11 Cancer Navigation is a new service, 1 year old, for the City of Winnipeg Team consists of: 4 Nurse Navigators, 1 Psychosocial Oncology Clinician, 1 Family Physician specializing in Oncology We are a virtual / mobile service Available 7 days a week via telephone Upon receiving a referral, a nurse will contact the patient within hours by telephone
12 A person is seen by a Health Care Practitioner for investigation of a health concern Tests are arranged Findings indicate a high clinical suspicion of a cancer The Winnipeg Cancer Hub receives a referral Patient is contacted within hours by a Nurse Navigator Navigation plan: keep moving forward Goal: first treatment within 60 days
13 Deliver respectful and responsive care Validate and recognize anxiety in patients and families Ongoing communications to support, connect, educate and coordinate care with patient and family Ongoing communication with primary care To identifying gaps and system barriers to care To track delays in investigations
14 As to the Navigation Program, it turned out to not only be a life saver, but it fills that dreadful gap between being given a death sentence and being given hope - that seemingly interminable time when one actually gets into the Cancer Care Stream to get a test or see a specialist. They were there to ease our fears and concerns, right away No one should have to endure the anguish of simply waiting and wondering..and waiting some more.
