Box , Nairobi. oedwin6@yahoo.com It is now widely recognized that breast cancer is indeed a heterogeneous disease. About two thirds of breast cancer patients present with early, node negative disease in the West. In this part of the world it is increasingly being asked which specific group in this cohort would truly benefit from adjuvant radio-chemo-hormonal therapy. New techniques in the last decade have been developed and adapted to identify sub groups of breast cancer with prognostic implications. Gene expression profiling (GEP) using oligonucleotide and cdna microarrays are now an accepted part of the armamentum that is dissecting breast cancer into subgroups that can be targeted therapeutically. Two main subgroups that have been researched upon extensively in the West despite their rarity there are basallike breast cancer (BLBC) and triple negative breast cancer (TNBC). These two are known to be over expressed in the African American, represent a very aggressive type of breast cancer and does typically occur in the younger age bracket which are features characteristic of breast cancer seen in the African Sub continent.
