2 METHOD OF PAYMENT Insurance will be filed at the time of the procedure if you have mammography coverage. If you have no mammography coverage, payment is expected at time of procedure.. *The price above does not include the Radiologist Reading fee. * If you are a candidate for 3D mammography please keep in mind that all insurances do not cover 3D mammography. You may want to check with your provider about 3D insurance coverage before you have your mammogram Guidelines. AMERICAN CANCER SOCIETY GUIDELINES Age 40 and over mammogram every other year Day of Screening Please give the Breast Health Center instructions on where to park the mobile unit. The unit is 40 feet long (about six (6) parking spaces). No power hookup is required. Please choose a level space for parking. The Schedule The schedule will begin at 8:00 a.m. If you are located more than 50 miles from Columbia, the schedule will begin at 9:00 a.m. The van will only stay as long as there are patients scheduled to be done. There is a minimum of twenty-five (25) patients required in order to have the van come to your site. If you are located outside the Columbia area, we require a minimum of thirty-two (32) patients. We can do up to 40 patients per day.

3 PALMETTO HEALTH S 3D DIGITAL MOBILE MAMMOGRAPHY UNIT 1) Accredited by the American College of Radiology. 2) Certified by FDA. 3) Staffed by two female mammographers registered by the American Registry of Radiologic Technologists in the specialty of mammography. 4) Mammograms are read by board certified radiologists from Pitts Radiological Associates. 5) Every twelve minutes a woman in the United States dies of breast cancer. 6) Breast cancer is the leading killer of women between 35 and 55. 7) One in eight (8) women will have breast cancer. 8) Mammography is the most effective tool in the diagnosis of breast cancer. 9) Total breast health care consists of: 1) Monthly breast self-examination 2) Mammograms according to the American Cancer Society 3) Yearly physical examination by a physician

4 PALMETTO HEALTH BAPTIST 3D DIGITAL MOBILE MAMMOGRAPHY Name Appointment Time Date WHAT TO EXPECT ON THE DAY OF YOUR MAMMOGRAM 1) Patient must come to the van at her designated appointment time. The appointment will take minutes. Please be prompt. 2) Upon arrival at the van, the patient will be asked several routine questions about her personal history. 3) The patient will be asked to undress from the waist up. Patients may want to wear a two-piece outfit to make undressing easier. Patients may wear deodorant as long as it does not cover the breast area. 4) No powder or lotion should be worn on your breasts that day. 5) The patient will then be taken into the exam room where routine views of the breast will be performed. 6) The patient will then enter the dressing room, re-dress, and exit the van. 7) The images will be taken to the Breast Health Center and will be read by one of our radiologists. If the radiologist needs any additional views or a breast ultrasound, the patient will be asked to come to the Breast Health Center. There is an additional charge for this follow up. 8) Patients who have implants can have the mammogram performed on the mobile unit. 9) The report of your mammogram will be sent to your physician. 10) Reminder: *practice monthly breast exams *have a yearly clinical exam by physician *obtain your mammogram on the recommended basis
