S druge strane, postojeće invazivne (bronhoskopija) i poluinvazivne (indukcija sputuma) metode mjerenja upale dišnih putova teško je primjenjivati opetovano u kliničkoj praksi. Zadnjih je godina stoga poraslo zanimanje za korištenjem izdaha, odnosno za jednostavnim, neinvazivnim skupljanjem uzorka iz donjeg dijela dišnog sustava ljudi. Neinvazivno praćenje bolesti može pomoći u diferencijalnoj dijagnostici bolesti pluća, njezine težine te kod procjene odgovora na terapiju. Neprestano se povećava broj literaturnih dokaza o tome da analiza izdaha, posebice kondenzata izdaha, može imati važno mjesto u dijagnostici i kliničkoj obradi bolesnika s astmom, kroničnom opstrukcijskom bolesti i drugim bolestima pluća. Članak pregledno opisuje ovaj novi uzorak, njegove sastojke i probleme koje treba razjasniti prije moguće primjene u rutinskoj kliničkoj praksi. Ključne riječi: dišni testovi, kondenzat izdaha, biljezi upale, plućne bolesti Adresa za dopisivanje: UVOD U skladu s kontinuiranim stjecanjem novih spoznaja o temeljnim patofiziološkim procesima plućnih bolesti, neupitno će se mijenjati i dijagnostička strategija kod tih bolesti. Tako se pretpostavlja da će praćenje upale dišnih putova, koja je u osnovi većine plućnih bolesti, dati novi uvid u patološke procese dišnih putova i to ranije i točnije nego tradicionalna dijagnostika. Upala dišnih putova je od središnje važnosti u astmi i drugim bolestima pluća i glavni je uzrok simptoma astme. Međunarodne smjernice i preporuke pridaju veliku važnost terapijskom tretmanu upale u astmi (1). Međutim, procjena težine i progresije bolesti temelji se većim dijelom na kliničkim simptomima bolesti i mjerenju plućne funkcije. Klinički simptomi i spirometrijski nalazi variraju ovisno o stupnju bronhoopstrukcije i ne odražavaju točno stupanj upale dišnih putova. Intenzivni upalni procesi, mogu međutim znatno Prof. dr. Izravna procjena upale dišnih putova primjenom postojećih načina dobivanja uzorka za ispitivanje, primjerice bronhoskopije (bronhoalveolarni ispirak kao uzorak), invazivan je postupak, neugodan za bolesnika, neprimjenjiv kod djece i teških bolesnika. Inducirani iskašljaj, kao poluinvazivan postupak dobivanja uzorka (inhalacija hipertonične otopine soli) je također ograničene primjenjivosti s obzirom na dob ispitanika, mogući proinflamacijski učinak i bronhospazam koji se javlja kod 14% postupaka indukcije iskašljaja u umjerenoj i 25% postupaka indukcije u teškoj astmi (4). Upotrebu iskašljaja kao uzorka djelomično ograničava i to što je za njegovo dobivanje potrebno dulje vrijeme (oko 2 sata). Praćenje upale tako nije uključeno u postojeće smjernice/preporuke o astmi (ili drugoj bolesti pluća), premda je dokazano da definiranje prirode, stupnja i intenziteta upale poboljšava kontrolu astme (5). Važno je također naglasiti da ispitivanja biokemijskih upalnih medijatora u plazmi i mokraći kod bolesnika s plućnim bolestima odražavaju prije sistemsku nego lokalnu, plućnu upalu. Dakle, premda su relativno dobri uzorci za ispitivanja raznih biokemijskih pokazatelja kod plućnih bolesti, iskašljaj i bronhoalveolarni ispirak se ograničeno ispituju zbog invazivne prirode postupka njihova dobivanja. Stoga se niz godina kontinuirano rade istraživanja na manje invazivnim, točnije neinvazivnim postupcima dobivanja odgovarajućih uzoraka za ispitivanje. To su izdah i kondenzat izdaha, u kojima se izvode tzv. dišni testovi. Neinvazivni postupci dobivanja uzorka primjenjivi su kod djece, ambulantnih bolesnika, u kućnoj primjeni, kod bolesnika s teškim oblicima bolesti te bolesnika na strojnoj, potpomognutoj ventilaciji (strojnom prodisavanju). Analiza tih uzoraka omogućuje longitudinalno praćenje bolesti, pomaže kod diferencijalne dijagnostike plućnih bolesti, procjeni težine bolesti, odgovora na terapijski postupak te daje uvid u kinetiku bolesti. Analize uzoraka dobivenih neinvazivnim postupcima većim su dijelom u postupku istraživanja, ali sve je više dokaza o njihovoj budućoj ulozi u dijagnostičkoj obradi bolesti kao što su astma, kronična opstrukcijska plućna bolest KOPB, primarna cilijarna diskinezija i druge bolesti pluća (6,7,8,9,10). Predviđa se da će u dijagnostičkom laboratoriju ovi uzorci u većini slučajeva zamijeniti bronhoalveolarni ispirak i dijelom inducirani iskašljaj. Unatrag nekoliko godina došlo je zapravo do prave eksplozije zanimanja za dišne testove kod plućnih bolesti, odnosno analizi sastojaka izdaha i kondenzata izdaha. U određenom smislu dišni testovi su u medicini poznati već stoljećima (npr. dijabetička koma - zadah acetona; uremijska koma - zadah po mokraći). Zahvaljujući tehnološkom razvoju i neprestanom napretku analitičkih postupaka danas je omogućena i analiza izdaha, što ima svoju primjenu u raznim područjima medicine. Izdisajni test vodika ili vodikov dišni test u upotrebi je od godine u gastroenterološkoj dijagnostici, primjerice ispitivanju nepodnošenja laktoze, određivanju vremena gastrointestinalnog tranzita (laktuloza-test), ispitivanju manjka disaharidaza, otkrivanju intestinalne malapsorpcije fruktoze te otkrivanju bakterijskog bujanja u crijevima. Tu su nadalje testovi koji uključuju peroralno ili intravensko davanje spojeva obilježenih s 13 C (prisutnost Helicobacter pylori, praćenje pražnjenja želuca), dišni testovi za ispitivanje funkcije jetre uz primjenu obilježenih lijekova, masnih kiselina ili ugljikohidrata te dišni testovi ispitivanja anorganskih hlapljivih spojeva, odnosno plinova (npr. NO, CO) u izdahu. Objavljeni su i rezultati preliminarnih ispitivanja izdaha kod jetrenih bolesnika, koja uključuju davanje spojeva sa sumporom. (11). Bolesnici sa cirozom jetre imaju, zbog promijenjenog metabolizma metionina, u izdahu povećane vrijednosti metil-merkaptana, etil-merkaptana i dimetil-sulfida. Usprkos brojnim prednostima neinvazivnih dišnih testova, isti su u svakodnevnoj kliničkoj praksi gotovo nezamijećeni ili im se ne poklanja odgovarajuća pažnja. Neinvazivni postupci dobivanja uzoraka izdaha i njihova primjena kod plućnih bolesti Neinvazivni postupci skupljanja uzoraka za dišne testove uključuju a) skupljanje izdaha i b) skupljanje kondenzata izdaha, koje će biti opširnije opisano u ovom preglednom članku. Od plinovitih sastojaka izdaha najviše je ispitano značenje promjena NO u izdahu (više od 2000 publikacija). Stvara ga nekoliko tipova stanica pluća, uključujući upalne, endotelne i epitelne stanice, a lako se određuje u izdahu. Danas je poznato da je standardizirano mjerenje frakcionalno izdahnutog NO (FE NO ) cjelovit neinvazivan način praćenja upale dišnih putova u astmi (12). Promjene serijskog mjerenja FE NO, imaju također veću prediktivnu vrijednost za dijagnosticiranje pogoršanja astme nego pojedinačna mjerenja (13, 14). Danas postoje međunarodno prihvaćene smjernice i preporuke za mjerenje izdahnutog NO kod odraslih i djece (prema European Respiratory Society i American Thoracic Society iz 1997, 1999). 3 pod posebnim uvjetima, primjerice u male djece, bolesnika na strojnoj potpomognutoj ventilaciji, detalje o mjerenju nazalnog NO te sugestije o aplikaciji za diferencijalnu dijagnozu primarne cilijarne diskinezije. U Tablici 1. navedeni su neki od biokemijskih biljega ili medijatora upale koji se ispituju u izdahu ili kondenzatu izdaha. Kondenzat izdaha S l i k a 1. NO, CO) i vodene faze koja se uglavnom sastoji od vodene pare i nekih nehlapljivih aerosolskih čestica iz donjega dijela dišnog sustava. Tekućina koja oblaže donje dišne putove sadrži preko 200 hlapljivih spojeva i razne nehlapljive spojeve (proteini, lipidi, oksidansi, nukleotidi) (16, 17), čija analiza zahtijeva hlađenje izdaha koje rezultira kondenzacijom. Tako nastaje uzorak za ispitivanje koji se naziva izdahnuti kondenzat ili kondenzat izdaha (engl. exhaled breath condensate, EBC). Još uvijek se raspravlja o podrijetlu sadržaja kondenzata i točnom mehanizmu njegova nastajanja, ali čini se da vrtloženje za vrijeme disanja igra važnu ulogu u nastanku aerosola. Naime, premda se glavnina kondenzata sastoji od vodene pare, kondenzat sadrži i aerosolske čestice iz donjega dijela dišnog sustava. Postoje, iako malobrojni, dokazi koji upućuju na to da se nehlapljivi spojevi donjega dišnog sustava transportiraju u obliku aerosola u izdah (16). Za vrijeme tidalnog disanja, broj aerosolskih čestica je između 0,1 i 4 čestice/cm 3, a srednji promjer čestica je manji od 0,3 µm (18). Broj nastalih aerosolskih čestica ovisi o trenutnoj brzini protoka zraka i površinskog tlaka izvanstanične tekućine koja oblaže donji dišni sustav. Turbulentni tok i veliki broj bridova u točkama grananja dišnih putova mogu olakšati stvaranje aerosola (19, 20, 21). Prvo ispitivanje skupljanja i analize kondenzata načinjeno je još godine u Rusiji (22, 23) hlađenjem izdaha u kolektoru domaće, kućne izrade, koji je imao mnogo nedostataka. Danas je skupljanje kondenzata brzo i jednostavno, zahvaljujući komercijalno dostupnom kolektoru kondenzata, tzv. EcoScreen tvrtke Jaeger-Toennis, Hoechberg, Njemačka, ili kolektorima drugih proizvođača. Na slici 1. prikazana je shema kolektora za skupljanje kondenzata. Bolesnik diše tidalno preko usnika kroz cijev u kojoj su odvojeni udahnuti i izdahnuti zrak. Za vrijeme izdisanja zrak prolazi kroz kondenzor koji se hladi na 0 o C, -10 o C ili 20 o C (ovisno o proizvođaču), a kondenzat se skuplja u rashlađeni spremnik za skupljanje. Za vrijeme perioda skupljanja od obično minuta dobije se oko 1-3 ml kondenzata. Niska temperatura je važna zbog labilnosti nekih biljega upale, primjerice lipidnih medijatora upale. Skupljeni kondenzat potrebno je, ovisno o biljegu koji će se odrediti, analizirati odmah, pohraniti na odgovarajućoj temperaturi (preporuka je -70 o C) ili liofilizirati. S modificiranom opremom kondenzat je moguće skupiti i kod novorođenčadi. Kontaminacija sa slinom može uzrokovati probleme, ali se može lako otkriti određivanjem katalitičke aktivnosti amilaze. Međutim, kod današnjih spomenutih i dostupnih kolektora taj problem više ne postoji. Treba također naglasiti da je tekućina u kondenzatu puno razrjeđenija nego što je ta tekućina in vivo. Stupanj razrjeđenja je još nepoznat, ali uz pomoć skupljanja pomoću mikrokapilara samo je pitanje vremena kada će nam postati poznat (20). Nadalje, kako su kondenzirajuće komore načinjene od stakla, polistirena ili polipropilena, važno je naglasiti da sastav kondenzata može biti izmijenjen ovisno o adhezijskim svojstvima materijala od kojeg je spremnik načinjen. Kada se, primjerice, planira analiza bioaktivnih lipida, trebalo bi uzeti propilenske spremnike za skupljanje. Time se izbjegava problem adsorpcije koji postoji pri uporabi polistirenskih spremnika. Minutna ventilacija ostaje glavna odrednica količine kondenzata, a nema veći utjecaj na vrijednosti izmjerenih spojeva u kondenzatu (24). Postoje također kolektori za skupljanje kondenzata u kućnim uvjetima, a kondenzat se potom dostavlja u laboratorij za analizu. Prednost im je što omogućuju da se uzorak dobiva u vrijeme akutnog pogoršanja bolesti, pa liječnik može pratiti učinak primijenjene terapije. Nedostatak je takvih kućnih kolektora što nema nadzora stručnoga medicinskog osoblja nad skupljanjem uzorka. Sadržaj kondenzata izdaha je danas područje intenzivnog istraživanja. U Tablici 2. prikazani su neki od do sada otkrivenih spojeva kondenzata izdaha, s navođenjem određenih koncentracija. U Tablici 3. navedeni su medijatori upale otkriveni u kondenzatu izdaha bolesnika raznih plućnih bolesti (17). Većina biokemijskih biljega koji se određuju u kondenzatu izdaha (kao i samom izdahu) nije nepoznata, odnosno određuje se i u induciranom sputumu ili bronhoalveolarnom ispirku. U Tablici 4. prikazani su tako biljezi upale u kondenzatu i sputumu čija je povezanost s astmom dokazana. Suprotno upalnim biljezima aciditet kondenzata može se trenutno i brzo odrediti u odraslih i djece koristeći male ph-metre (on-line ph-metri). Visoko reproducibilno mjerenje ph kondenzata nije izravna mjera aciditeta tekućine koja oblaže dišne putove. Točnije, ph predstavlja T a b l i c a 3. 5 integrirani signal koji odražava povećano stvaranje i hlapljenje kiselina s jednog ili više mjesta u dišnim putovima. Niski ph kondenzata je nađen kod bolesnika s pogoršanjima astme kao i u stabilnoj kroničnoj astmi (10, 28). Prema dosadašnjim ispitivanjima objavljenima u znanstvenoj literaturi najviše su ispitivane promjene biokemijskih biljega u kondenzatu bolesnika s astmom, KOPB, cističnom fibrozom, karcinomom pluća, a manje drugih bolesti pluća. Od biokemijskih biljega kondenzata najveći broj objavljenih rezultata odnosi se na vodikov peroksid (20, 27), određivanje ph (10, 28), nitrotirozin, nitrate/nitrite (29,30), različite eikozanoide, primjerice PGE 2, LTB 4, LTC 4, LTD 4 i dr., (31,32) te izoprostane, posebice 8-izoprostane (27, 33, 34, 35). Najveći diferencijalno dijagnostički potencijal prema literaturnim podatcima ima određivanje koncentracije 8-izoprostana, nove klase prostanoida, autooksidacijskih produkata polinezasićenih masnih kiselina ili, točnije, arahidonske kiseline. Metaboliti arahidonske kiseline su u središtu kako dijagnostičkih, tako i terapijskih ispitivanja upale dišnih putova u astmi i KOPB. Metode određivanja za većinu sastojaka kondenzata uglavnom su komercijalno dostupne, manje ili više zadovoljavajuće imunokemijske metode, dok se za neke biljege, primjerice 8- izoprostane, preporuča primjena GC-MS kao mnogo pouzdanije metode. Usprkos analitičkim problemima koji se odražavaju na apsolutne vrijednosti biokemijskih biljega u kondenzatu izdaha, relativne razlike između skupina oboljelih i zdravih ispitanika upućuju, međutim, na kliničko značenje određivanja biljega upale i drugih spojeva u ovom uzorku. Manje ispitani biljezi upale u kondenzatu izdaha su, primjerice, citokini (20), glutation i malondialdehid (19,20), proteini, neki antioksidansi i drugi spojevi. Temeljem pregleda literature može se zaključiti sljedeće: a) Za rutinsko praćenje upale dišnih putova u astmi ovog trenutka primjenjuje se određivanje NO u izdahu komercijalno dostupnim analizatorima različitih proizvođača (tzv. NIOX NO analyser- Aerocrine, Solna, Švedska, u upotrebi je u zemljama Europske unije). b) Analiza kondenzata izdaha je metoda sa značajnim prednostima za praćenje medijatora upale u odnosu na analizu bronhoalveolarnog ispirka ili sputuma. Predstavlja novi, lako izvediv i ponovljiv način dijagnostike plućnih bolesti. Za pretpostaviti je da će nakon većih spoznaja o, primjerice, izoprostanima biti primjenjiv i šire u medicinskoj dijagnostici. S obzirom na gotovo iste biljege koji se mogu naći u bronhoalveolarnom ispirku i induciranom sputumu velika je vjerojatnost da će u budućnosti zamijeniti ove uzorke. Usprkos toj privlačnosti kondenzata izdaha kao uzorka i naizgled jednostavnog postupka skupljanja kondenzata, postoje još uvijek mnogi metodološki problemi povezani s ovim neinvazivnim postupkom. Uz nedostatke navedene u Tablici 5. to su, primjerice, i nepostojanost ili neoptimiranost metoda visoke analitičke osjetljivosti i specifičnosti za pojedini biokemijski biljeg te neujednačenost načina izražavanja rezultata. Advantages and limitations of collection and analysis of exhaled breath condensate Prednosti: Advantages: * jednostavnost i ponovljivost (nema kontraindikacija) * intervencija uz krevet bolesnika * odražava promjene kao i tradicionalni uzorci (inducirani sputum i bronhoalveolarni ispirak) * ne oštećuje mukozu dišnih putova * ne postoji značajan dilucijski čimbenik kao kod bronhoalveolarnog ispirka * omogućuje longitudinalna ispitivanja, odnosno češće uzorkovanje * uzimanje uzorka kod bolesnika svih dobnih skupina * može se uzimati kod svjesnih, spontano dišućih bolesnika, novorođenčadi i bolesnika na strojnoj, potpomognutoj respiraciji * određuju se nehlapljivi spojevi povezani s plućnom patofiziologijom Nedostaci: Limitations: * metoda skupljanja uzorka nije standardizirana * nije poznato anatomski specifično mjesto podrijetla sastojaka kondenzata * nema dokaza o podrijetlu aerosolskih čestica (bronhi/ terminalni dišni putovi) * mogućnost koncentracijskog artefakta zbog evaporacije uzorka prije kondenzacije * malo je podataka o biljezima intersticijskih plućnih bolesti Ispitivanja na otklanjanju navedenih nedostataka su u tijeku. U skoroj budućnosti, uz aplikaciju novih, optimiranih i standardiziranih postupaka i analitičkih metoda, treba provesti opsežnija klinička ispitivanja kojima će se ispitati dijagnostička djelotvornost pretraga u izdahu, odnosno klinički ishod za bolesnike s plućnim bolestima. U tom smislu trebalo bi ispitati, primjerice, dnevne varijacije biljega upale u kondenzatu u zdravih BIOCHEMIA MEDICA god. 15, br. 3-4,

6 Neinvazivni postupci u dijagnostici i praćenju upale dišnih putova: kondenzat izdaha I. Čepelak i sur. i bolesnih ispitanika, korelirati vrijednosti dobivene u kondenzatu s drugim metodama procjene plućne upale (bronhoalveolarni ispirak i sputum), definirati značenje određivanja u praćenju učinka terapije i dr. Uz medijatore upale, u kondenzatu su prisutni i drugi spojevi, kao drugi proteini, lipidi, DNA, RNA iz stanica i/ili mikroorganizama te lijekovi. Kondenzat izdaha tako je područje znanstvenog interesa i za mikrobiološka i istraživanja gena. Očigledno je da su potrebna usklađena istraživanja znanstvenika iz raznih područja. Trenutno se značenje pojedinačnih biljega upale u kondenzatu procjenjuje izolirano. Stupanj promjena biljega je različit u raznim bolestima, a vrijednosti biljega mijenjaju se ovisno o dijagnostičkim i terapijskim zahvatima. Očekuje se da će ispitivanjima karakterizacije većeg broja biljega situacija biti puno jasnija i da će svaka plućna bolest imati karakterističan profil ili fingerprint raznih biljega koji mogu biti važni za postavljanje dijagnoze. To će se, dakako, odraziti i na terapijski pristup plućnim bolestima. Skupljanje i analiza kondenzata izdaha tako će biti dragocjen, jednostavan i neinvazivan način evaluacije tretmana i praćenja upale (neka vrsta inflammometra ) bolesnika s raznim plućnim bolestima, bolesnika raznih dobnih skupina i bolesnika različite težine bolesti. NONINVASIVE METHODS FOR DIAGNOSING RESPIRATORY INFLAMMATIONS: EXCHALED BREATH SYNDROME ABSTRACT - The presence and type of airway inflammation can be difficult to detect clinically and may result in delays in initiating appropriate therapy. On the other hand, direct invasive (bronchoscopy) or semi-invasive (sputum induction) methods for measurement of airway inflammation are difficult to use repeatedly in clinical practice. In the last years there has been an increasing interest in using exhaled breath as a simple, non-invasive method for collecting a sample from the lower respiratory tract in humans. Non-invasive monitoring may assist in differential diagnosis of lung disease assessment of their severity and response to treatment. There is increasing evidence that breath analysis, particularly exhaled breath condensate analysis may have an important place in diagnosis and clinical management of asthma, chronic obstructive pulmonary disease and other major lung diseases.
