NAME DATE OF BIRTH CONSTANCE NKONYANE (MOFOKENG) 10/06/1962 OCCUPATION DATE OF ASSAULT DATE OF EXAMINATION 08/ 04/ / 09/1993 HISTORY At about 9 p.m. on 08/04/1992 Constance Nkonyane was hit by several pellets and a bullet. As a result she suffered injuries to her left buttock, her left hand and her right thigh. She was bleeding profusely, particularly from the thigh wound. 2/...She was Emergency: Code 440 Channel 11
2 She was taken by neighbour's to the Willem Kruywagen Hospital in Germiston where X-rays were taken of her right leg, her left hand and buttock. Following this the bullet in her right thigh was removed under local anaesthetic in theatre. She remained in hospital for three days where dressings were done and she was treated with antibiotics. Following discharge her dressings were changed daily or every second day and she attended the Natalspruit Hospital approximately at two weekly intervals. The thigh injury healed after approximately six weeks the hand injury however, took longer to heal. At the time of being shot she was pregnant. PRESENT COMPLAINTS 1) Her left hand is very painful while doing washing. 2) Her right knee is painful and becomes swollen in cold weather. She feels that her right knee pain is getting worse. 3/...EMPLOYMENT
3 3. EMPLOYMENT Prior to her being shot she was a housewife and did her own house work. Following the shooting she is unable to do heavy washing because of pain in her left hand. Her left knee is painful during cold weather. SOCIAL AMENETIES She experiences pain in her right knee while walking in cold weather. HEALTH PRIOR TO THE SHOOTING Her health was good. She was on no medications, and specifically had no problems with her left hand or right leg. EXAMINATION FINDINGS Mrs Constance Nkonyane appeared generelly well. There was a 6 mm hyper-pigmented palpable scar in the web space between the first and second finger of the left hand and adjacent to the metacarpophalangeal joint of the dorsum of the thumb. 4/...There was
4 4. There was a small palpable tender scar over the palmar aspect of the hand in the web space between the first and second finger of the left hand. The scar was approximately 5 mm in diameter. There was decreased sensation over the medial side of the dorsum of the left thumb. There was a hyper-pigmented scar approximately 2 cm by 1.2 cm over the postero-medial aspect of of the mid right thigh. There was a small hyper-pigmented scar over the left buttock. In the scar was a very small palpable nodule. SPECIAL INVESTIGATIONS X-rays of Mrs Nkonyane's pelvis were taken on 29/09/1993. (radiologists report enclosed). DIAGNOSIS As a result of her being shot Mrs Nkonyane suffered a soft tissue injury to her left hand. This took a long time to heal and has left permanent scars in her hand. The palmer scar is tender to the touch. She also has decreased sensation over the dorsum of the medial side of her left thumb. She has a scar over her right thigh and a scar over her left buttock. There is a large foreign body lodged in her left buttock. 5/...PROGNOSIS
5 5. PROGNOSIS AND FUTURE MEDICAL COSTS More than 17 months after being shot Mrs Nkonyane has a tender scar over the volar aspect of her hand in the web between the first and second fingers. She has a large radio opaque foreign body in her left buttock. It would be reasonable to make provision for removal of the foreign body in her left buttock and at the same time revise the scar on her hand. This would entail a period of hospitalisation of approximately two days, a period of incapacity of approximately two weeks and would cost in the region of R4 000,00 at todays prices. CONCLUSION Mrs Constance Nkonyane suffered a shocking experience when she was shot in her left hand, left buttock and right thigh. She suffered severe pain and bled profusely and required admission to hospital for a three day period. The wound on her left hand took more than six weeks to heal. She retains residual disability in that there is decreased sensation over part of her left thumb and continues to experience pain in her left hand while doing heavy house work and pain in her leg during cold weather. 6/...The scars
6 The scars will remain as permantly disfiguring marks. I find no reason to suspect that her life expectancy has been altered by her being shot. FCS (S A) Orth.

ASSOCIATES PO BOX 8694 JHB SOOO 29/09/93 DR 6 VERSFELD PELVIS A large irregular shaped metallic shrapnel -fragment as well as several smaller metallic shrapnel -fragments are noted in the soft tissues of the left buttock. The pelvic bones as well as lower lumbar spine region are intact and no traumatic deformity is identified in the left and right hemipelvis as well as in the sacrum. No disruption of the sacral arch. The hip joints are normal in the AF projection. No diastasis of the SI joint or symphysis pubis. CONCLUSIONS Metallic shrapnel fragments are noted lodged in the soft tissue of the left buttock. The underlying bony pelvis is intact and no pelvic bone fractures or other bony abnormalities are visualised. DR BRIAN BP/vj
