1 20 PROSTATE BRACHYTHERAPY STUDY CRITERIA Patients receiving prostate brachytherapy at Cox from In this study, we will look at patients with the following prognostic features: PSA <0 Gleason score less than or equal to 6 (<7) Clinical stage less than or equal to T2a No hormone therapy (except patients receiving pre-radiation hormone for downsizing only) To examine: Median follow-up PSA results. The incidence of benign PSA spike. Time to treatment failure chemical and biopsy Late Toxicities: ) Post-radiation impotence in previously potent patients 2) Urinary: Repeated catheterizations 3 months after treatment The need for TURP for persistent obstructive symptoms Bladder perforation/fistula formation 3) Rectal: Persistent rectal bleeding requiring cauterization. Rectal perforation or fistula formation of 7

2 PROSTATE CANCER FACTS & FIGURES o Among US men, cancer of the prostate is the most common types of cancer (other than skin cancer) and the second leading cause of cancer death. o The American Cancer Society estimates new prostate cancer cases for Missouri in 20 will reach 4,230 with 240,890 cases nationwide. o Estimates for 20 prostate cancer deaths are 540 deaths in Missouri and 33,720 prostate cancer deaths nationwide. o Mortality trends for prostate cancer have been declining, which may suggest that early detection using the prostate-specific antigen test (PSA) or digital rectal exam (DRE) may be beneficial. However, the results of three large clinical trials designed to determine the efficacy of PSA testing were not in agreement. o The American Cancer Society recommends that asymptomatic men who have at least a 0-year life expectancy have an opportunity to make an informed decision with their healthcare provider about whether to be screened for prostate cancer after receiving information about the uncertainties and potential benefits associated with PSA screening. 2 of 7

3 Prostate cancer was the second most common cancer treated at our institution in 200. The number of prostate cancers treated with brachytherapy, at our institution from , is 449 patients. 00 TOTAL COX PROSTATE BRACHYTHERAPY PATIENTS YEARS Within that population we studied only patients with favorable prognostic features: PSA <0 ; Gleason score less than or equal to 6; clinical stage less than or equal to T2a; no hormone therapy except patients receiving pre-radiation hormone for downsizing only - for a total of 266 patients. Additionally, we attribute the peak in cases by 2006 to positive results achieved with brachytherapy and the subsequent decline to the advent and promotion of robotic assisted prostatectomy and cryotherapy at CoxHealth COX PROSTATE BRACHYTHERAPY PATIENTS (FAVORABLE PROGNOSTIC FACTORS) YEARS 3 of 7

4 Our stage at diagnosis is in line with the staging of other hospitals in our ACS division, factoring the group of cases identified as unknown stage. The HighPlains Division is comprised of Missouri, Hawaii, Kansas, Nebraska, Texas and Oklahoma. 4 of 7

5 ANALYSIS The median follow up was 3. years. The PSA results showed one patient had PSA increase with recurrence 4 years after treatment and was treated with hormones. Four patients had benign psa spikes. Cancer recurrence is defined as PSA-Nadir Plus 2ng/ml. PSA Results of 266 Patients 4 Acceptable PSA 26 Increased PSA w ith Recurrence Benign PSA Spike Toxicities related to treatment included three patients reporting impotence after treatment, one patient had TURP for BPH and one patient needed repeated catheterizations. Other findings included one patient reported improved sexual function, two patients were lost to follow up and 6 patients expired during the time frame from unrelated causes. Reported Toxicities After Treatment of 266 Patients 3 No Reported Toxicites Post-Radiation Impotence 26 Repeated Cath 3 Months After Treatment Surgical Intervention 5 of 7

6 SURVIVAL COMPARISONS A survival comparison for favorable brachytherapy prostate cancer cases was greater than 95% at the 5 year mark. Additional comparison of NCDB 5 year survival of stage II prostate cancers showed a higher survival rate for Cox patients than the national average. 7 CONCLUSIONS I. Prostate cancer was the second most common cancer treated at our institution in 200. The number of prostate cancers treated with brachytherapy at our institution between is 449 patients. II. II. III. IV. We attribute the peak in cases by 2006 to positive results achieved with brachytherapy and the subsequent decline to the advent and promotion of robotic assisted prostatectomy and cryotherapy at CoxHealth.
