1 AN OPEN LABEL, SINGLE ARM, MULTICENTER, SAFETY STUDY OF ATEZOLIZUMAB IN LOCALLY ADVANCED OR METASTATIC UROTHELIAL OR NON-UROTHELIAL CARCINOMA OF THE URINARY TRACT Etude SAUL Sponsor : F. Hoffmann-La Roche Ltd CONTACTS AUX HUG : Unité de recherche en Onco-Hématologie, Fondation DFDL Coordinatrice de l étude : Mme Virginie Ancrenaz-Sirot, Investigateur responsable: Dr Samuel Blaser, Secrétariat: Mme Florence Marti-Klay, DESIGN DE L ETUDE Environ 1000 patients participeront à cette étude dans le monde, dont environ 25 en Suisse selon le schéma suivant : Déroulement de l étude Phase d examens préliminaires Phase de traitement Phase de suivi (au max. 4 semaines) (au max. 4 ans) Examens préliminaires Atezolizumab (une perfusion au jour 1, puis une perfusion toutes les 3 semaines) Visite après la fin du traitement (30 jours après la dernière administration d atézolizumab ou au début d un nouveau traitement) Phase de suivi (par téléphone tous les 3 mois) OBJECTIF DE L ETUDE L objectif de cette étude est d évaluer la sécurité de l atézolizumab et de découvrir les effets positifs et négatifs de l atézolizumab sur le cancer.
2 CRITERES D ELIGIBILITE # INCLUSION CRITERIA YES NO 1 Signed informed consent form 2 Age 18 years 3 Is, according to the investigator s judgment, able to comply with the study protocol 4 Histologically documented locally advanced (T4b, any N; or any T, N 2 3) or metastatic (M1, Stage IV) urothelial or non-urothelial carcinoma of the urinary tract Urinary tract carcinoma includes carcinoma of the bladder, ureter, urethra, and renal pelvis Primary non-urothelial carcinoma includes all sub-types listed in the WHO classification (see Appendix 8) Lymphoma, sarcoma, and renal cell carcinoma are excluded Bellini collecting duct tumours are allowed if independently double-reviewed by an expert pathologist 5 Patients with measurable and non-measurable disease according to RECIST v1.1 are allowed 6 Must have received one prior combination chemotherapy regimen (e.g., MVAC, GC, etc.) for inoperable, locally advanced or metastatic urothelial or non-urothelial carcinoma of the urinary tract Patients who received prior adjuvant/neoadjuvant chemotherapy and progressed within 12 months of treatment are allowed Patients who were intolerant to prior systemic treatment are allowed as long as they had received at least 2 treatment cycles Platinum or non-platinum doublet chemotherapy regimens are allowed 7 Representative formalin-fixed paraffin-embedded (FFPE) tumor specimen block available for submission at baseline Only tissue from core needle (with 2 cores), punch, incisional, excisional, or forceps biopsies will be accepted. Fine-needle aspiration, brushing, bone tissue, and lavage samples are not acceptable If an appropriate tumor specimen is not available, a sample will be collected by biopsy during the study screening period ONLY at sites where local laws and regulations prohibit the sending of tumor specimen blocks to an outside central laboratory, specimen slides are acceptable. A minimum of 15 unstained serially-cut sections 4 μm in thickness must be prepared and sent per tumor sample. For details, please refer to the Sample Handling Manual 8 Eastern Cooperative Oncology Group (ECOG) Performance Status 0, 1 or 2 9 Life expectancy 12 weeks Absolute neutrophil count 1500 cells/μl (without granulocyte colonystimulating factor support within 2 weeks prior to the first study treatment) White blood cell count > 2500/μL 10 Adequate hematologic and end-organ function, defined by the following laboratory results obtained within 2 weeks prior to the first study treatment: Lymphocyte count 500/μL Platelet count 100,000/μL (without transfusion within 2 weeks prior to the first study treatment) Hemoglobin 9.0 g/dl (patients may be transfused or receive erythropoietic treatment to meet this criterion) AST, ALT, and alkaline phosphatase 2.5 times the upper limit of normal (ULN), with the following exceptions: Patients with documented liver metastases: AST
3 11 12 and/or ALT 5 ULN Patients with documented liver or bone metastases: alkaline phosphatase 5 ULN Serum bilirubin 1.5 ULN. Patients with known Gilbert disease who have serum bilirubin level 3 ULN may be enrolled Calculated creatinine clearance 15 ml/min (Cockcroft-Gault formula) INR and aptt 1.5 ULN This applies only to patients who are not receiving therapeutic anticoagulation; patients receiving therapeutic anticoagulation must be on a stable dose Patients with treated, asymptomatic central nervous system (CNS) metastases are eligible, provided they meet all of the following criteria: Evaluable disease outside the CNS No metastases to midbrain, pons, medulla, cerebellum, or within 10 mm of the optic apparatus (optic nerves and chiasm) No history of intracranial or spinal cord hemorrhage No evidence of significant vasogenic edema No stereotactic radiation, whole-brain radiation or neurosurgical resection within 2 weeks before the start of study treatment Screening CNS radiographic study 2 weeks since completion of radiotherapy or surgical resection Radiographic demonstration of interim stability (i.e. no progression) between the completion of CNS-directed therapy and the screening radiographic study Note: Patients on stable doses of anticonvulsants or on prednisone doses (or dose equivalents) of 20 mg/day are allowed For women of childbearing potential: agreement to remain abstinent (refrain from heterosexual intercourse) or use contraceptive methods that result in a failure rate of 1% per year during the treatment period and for at least 5 months after the last dose of atezolizumab A woman is considered to be of childbearing potential if she is postmenarchal, has not reached a postmenopausal state ( 12 continuous months of amenorrhea with no identified cause other than menopause), and has not undergone surgical sterilization (removal of ovaries and/or uterus) Examples of contraceptive methods with a failure rate of 1% per year include bilateral tubal ligation, male sterilization, established, proper use of hormonal contraceptives that inhibit ovulation, hormone-releasing intrauterine devices, and copper intrauterine devices The reliability of sexual abstinence should be evaluated in relation to the duration of the clinical trial and the preferred and usual lifestyle of the patient. Periodic abstinence (e.g., calendar, ovulation, symptothermal, or postovulation methods) and withdrawal are not acceptable methods of contraception
4 # EXCLUSION CRITERIA YES NO 1 Treatment with more than one prior line of systemic therapy for inoperable, locally advanced or metastatic urothelial or non-urothelial carcinoma of the urinary tract Low dose chemotherapy given as radiosensitiser for curative treatment of localized disease does not count as a line of systemic chemotherapy and does not constitute grounds for exclusion 2 Treatment with any other investigational agent or participation in another clinical trial with therapeutic intent within 4 weeks prior to study treatment initiation 3 Treatment with chemotherapy within 2 weeks prior to study treatment initiation 4 Treatment with radiotherapy ongoing at the time of study entry (for CNS-directed radiotherapy, please refer to Inclusion Criterion 11) 5 Pregnant or lactating, or intending to become pregnant during the study Women who are not postmenopausal (postmenopausal defined as 12 months of non-therapy-induced amenorrhea) or surgically sterile must have a negative serum pregnancy test result within 2 weeks prior to initiation of study drug 6 Evidence of significant uncontrolled concomitant disease that could affect compliance with the protocol, including significant liver disease (such as cirrhosis, uncontrolled major seizure disorder, or superior vena cava syndrome) 7 Significant cardiovascular disease, such as New York Heart Association cardiac disease Class III, myocardial infarction within 3 months, unstable arrhythmias, or unstable angina Patients with known coronary artery disease or left ventricular ejection fraction < 50% must be on a stable medical regimen that is optimized in the opinion of the treating physician, in consultation with a cardiologist if appropriate 8 Significant renal disorder requiring dialysis or indication for renal transplant 9 Signs or symptoms of severe infection within 2 weeks prior to initiation of study treatment, including but not limited to, hospitalization for complications of infection, bacteremia, or severe pneumonia 10 Major surgical procedure within 4 weeks prior to study treatment initiation or anticipation of need for a major surgical procedure during the course of the study other than for diagnosis 11 History of severe allergic, anaphylactic, or other hypersensitivity reactions to chimeric or humanized antibodies or fusion proteins 12 Known hypersensitivity or allergy to biopharmaceuticals produced in Chinese hamster ovary cells or any component of the atezolizumab formulation 13 History of autoimmune disease (Appendix 5) are allowed if controlled and on stable treatment (i.e., same treatment, same dose) for the last 12 weeks, with the exception of: Patients taking concurrent abatacept or belatacept treatment, unless therapy has been withdrawn for > 8 weeks Patients with a history of serious or life threatening immune-related events No more than 1 concomitant autoimmune disease at the time of study entry is allowed unless one of them is: Autoimmune-mediated hypothyroidism on a stable dose of thyroid replacement hormone Controlled Type I diabetes mellitus on a stable dose of insulin regimen A medical history of such entities as atopic disease or childhood arthralgias, where the clinical suspicion of autoimmune disease is low. In addition, transient autoimmune manifestations of an acute infectious disease that resolved upon treatment of the infectious agent are not excluded (e.g., acute Lyme arthritis) 14 Prior allogeneic stem cell or solid organ transplantation 15 History of idiopathic pulmonary fibrosis (including pneumonitis, drug-induced pneumonitis, organizing pneumonia (i.e. bronchiolitis obliterans, cryptogenic organizing pneumonia), or evidence of active pneumonitis on screening chest computed tomography (CT) scan History of radiation pneumonitis in the radiation field (fibrosis) is permitted 16 Patients with active hepatitis B (defined as having a positive hepatitis B surface antigen [HBsAg] test at screening) or hepatitis C Patients with past hepatitis B virus (HBV) infection or resolved HBV infection (defined as having a negative HBsAg test and a positive antibody to hepatitis B
5 core antigen [anti-hbc] antibody test) are eligible Patients positive for hepatitis C virus (HCV) antibody are eligible only if polymerase chain reaction (PCR) is negative for HCV RNA 17 Active tuberculosis Administration of a live, attenuated vaccine within 4 weeks prior to study treatment initiation Influenza vaccination should be given during influenza season only (e.g., approximately October to March in the Northern Hemisphere) Patients must not receive live, attenuated influenza vaccine (e.g., FluMist ) within 4 weeks prior to study treatment initiation or at any time during the study treatment, and so until 5 months thereafter Prior treatment with CD137 agonists or immune checkpoint blockade therapies, including anti CTLA-4, anti PD-1, and anti PD-L1 therapeutic antibodies Patients who have received prior treatment with anti-ctla-4 may be enrolled provided at least 5 half-lives (approximately 75 days) have elapsed from the last dose of anti CTLA-4 to the first dose of atezolizumab and there was no history of severe immune-mediated adverse effects from anti-ctla-4 (NCI CTCAE Grade 3 or 4) Treatment with systemic immunostimulatory agents (including, but not limited to, interferons or interleukin-2) within 4 weeks or five half-lives of the drug, whichever is shorter, prior to initiation of study treatment Prior cancer vaccines and cellular immunotherapy are permitted Specifically for patients without autoimmune disease: treatment with systemic corticosteroids or other systemic immunosuppressive medications (including, but not limited to, prednisone, dexamethasone, cyclophosphamide, azathioprine, methotrexate, thalidomide, and anti-tumor necrosis factor agents) within 2 weeks prior to study treatment initiation or anticipated requirement for systemic immunosuppressive medications during the study treatment period For patients with CNS metastases, use of prednisone at a dose (or dose equivalent) of 20 mg/day is acceptable Chronic use of prednisone or equivalent should be discussed with the Medical Monitor The use of inhaled corticosteroids for chronic obstructive pulmonary disease, mineralocorticoids (e.g., fludrocortisone) for patients with orthostatic hypotension, and lowdose supplemental corticosteroids for adrenocortical insufficiency and topical steroids for cutaneous diseases are allowed. Note: For patients with autoimmune disease, immunosuppressive medications are permitted if the patient has controlled autoimmune disease and stable treatment (i.e., same treatment, same dose) for the previous 12 weeks.
