POLICY STATEMENT: Highmark Health Options provides coverage under the medical or pharmacy benefits of the Company s Medicaid products for medically necessary Opdivo (nivolumab) intravenous injection. This policy is designed to address medical necessity guidelines that are appropriate for the majority of individuals with a particular disease, illness or condition. Each person s unique clinical circumstances warrants individual consideration, based upon review of applicable medical records. The qualifications of the policy will meet the standards of the National Committee for Quality Assurance (NCQA) and the Delaware Department of Health and Social Services (DHSS) and all applicable state and federal regulations. DEFINITIONS: Medical Necessity: A service or benefit is medically necessary if it is compensable under the MA program and if it meets any one of the following standards: The service or benefit will, or is reasonably expected to prevent the onset of an illness, condition, or disability The service or benefit will, or is reasonably expected to, reduce or ameliorate the physical, mental, or developmental effects of an illness, condition, injury, or disability The service or benefit will assist the member to achieve or maintain maximum functional capacity in performing daily activities, taking into account both the functional capacity of the member and those functional capacities that are appropriate for members of the same age Prior Authorization: A medication benefit that is approved once established criteria are met. The criteria are medication specific and may be based on FDA and manufacturer guidelines, medical literature, safety, appropriate use, and benefit design. A prior authorization may be established to address appropriate utilization due to patient safety concerns, limited indications, and potential for misuse/abuse.

2 PROCEDURES: Opdivo is considered medically necessary for the treatment of when the member and prescriber meet the following criteria. Opdivo Prior Authorization Criteria: Benefit coverage is provided for the following conditions: Coverage may be provided when the diagnosis is unresectable or metastatic melanoma, AND o The patient is age 18 years or older, AND o Treatment is prescribed by an oncologist/hematologist, AND o The patient has Stage III (unresectable) or IV (metastatic) disease, AND o Treatment will be used for BRAF V600 mutation positive or wild-type, as a single agent, OR o Treatment will be used in combination with Yervoy (ipilimumab), AND o The dosing is within the following prescribing-supported parameters: Monotherapy: Does not exceed 240 mg every 2 weeks, OR Opdivo in combination with Yervoy: Does not exceed 1 mg/kg (Opdivo), followed by 3 mg/kg (Yervoy) on the same day, every 3 weeks for 4 doses, then 240 mg (Opdivo) every 2 weeks Coverage may be provided when the diagnosis is metastatic non-small cell lung cancer, AND o The patient is age 18 years or older, AND o Treatment is prescribed by an oncologist/hematologist, AND o Treatment will be used for progression on or after platinum-based chemotherapy, AND Patients with EGFR or ALK genomic tumor aberrations should have disease progression on FDA-approved therapy for these aberrations prior to receiving Opdivo o The dosing is within the following prescribing-supported parameters: Does not exceed 240 mg every 2 weeks Coverage may be provided when the diagnosis is renal cell carcinoma, AND o The patient is age 18 years or older, AND o Treatment is prescribed by an oncologist/hematologist, AND o Treatment will be used for advanced disease, AND o Treatment will be used in patients who have received prior anti-angiogenic, AND o The dosing is within the following prescribing-supported parameters: Does not exceed 240 mg every 2 weeks Coverage may be provided when the diagnosis is classical Hodgkin Lymphoma, AND o The patient is age 18 years or older, AND o Treatment is prescribed by an oncologist/hematologist, AND o The patient has relapsed or has progressed disease after autologous hematopoietic stem cell transplant AND post-transplant Adcetris (brentuximab vedotin), AND o The dosing is within the following prescribing-supported parameters: Does not exceed 3mg/kg every 2 weeks

3 Coverage may be provided when the diagnosis is recurrent or metastatic squamous cell carcinoma of the head and neck, AND o The patient is age 18 years or older, AND o Treatment is prescribed by an oncologist/hematologist, AND o Treatment will be used for patients with disease progression on or after a platinumbased therapy, AND o The dosing is within the following prescribing-supported parameters: Does not exceed 3mg/kg every 2 weeks 1. Contraindications None 2. When Opdivo is Not Covered Coverage may be provided for any non-fda labeled indication or a medically accepted indication that is supported by nationally recognized pharmacy compendia or peer-reviewed medical literature for treatment of the diagnosis (es) for which it is prescribed and will be reviewed on a case by case basis to determine medical necessity. 3. Post-Payment Audit Statement The medical record must include documentation that reflects the medical necessity criteria and is subject to audit by Highmark Health Options at any time pursuant to the terms of your provider agreement. 4. Place of Service The site of service for the administration of Opdivo is outpatient centers. 5. Length of Coverage Initial approval duration will be for 12 weeks Reauthorization will be for 6 months upon receiving documentation that the patient is tolerating and responding to treatment and the patient s liver, thyroid, and kidneys are monitored throughout treatment. Adverse events to treatment should be monitored (e.g. immune-related reactions) and dose modifications made as necessary. Governing Bodies Approval On December 22, 2014 Opdivo was approved for the treatment of patients with unresectable or metastatic melanoma and disease progression following ipilimumab and, if BRAF V600 mutation positive, a BRAF inhibitor. This indication was approved under the accelerated approval base on tumor response rate and durability of response. On March 4, 2015 the FDA granted approved for Opdivo for the treatment of patients with metastatic squamous non-small cell lung cancer with progression on or after platinum-based chemotherapy. On November 23, 2015, the FDA granted approval of Opdivo for the treatment of patients with advanced (metastatic) renal cell carcinoma who have received a prior therapy. On May 16, 2016, the FDA granted accelerated approval to nivolumab (Opdivo) for the treatment of patients with classical Hodgkin lymphoma (chl) that has relapsed or progressed after autologous hematopoietic stem cell transplantation (HSCT) and post-transplantation Brentuximab Vedotin (Adcetris).
