1 The School Nurse Role In Human Papillomavirus Vaccine Series Completion Karen Schwind BSN RN NCSN TSNO Region 1 Conference March 4, 2017 OBJECTIVES The attendee will: Understand the disease prevention with new 2 dose series for HPV recently released for approval by the CDC s Advisory Committee on Immunization Practices (ACIP). Learn how evidence based data and statistical information support the need for HPV series completion and cancer prevention. Learn the importance of and review samples of tools: announcements and newsletters to communicate this information to parents for health education outreach. Learn how to utilize available programs such as Stats4Vax and the CDC immunization mobile schedule. Correlate principles of the Framework for 21 st Century School Nurse Practice The purpose of this activity is to enable the learner to use the knowledge shared to promote the Human Papillomavirus Vaccine to the students and parents in their school and community. PURPOSE 1

2 Despite the effectiveness of the vaccine, HPV immunization rates have been slow to increase, with only 42% of girls in the U.S. ages 13 to 17 having received all three doses, and only 28% of boys. ~Dr. Lauri Markowitz, CDC Epidemiologist CURRENTLY IN SCHOOL: Students present with a vaccination record without any HPV vaccine or will have completed one dose of vaccine. Students begin the series when obtaining their 7 th grade vaccines and never return to complete the last 2 vaccines. Students receiving 3 dose vaccine while 2 dose vaccine available. Lack of parental education is an important aspect of this issue. Many primary care providers mention the vaccine without the parent comprehending the importance in health and wellness and future cancer prevention. Current misconception regarding purpose and need for HPV vaccine series. HPV is short for human papillomavirus. HPV is a group of more than 150 related viruses. Each HPV virus in this large group is given a number which is called its HPV type. HPV is named for the warts (papillomas) some HPV types can cause. Some other HPV types can lead to cancer. Men and women can get cancer of mouth/ throat, and anus/rectum caused by HPV infections. Men can also get penile HPV cancer. In women, HPV infection can also cause cervical, vaginal, and vulvar HPV cancers. WHAT IS HPV? But there are vaccines that can prevent infection with the types of HPV that most commonly cause cancer. 2

3 HOW DO PEOPLE GET HPV? HPV is transmitted through intimate skin-to-skin contact. You can get HPV by having vaginal, anal, or oral sex with someone who has the virus. It is most commonly spread during vaginal or anal sex. HPV is so common that nearly all men and women get it at some point in their lives. HPV can be passed even when an infected person has no signs or symptoms. You can develop symptoms years after being infected, making it hard to know when you first became infected. In most cases, HPV goes away on its own and does not cause any health problems. But when HPV does not go away, it can cause health problems like genital warts and cancer. Genital warts usually appear as a small bump or groups of bumps in the genital area. HPV cancers include cancer of the cervix, vulva, vagina, penis, or anus. HPV infection can also cause cancer in the back of the throat, including the base of the tongue and tonsils. There are three HPV vaccines approved by the U.S. Food and Drug Administration Cervarix (HPV 2) and Gardasil (HPV 4) protect against the most common HPV types and against cervical, anal, vulvar, vaginal, penile and oropharyngeal cancers. Only Gardasil protects against the HPV types most likely to cause genital warts. These vaccines prevent about 70% of cervical cancers. Gardasil 9 (HPV 9), protects against nine types of HPV, including five that other vaccines don t cover. It also will prevent about 90% of cervical cancers. Gardisil-9 is effective for females age 9-26 and males age MD Anderson recommends that girls and boys receive the vaccine around age 11 to 12. The vaccine is given at this age so boys and girls have time to develop an immune response. HPV vaccines are given in two shots over six months. Your child must get both shots for maximum protection from cancer Screening for Cervical Cancer is completed with the Pap test. There is no routine screening for anal, penile, or oropharyngeal cancers. The HPV vaccine is a safe and effective vaccination against cancer. No research links the HPV vaccine to earlier or increased sexual activity in boys or girls. Becoming pregnant after receiving the HPV vaccine is safe for both mother and fetus. 2 DOSE VERSUS 3 DOSE THREE Three doses of HPV vaccine are recommended for people starting the vaccination series on or after the 15th birthday and for people with certain immunocompromising conditions The second dose should be given 1 2 months after the first dose, and the third dose should be given 6 months after the first dose (0, 1 2, 6 month schedule). CDC continues to recommend routine vaccination for girls and boys at age 11 or 12 years. The vaccination series can be started at age 9 years. CDC also recommends vaccination through age 26 years for females and through age 21 years for males. Males age years may be vaccinated. 3

4 Why did CDC make the recommendation change to a 2-dose schedule? Over the past year, CDC and the Advisory Committee on Immunization Practices (ACIP) have been reviewing data on 2-dose schedules, including results from studies of HPV vaccines that compared the antibody responses after 2 doses and 3 doses. These studies showed that the antibody response after 2 doses given at least 6 months apart to 9 14 year-olds was as good or better than the antibody response after 3 doses given to older adolescents and young adults, the age group in which efficacy was demonstrated in clinical trials. hcvg15-ptt-hpv-2dose.pdf COMPLETION RATES: MALE AND FEMALE ***A high national Tdap vaccination rate of 86% shows that it is possible to achieve high HPV vaccination coverage HPV STATS FOR TEXAS 4
