11 GENERAL INTRODUCTION Take the test is a campaign slogan of the National Health Service (NHS) Bowel Cancer screening program in the United Kingdom, promoting the general population to participate in the national colorectal cancer (CRC) screening program. In the Western world, CRC is a major health problem as it is one of the leading causes of cancer-related death [1]. Its prognosis is largely dependent on stage at diagnosis and in the absence of screening programs the morality rate is approximately 40%. Population-based CRC screening aims to detect this cancer at an early stage, or even at a precursor stage (e.g. advanced adenoma), thereby reducing CRC-related morbidity and mortality [2]. Different CRC screening methods are available, varying in invasiveness and diagnostic accuracy. Of all available screening modalities, fecal occult blood testing (FOBT), with either the guaiac fecal occult blood test (gfobt) or the fecal immunochemical test (FIT), flexible sigmoidoscopy and colonoscopy are the most commonly used [3]. Sigmoidoscopy and FOBT-screening involve a two-step method of CRC screening: only if the first test is positive, individuals are referred for colonoscopy. In primary colonoscopy screening, all individuals are invited for colonoscopy. At this time, it is still unclear which modality has the largest effect on CRC-related morbidity and mortality in the population [4]. Besides the accuracy of the screening method, the effectiveness of a screening program is highly dependent on the willingness of invitees to participate and to adhere to repeated screening [5]. Although colonoscopy is currently considered as the reference standard for diagnosing CRC and its precursor lesions, participation rates in primary colonoscopy screening programs are generally low [6]. Besides, at this moment there is not yet randomized trails evidence available on the effect of primary colonoscopy screening on mortality. Several randomized controlled trials on gfobt-based screening have shown a reduction in CRC-related mortality [7-9], and a meta-analysis on these studies demonstrated a mean 15% reduction in CRC-related mortality [10]. In the last decade, several studies have shown that the performance of the FIT is superior to that of gfobt. A FIT requires only one stool sample, leads to higher participation rates, selectively detects human globin with the use of antibodies and allows a quantitative measurement. This allows the use of different positivity thresholds, which can be adjusted to accommodate the limited colonoscopy capacity in many countries. [11-13]. A recent observational study demonstrated a 22% reduction in CRC mortality in areas where FIT-screening programs were implemented, compared to areas without screening [14]. In the Netherlands, a national biennial, single test, FIT-based CRC screening program was started in January All Dutch citizens aged years are invited every two years for FIT screening. The structure and logistics of the national program are mainly based on the results of two pilot-programs in the regions of Amsterdam and Rijnmond. Since 2006, these two pilotprograms have been inviting approximately 27,000 persons for biennial FIT. In 2014, these two cohorts were combined. Whereas several different brands of FITs are available on the market, all pilot studies in our country until 2014 have been performed with the OC-Sensor test (Eiken, Japan). For the national screening program, a European tender selected another brand: the FOB-Gold (Sentinel, Italy). Only sparse data on screening with the FOB-Gold are available, without a head-tohead comparison between FOB-Gold and OC-Sensor. At present, there is no convincing evidence for favoring a specific FIT brand [15]. 9

12 GENERAL INTRODUCTION Although FIT screening seems effective in reducing CRC-related mortality, not all CRCs will be detected, due to the relatively low sensitivity FIT for CRC and its precursor lesions. A single round colonoscopy-controlled FIT screening study reported cancer miss rate of up to 25% [16]. This means a number of screening participants will develop CRC in time despite a recent negative FIT result or a negative colonoscopy after a positive FIT [17]. These non-screen-detected CRCs are called interval cancers, the screening participants developed CRC after a recent negative screening exam and before the date of the next recommended screening. Monitoring the rate of interval cancers within a screening program is regarded as an indicator of program sensitivity. So far, a number of studies have reported on interval cancers in gfobt-based screening programs, but data on interval cancers in a FIT-based screening program with long-term follow-up are lacking. In over half of all persons undergoing colonoscopy after a positive FIT in a screening program no cancer or advanced adenomas are found [18]. As fecal blood might also come from the upper gastrointestinal (GI) tract, one could consider performing an esophagogastroduodenoscopy (EGD) to detect upper GI cancers. A review by Allard et al. concluded that there is not enough evidence to recommend for or against routine EGD in patients with a positive FOBT and negative colonoscopy based on published data [19], but this review was predominantly based on studies using gfobt or did not solely include average-risk subjects. With the ongoing implementation of CRC screening programs in various countries worldwide, an increasing number of asymptomatic, early cancers (T1 cancers, with maximum depth of invasion into the submucosa) are being detected. These cancers could be cured by appropriate endoscopic treatment if they are confined to the upper layer of the submucosa [20,21]. However, to select the optimal treatment strategy for T1 cancers, endoscopists should detect these lesions during endoscopy. Several Asian studies have demonstrated that experts in academic centers have a high accuracy in detecting T1 cancers. However, in the Western world, an accurate optical diagnosis of T1 cancers still seems very challenging [22-29]. Inadequate recognition of these lesions could urge a specialized training program in optical identification for endoscopists. The studies included in this thesis were performed to improve our understanding of the effectiveness of CRC screening in terms of participation rate, adherence, test-sensitivity and disease outcomes of persons diagnosed with CRC within a FIT-based CRC screening program, and to compare the yield of different pilot screening programs in the Netherlands. OUTLINE OF THIS THESIS Chapter one describes the willingness of invitees to participate and adhere to a biennially FIT-based CRC screening program over four rounds and identifies the determinants for consistent participation. The diagnostic yield of four rounds of FIT-based screening is compared to endoscopic screening strategies (primary colonoscopy and sigmoidoscopy) in a randomized comparison in Chapter two. Chapter three describes a randomized comparison of two different brands of FIT in a fourth round of CRC screening. We evaluate participation rate, usability, positivity rate and diagnostic yield for both tests. 10

13 GENERAL INTRODUCTION FIT screening has a relatively low sensitivity in detecting CRC, with miss rates reported up to 25% after a single screening round. In Chapter four we report the proportion of FIT interval cancers over multiple screening rounds and compare outcomes of screen-detected cancers to outcomes in non-screen-detected CRCs. As more than half of all FIT-positives have no advanced neoplasia at colonoscopy, it could be questioned whether upper GI cancer should be ruled out in these persons. In the study described in Chapter five we linked all participants of our screening cohort to the Dutch Cancer Registry to identify all proximal tumors (i.e. oral cavity, throat, esophageal, gastric and small bowel cancer) and we assessed the number of proximal cancers within two years after a FIT test in FIT-positive and FIT-negative participants. With the worldwide spread of CRC screening programs, the incidence of T1 cancers is gradually increasing. To select the optimal treatment strategy for T1 cancers (endoscopic treatment vs surgical resection), it is important that these lesions are recognized during endoscopy. In daily practice, optical diagnosis of early CRC still seems challenging. We evaluate the performance of optical diagnosis of T1 cancers by European colonoscopy experts, general gastroenterologists and gastrointestinal fellows in an image-based study, described in Chapter six. In the final Chapter seven we summarize our main findings and discuss future perspectives for colorectal cancer screening. 11
