30 Rationale and Clinical Recommendation Statements NQF #0390 Rationale o Research studies have shown with androgen deprivation therapy in combination with EBRT: Patients can live longer and have lower risk of recurrence. Cost-effectiveness and quality-adjusted life years increase. Utilization rates have increased, but they still remain suboptimal. Clinical rationale statements updates o Recommended options for patients at very high risk: Option for EBRT plus ADT and docetaxel has been removed. Radical prostatectomy plus PLND is for younger, healthier patients with no tumor fixation to the pelvic side wall. ADT or observation is for patients not candidates for definitive therapy. 02/22/
