops on the following subjects were organized. 16 The second Arthritis Patient Conference was held in July 2010 at the Hong Kong Convention and Exhibition Centre. A total of 14 subjects were covered in 3 sessions of 3 time slots and a total 474 participated in the Conference. There was a Forum at the end of the event and almost all participants stayed on until the completion of the Forum and many patients were actively engaged in providing suggestions. A report of the Forum had been compiled and the collected views would be used to advocate the patients needs to the Hospital Authority. The newsletter was sent to different arthritis related Patient Self-Help Groups members, hospitals, doctors, elderly centres, and other social service centres. 14 Hong Kong Arthritis & Rheumatism Foundation Annual Report
17 6) HKARF Website The Foundation s website, in English and Chinese, was launched in 2006 to provide an online platform for arthritis related information, news and events of the Foundation, videos, and also a Q&A section. In 2011, the web-site was revamped to provide a more user-friendly interface. Besides, all the arthritis information would be updated and provided in both HTML and PDF files to suit users need. To facilitate all Chinese population around the world, both Chinese in Traditional Characters and Simplified Characters had been made available. The new website was launched in July, ) Apple Daily Column The Foundation contributed to a bi-weekly special column on arthritis diseases in the Apple Daily newspaper between January 2010 and April, Different doctors and allied health professionals were invited to write about treatment and exercise for different arthritis diseases. A total of 32 articles was published. 8) Health Action Magazine Interviews The Foundation cooperated with the monthly Health Action magazine and arranged to publish 12 special articles on the Foundation s work. Editors and Reporters from the magazine would interview the Foundation s Executive Committee Members, Hon. Advisors, doctors, HKARF Ambassadors, other allied health professionals and different arthritis Patients Self-Help Groups and write stories on different arthritis diseases, and how the patients cope with fighting with these diseases. The first issue was published in March 2010 and the last article was completed in March The 12 issues of interview were then compiled into a book to commemorate the Foundation 10th Anniversary. 18 FUND-RAISING COMMITTEE The Foundation is a charitable organization without financial support from the government. Our source of funding relies solely on corporate and individual donations through various fund-raising activities. Every year, different kinds of fund-raising events were held to attract donations. 1) Charity Dinner Charity Dinners had been the major fund-raising activity of the Foundation. There were 4 Charity Dinners held from 2005 to The first Charity Dinner was held on 21 October 2005 at the Miramar Hotel Ballroom. 155 guests attended the Dinner. Dr. Patrick Ho Chi-ping was invited to be Guest of Honour and there was different entertainment including singing performance, patient s musical performance and lucky draw. A total of HK$504, was raised after deduction of expenses. The second Charity Dinner was held on 22 March 2007 also at the Miramar Hotel Ballroom and 300 guests attended. Mr. Anthony Wu was invited to be Guest of Honour and all the guests enjoyed the programme including singing performance and charity auction. A total of HK$1,026, was raised after deduction of expenses. The third Charity Dinner was held on 28 June 2008 at the renovated Miramar Hotel Ballroom and 288 guests attended. Dr. David Fang was invited to be Guest of Honour and the enjoyable programme including singing performances by Dr Fang and Mr Johnny Yip, and a charity auction was conducted. A total of HK$966, was raised after deduction of expenses. The latest Charity Dinner was held on 12 May 2011 at the Mira Hotel Ballroom and 300 guests attended. Ms. Florence Hui was invited to be Guest of Honour. A charity auction of the famous calligraphy of Prof Jao Tsung-I had attracted very competitive bidding. Total donations and income amounted to $2.69 million, and a record net proceed of $2,478, was recorded after expenses. 16 Hong Kong Arthritis & Rheumatism Foundation Annual Report
19 2) Charity Concert The Foundation cooperated with different organizations to hold Charity Concerts for fund raising. These partners included the Chinese Operatic Artists Association ( 八和會館 ), Kam Sing Fai Artist Group ( 錦陞輝劇團 ) and the SAR Philharmonic. The first Chinese Opera Charity Concert was held on 9 October 2003 at the City Hall Theatre. A number of popular Chinese opera singers, many of whom members of the Chinese Operatic Artists Association, and other artists performed during the concert which was attended by a full house of audience. A total of HK$348, was raised after deduction of expenses from this concert. The second Chinese Opera Fundraising Concert was held on 22 October, 2004 at the Sunbeam Theatre. We were fortunate to have a number of popular artists from Kam Sing Fai Artist Group to perform during the show which attracted a full house audience. A total of HK$349, was raised after deduction of expenses. The Foundation co-organized with the SAR Philharmonic a Charity Concert on 12 April 2009 (Sunday). The concert not only provided the rheumatic patients and their families with a pleasurable opportunity to socialize and experience a philharmonic concert in the well equipped Hong Kong Cultural Centre Concert Hall, but also yielded sponsorships and donations amounting to HK$276,400, with net proceeds of HK$204, recorded after expenses. The event also helped publicize the work of the Foundation and enable the Foundation to get new connections. 3) Flag Day The first Flag Day of the Foundation was held on 25 November A total of 800 volunteers participated in this event which raised HK$616, net. The second Flag Day was held on 12 September 2009, in Hong Kong Island only. 750 volunteers participated in this event. Total donations and income amounted to $301,709.20, and a net proceed of $286, was recorded after expenses. Hong Kong Arthritis & Rheumatism Foundation Annual Report 17
20 4) 7-Eleven Donation Collection Service for Wise-Giving Charity The Foundation joined the Wise-Giving fundraising platform provided by the Hong Kong Council of Social Services, to make the public better aware of the charity work of the Foundation. Through this arrangement, donors could make donations to the Foundation with a special bar-code through 7-11 convenience stores. A total of HK$5,600 donation was collected from this platform. 請攜印有捐款條碼到全港任何一間 捐款 5) Donation Boxes at SOGO Supermarket With the kind permission of the SOGO Department Store, two donation boxes were placed at the Causeway Bay SOGO Supermarket check-out counters to collect donations from shoppers. The boxes would be placed from June 2010 to December 2011 and, up to 31 March 2011, the donation collected was HK$6, ) Donation from other activities There were other organizations which held events to collect donations for the Foundation. In February 2005, New Method College set up two charity sales counters for the Foundation at the Lunar New Year Fair. All proceeds after deducting cost had been donated to the HKARF. The total donation was HK$28, The Hon. President, Dr. Tam Wah-ching donated 300 tickets for the concerts, Three Broadway Divas, performed by three of the world s famous stars of the musical stage - Christiane Noll, Jan Horvath, and Debbie Gravitte - and accompanied by the Hong Kong Philharmonic Orchestra. The concerts were held in the Hong Kong Cultural Centre on 25 May and 26 May There was totally HK$155,460 donation collected from this event. The Foundation joined the 2nd Victoria to Peak Challenge organized by Club Balance on 1 October The Foundation assisted the event by providing 10 volunteers to help with the logistics of the race and Club Balance donated HK$10,000 to the Foundation. The Foundation also joined the 5K Cross Country Challenge & Family Day, which was held on 17 April 2011, organized by the Hong Kong Multisports Association. A Long Distance Running Sports Injury Sports Seminar was organized pre-race and a talk was provided by the Foundation. Participants of the seminar would each donate at least HK$50 to the Foundation. In addition, the Foundation was invited to participate in setting up a booth at the race assembling site to exhibit the Foundation s work and to conduct a charity of products donated by its volunteers and business corporations. The Foundation became the beneficiary of this series of events and a total of HK$24, donation was collected from the pre-race talk, the sponsorship to the racers, and the charity sale at the event. 18 Hong Kong Arthritis & Rheumatism Foundation Annual Report
21 SCIENTIFIC COMMITTEE The Scientific Committee of the Foundation was to bridge the gap for education and services not provided by the various specialties. Besides, the committee would promote training and research in Rheumatology related specialties to fulfil the objective of promoting the understanding of arthritis and rheumatic disorders among the medical professionals in Hong Kong. 1) HKARF Research Grant The HKARF Research Grant was launched from January The aim of the Research Grant is to support Hong Kong doctors health care professionals scientists involved in the management study of patients with chronic arthritis and rheumatic disorders, in conducting research projects which provide new insights for the care of this group of patients. Winners of HKARF Research Grant are listed below: Dr. Tam Lai-shan, The Prince of Wales Hospital Project: Cardiovascular risk profile of patients with psoriatic arthritis compared to controls. Dr. Mok Mo-yin, Temy, Queen Mary Hospital Project: Prevalence of coronary artery disease in patients with systemic sclerosis Dr. Leung Ying-ying, North District Hospital Project: Evaluation of the Arthritis Disease Self-management Program in Chinese patients with Psoriatic Arthritis as compared to Rheumatoid Arthritis (CRE ) Dr. Mok Mo-yin, Temy, Queen Mary Hospital Project: A pliot study on the clinical usefulness of interferon-gamma assay in detection of latent tuberculosis in patients with rheumatic diseases given anti-tnf-therapy in Hong Kong Dr. Leung Moon-ho, Queen Elizabeth Hospital Project: Leisure activities, participation in rheumatic patients and their inter-correlation with disease activities, subjective well-being and satisfaction with life 2) HKARFHKSR Training Scholarship The HKARFHKSR Scholarship is provided jointly by the Hong Kong Society of Rheumatology and the Foundation, to Hong Kong medical professionals in the field of Rheumatology to pursue training overseas since The scholarship has been postponed in upon further discussion with the Society with the view to revising the rules and offering more appealing package for rheumatologists as well as allied-health professionals. From 2011, there would be one scholarship for medical trainees in rheumatology (1 quota per year) and scholarships for allied-health professionals (6 quotas per year). The scholarships would be in the form of lump-sum grants to cover costs of airfare and accommodation. Hong Kong Arthritis & Rheumatism Foundation Annual Report 19
22 3) East Meets West Series Since 2009, the Foundation had organized East Meets West series for allied health professionals. This educational series aimed at promoting interflow of knowledge and experience on patient management between health care professionals practicing western medicine and traditional Chinese medicine in the field of rheumatology. A Symposium of acupuncture was held on 26 September 2009 and the second symposium on Tai Chi was held on 30 October These symposiums were well received. The third East meets West Series symposium on Traditional Chinese Medicine in Arthritis was held on 17 September ) CHARM Newsletter for Professionals From February 2009, the Foundation published a new topic-focused medical bulletin for medical and allied health professionals. This newsletter, entitled CHARM (Care, Health, Arthritis, Management) was published to promote an interflow of updated knowledge in the management of patients with rheumatic diseases on a multi-disciplinary basis. Quality management of patients with chronic rheumatic diseases entails efforts from various disciplines. Around 4,000 copies of each issue of CHARM were printed and distributed to the hospitals and various professional organizations including rhematologists, orthopaedic surgeons, physiotherapists, occupational therapists, paediatricians, and nurses.
