1 Chronic Illness Benefit medicine list (formulary) 2017 Who we are The Malcor Medical Aid Scheme (referred to as the Scheme ), registration number 1547, is the medical scheme that you a member of. This is a non-profit organisation, registered with the Council for Medical Schemes. Discovery Health (Pty) Ltd (referred to as the administrator ) is a separate company and an authorised financial services provider (registration number 1997/013480/07). Important information you need to know We may change the information contained in this document from time to time. We will communicate any changes to the way we pay medicine when we make these changes. The Chronic Illness Benefit (CIB) is determined by the Malcor Medical Aid Scheme Rules. The use of the benefits and programmes linked with the Chronic Illness Benefit are subject to specific conditions and rules. Members must make sure that they know these rules and conditions. Any instructions or advice on medicines and the management of the member s condition are intended to add to, and not replace, the knowledge, expertise, skill and judgement of the member s doctor, pharmacist or other healthcare professional. Malcor Medical Aid Scheme reserves the right to change the benefit at any time.

2 You need to apply for cover The Chronic Illness Benefit application form needs to be completed by the member and the doctor. This fully completed form, including any supporting tests that are required (detailed in the application form), must be submitted to Malcor Medical Aid Scheme for review. Submission details are on the application form. If the condition is approved, cover only applies from the date of approval. Explanation of terms Benefit year A benefit year refers to the benefits you are entitled to during a calendar year, 1 January to 31 December. This medicine list (formulary) is applicable for 2017 only. Prescribed Minimum Benefit (PMB) conditions The Prescribed Minimum Benefit Chronic Disease List is a list of conditions which all medical schemes need to cover on all the plans they offer to their members. This cover includes funding for the diagnosis, treatment and ongoing care for the listed conditions. Only claims for procedures and consultations listed in the Prescribed Minimum Benefit (PMB) treatment guidelines will be paid from the Chronic Illness Benefit. These are prorated based on the date of approval of your chronic condition. Chronic Disease List (CDL) The Chronic Disease List (CDL) specifies medication and treatment for the 26 chronic conditions that are covered in this section of the PMBs. Medicine class This describes medicines that have similar chemical structures or similar therapeutic effects. Active ingredient This is the main substance in a medicine that is responsible for the clinical action on a human body. Scheme Rate for medicine The Maximum Medical Aid Price (MMAP ) is the rate that applies to all approved chronic medicine. In the absence of MMAP, the approved chronic medicine will be refunded to a maximum of the Single Exit Price (SEP) plus the Dis-Chem dispensing fee. All other charges in excess of these approved rates will be for your own pocket.
