2 Axis Title Background This document serves as ampton s 2015 State of the County Health Report. The State of the County Health Report is conducted every year in the interim following The Community Health Assessment. The Community Health Assessment is completed every four years. The most recent Community Health Assessment (CHA) for ampton County was completed in The Community Health Assessment was conducted by distribution of surveys throughout the county. Using the analyses from the surveys, a review of the magnitude of the problem and the feasibility for successful interventions and preventions to address the problems, the health priorities identified from the 2014 CHA are Cancer - Prostate and Breast, Tobacco and Physical and Nutrition. The rate for chronic lower respiratory disease in ampton County is 1.4% greater than the State s rate. Life style, behavior and informed decision making are contributing factors to these health issues.

The rate of Infant deaths for ampton County was higher than the State by 4.4% in 2013, higher in 2014 by 11.1% and by 4.6% from Infant mortality is the death of a baby before its first birthday. Infant mortality rates are the number of infant deaths for every 1,000 live births within a given time frame. Infant mortality is thought to be a social problem with medical consequences and is often considered a measure of the general health of a community. For lung cancer ampton s rate is lower than the State rate by 0.07% and for prostate cancer the rate is lower than the State rate by 0.06%.

6 2014 Community Health Assessment Priorities Cancer Breast/Prostate Increase the percentage of adults reporting good health by 7 %. Physical Activity and Nutrition - Increase the percentage of adults getting the recommended amount of physical activity and the recommended fruit and vegetables by 5%. Tobacco Decrease tobacco use in public /multi-unit housing by 5% Progress to Date Cancer Breast Health Breast Health education has been provided in 12 venues in ampton County. 7 Faith Based Communities 1 Community College 1 Public housing setting 2 Community Health Fairs 1 Community Breast Health Conference 185 Women have been reached Education included timely screenings, healthy life styles, and family medical history and informed decision making. Prostate Health Prostate education and awareness has been provided in one faith based setting African American Men were in attendance. Education included informed decision making including family medical history and screening. Free Annual Prostate Screening Scheduled for June 2016 Physical Activity and Nutrition Faith Communities 1 Faith community is participating in the Faithful Families Eat Smart Move More Curriculum ampton Communities/Collaborations 4 Parents participated in the Cooperative Extension Cook Smart Eat Smart Classes 3 took Prediabetes Survey. 5 Faith Communities participated in a prediabetes survey and received education on diabetes control There were 51 participants. Active Routes to School 1 Elementary School students and parents walked a half mile around parking lot 45 Adults attended the back to school Recess Event They were provided with literature on the importance of good nutrition regular physical activity. Adults also participated in physical activity games with youth.

Cancer is a chronic disease and a major cause of death and disability in. The major causes of chronic diseases are physical inactivity, unhealthy eating, tobacco use and excessive alcohol consumption.

The 2014 Behavioral Risk Surveillance Survey indicates 20.5% of residents in Eastern are smokers. 9 Increased physical activity and improved nutrition aids individuals in reaching and maintaining a healthy weight. The target for s 2020 Health objective is to increase physical activity in adults to 60.6% and to increase the percentage of adults who consume five or more servings of fruit and vegetable per day to 29.3%. The 2013 BRFSS survey indicates 69.3% of residents in Eastern have a Body Mass Index greater than 25%. This is a 1.5% increase for Eastern from The increase for the State for the same time period was 0.3%.
