1 ASTRO's 54th Annual Meeting CE Tracking Form This form is for your personal use only and is not the CE evaluation. Therefore, it cannot be submitted to earn credit. To earn credit, you must complete an online evalaution. Directions on how to access the evaluation by credit type are included below: CME: All registed full conference and full day physician attendees are eligible to earn CME credit. To earn CME credit you will need to complete the online meeting evaluation. The evaluation can be completed on site at the CE Central access stations (North Lobby, Level 1) or online from your hotel, home, or office up to 60 days post meeting using the following link: Please note that you will need to enter your badge number in order to complete the online evaluation, so please make note of this number if you plan on completing the evaluation postmeeting. Certificates of completion will be available on the ASTRO website 60 days post meeting. CAMPEP: All registed full conference and full day physicist attendees are eligible to earn CAMPEP credit. All completed evaluations will be submitted to ONS within 60 days of the conclusion of the meeting. All registered full conference and full day physician and physicist attendees can earn SAM credit by pre registering ($35 per session) for one of the five Live SAM sessions. To earn credit, participants will need to answer all 13 of the required assessment and evlauation questions. Certificates of completion will be available on the ASTRO website 30 days post meeting.
