2 Bridging Cancer Care - US 2017 Strategy and Program Focus PROGRAM GOAL Advance strategies for engaging those at highest risk for or diagnosed with lung cancer in care and support programs including interventions throughout the care continuum (from prevention to survivorship to palliative care), with the goal of improving the quality of life, health outcomes and health equity for those most disproportionately affected. 5 Care across the Continuum - Survivorship Organization Project Care Continuum The University of South Carolina College of Nursing s Establish a statewide collaboration between cancer advocates and academic institutions to test the feasibility and preliminary effects of a Mindfulness-based Stress Reduction (MBSR) intervention called Breathe Easier. Survivorship The West Virginia University Cancer Institute Establishment of a monthly, interdisciplinary survivorship care clinics; Enhances the current surveillance system for recurrence of the primary and/or secondary cancer(s); Monitors and manages physical, psychosocial effects of diagnosis, treatment, and after effects; Survivorship The Mississippi Public Health Institute The American Cancer Society Patient centered community care model that will leverage nurse navigators, social workers, and community health workers for improving development of and adherence to survivorship care plans for lung cancer survivors in the Mississippi Delta region. Develop, implement and evaluate an innovative approach to survivorship care that includes both telephone based and in-hospital lung cancer survivorship navigation; a self-management smart phone app developed in partnership with NCI, across 8 southeast states. Development of LEADS National Center for Primary Care who s goal is to establish an education and training center to improve lung cancer care across the continuum of disease. Linking lung cancer patients to case management National Minority Quality Forum Technical Assistance/training for Quality Improvement of implementation of LC in KY. LUNGevity Development of novel process/tools for follow-up engagement for sites.

7 Care across the Continuum Funded Portfolio Organization Project Care Continuum University of Kentucky, University of Louisville and the Lung Cancer Alliance; KY-LEADS Collaborative effort to promote lung cancer survivorship and improve patient outcomes through education, prevention, and survivorship care. Levine Cancer Institute A pioneer project deploying a mobile CT scanning unit into communities to deliver care regardless of where a patient resides. The American Cancer Society Partnership with three federally qualified health centers (FQHCs) to develop, implement and evaluate a lung cancer navigation program to increase lung cancer rates within primary care systems and timely access to specialists. Georgia Cancer Center at Augusta University Utilizing healthcare workers in faith-based settings to provide lung cancer education and referrals for African American populations. Disease awareness and patient education Patient Advocate Foundation Lung Cancer Alliance A program in West Virginia to increase awareness about lung cancer and facilitate access to care for Medicaid eligible lung cancer patients by linking them to case management support. Establish a national network of lung cancer specific support groups. Linking lung cancer patients to case management Enhancing psychosocial support

8 Bristol-Myers Squibb Foundation Portfolio Outcomes As of September 2017, 10 Projects in the BMSF Portfolio have: Reached approximately 110,389 vulnerable patients Developed 48 new curricula/tools in the lung cancer space Facilitated 169 partnership that have contributed to projects Three projects have resulted in permanent changes in health systems/public health policies: ACS - Links of care, process mapping, quality improvement initiatives, workflow improvements and the implementation of evidence based practices in use in FQHC s KY Leads KY health system added LC to EMR C-care - Permanent tobacco free campus implementation, at 9 of the 13 enrolled sites.
