2 INTRODUCED ICD-10 As a part of mandatory requirement by the World Health Organization (WHO) in the near future, the system is enhanced for users to apply ICD-10 codes in both the PM and the EMR modules. The users may gradually want to migrate to ICD-10 from ICD-9. It may happen that the users may not be completely aware of the ICD-10 codes assigned to diagnose the problem. Considering this fact, the system facilitates the users to retrieve the ICD-10 code also by searching for the ICD-9 code. In other words, a user can also type in the ICD-9 code or description (if unaware of ICD-10 codes) and a list of ICD-10 codes will be populated to record an appropriate ICD-10. CHANGES IN CHARTING This section demonstrates the ICD-10 feature implemented in Charting. CONFIGURING ICD-10 AT PRACTICE LEVEL In the Administration section, you can configure the coding standard either as ICD-9 or ICD-9/ICD-10. If you want to specifically use only ICD-9 illness codes, you are recommended to configure ICD-9 option. If you want to use ICD-10 illness codes, then you should configure ICD-9/ICD-10 option. Configuring ICD-9/ICD-10 option enables you to search the ICD-10 codes also by typing an ICD-9 code or description in the Illness field. To configure ICD-10 at practice level: From the left menu items, select Administration > Practice. Click the Practice Defaults link. From the Practice Default Settings panel, select the ICD-9/ICD-10 option in the Coding Standard field. Click Save. Note: By default, the coding standard is configured as ICD-9. 3 Figure 1: Configuring ICD-10 at Practice Level ICD-10 MASTER LIST By default, the system will not contain any record for an ICD-10 illness code in the master list. As and when you record an illness in the system or add the illnesses in the Practice Favorite List, the ICD-10 Master List will be updated accordingly. To view ICD-10 Master List: From the left menu items, select Administration > Setup > ICD-10. On the Diagnosis Codes screen, the ICD-10 Master List option is selected by default. Note: By default, the ICD-10 Master List will be blank. When the illnesses are recorded in the system or added in the Practice Favorite List, the ICD-10 Master List will be updated accordingly. 4 Figure 2: Viewing ICD-10 Master List PRACTICE FAVORITE LIST You may want to use ICD-10 codes in your Practice Favorite List. Considering a possibility that you may not be aware of the exact ICD-10 code, the system facilitates you to type in the ICD-9 code or description that will populate the list of related ICD-10 codes. You can select an appropriate ICD-10 code to add it in your Practice Favorite List. If the modifiers are associated with the selected ICD-10 code, a pop-up screen will be displayed to enable you to drill down your search to select the specific illness. To add ICD-10 in your Practice Favorite List: From the left menu items, select Administration > Setup > ICD-10. On the Diagnosis Codes screen, select the Practice Favorite List option. In the Illness field, type in an ICD-10 code or description. If you are not aware of the ICD-10 code, you can also type in an ICD-9 code or description. A list of ICD-9/ICD-10 will be populated. Select an appropriate ICD-10 code from the list. Click the Move selected Illness to Practice favorite list button. Note: Once you add an ICD-10 code to the Practice Favorite List, it will be automatically added to the ICD- 10 Master List. Call for customer service or us at insyncsupport@mdol.com. Page 4 of 14
5 Figure 3: Adding ICD-10 to Practice Favorite List RECORDING ICD-10 CODES IN THE DIAGNOSIS SECTION On the Diagnosis screen, there are several ways to record illnesses. You can select illnesses from either Physician Favorite list, Frequently Used list, Previous Encounter(s), Problem List, or Category list. When you migrate to ICD- 10, by default, the Physician Favorite list, Frequently Used list, and Category list will be empty. As and when the illnesses are recorded, the ICD-10 codes will be added to these sections. Only the Previous Encounter(s) and Problem List sections will have the ICD-9 codes, if they are entered before migrating to ICD-10. In order to use the ICD-9 codes of these sections, you can select any ICD-9 code from any of these sections and an appropriate ICD-10 code will be recorded in the system. If the illness that you want to record is not available in any of these 5 sections, you can type in the ICD-10 code or description in the Illness field. If you are not aware of an ICD-10 code, you can also type in the ICD-9 code or description. This will populate the list of related ICD-10 codes. To record ICD-10 codes in the Diagnosis section: On the top menu bar, click Patient Search. To record the illness for a patient, click Facesheet. This will populate the list of related ICD-10 codes. You can select an appropriate ICD-10 code to record the illness. When the modifiers are associated with the selected ICD-10 code, a pop-up screen will be displayed to enable you to drill down your search to select the specific problem. To record ICD-10 Codes in the Problem List section: On the top menu bar, click Patient Search. To record the illness for a patient, click Facesheet. Figure 5: Recording ICD-10 Codes in the Problem List Section ABILITY TO INACTIVATE ICD-10 Only a practice administrator is authorized to inactivate ICD-10 codes from the configuration. To inactivate ICD-10: Log in as a practice admin user. From the left menu items, select Administration > Setup > ICD-10. On the Diagnosis Codes screen, select the ICD-10 Master List option. 8 Figure 6: Inactivating ICD-10 REFINING THE SEARCH FOR ICD-10 CODES Certain ICD-10 codes (for example, Fracture acetabulum-closed ) that have modifiers associated with them. When the modifiers are associated with the ICD-10 code, the system pops up a screen to enable the user to drill down the search to select the specific problem. The following screen illustrates how you can refine your search to record the specific problem. You can try different combinations from these columns: Encounter type, Fracture healing, Fracture type, and Laterality; the succeeding list of ICD-10 codes will be modified accordingly to enable you to search the specific problem. 9 Figure 7: Refining the Search for ICD-10 Codes CHANGES IN BILLING This section demonstrates the ICD-10 feature implemented in Billing. CONFIGURING ICD-10 AT INSURANCE LEVEL There is a possibility that not all the insurance companies to whom you transmit the claims through the clearinghouse use the ICD-10. Hence, you may want to use different coding standards (ICD-9 or ICD-10) for different insurances. In order to use ICD-10 for a specific insurance, you also need to configure ICD-10 at the insurance level after you configure ICD-10 at the practice level. To configure ICD-10 at insurance level: From the left menu items, select Administration > Insurance. Click the icon next to an insurance for which the ICD-10 is to be configured. In the Coding Standard field, select the ICD-9/ICD-10 option. Click Save Insurance. Call for customer service or us at insyncsupport@mdol.com. Page 9 of 14
10 Figure 8: Configuring ICD-10 at Insurance Level CREATING A NEW CHARGE WITH ICD-10 Once you configure ICD-10 at the practice and insurance levels, you can create a new charge with ICD-10 codes. If you have configured ICD-10 at the insurance level, the system will select the ICD-10 option by default when creating a new charge; however, you can toggle between ICD-9 and ICD-10 options on the New Charge screen. To create a new charge with ICD-10: On the top menu bar, click New Charge. Select a patient and enter other mandatory information. In the Line Item Details section, locate the options for ICD-9 and ICD-10. If ICD-10 is configured at the insurance level, by default, the system will select the ICD-10 option whenever you are creating a new charge. In the Diagnosis Code section, type in the ICD-10 code or description. If you are not aware of an ICD-10 code, you can also type in the ICD-9 code or description. 11 Figure 9: Creating New Charge With ICD-10 CONFIGURING ICD-10 IN SUPERBILL When the ICD-10 is configured at the practice level, the Superbill configuration section will allow you to configure ICD-10 alongside the ICD-9 codes. With this configuration, the users can use either the ICD-10 or the ICD-9 codes on any given visit. To configure ICD-10 in Superbill: From the left menu items, select Billing > Manage Superbills. On the Superbill Manager screen, click the Edit icon. Click the ICD-10 tab. In the Search field, type in the ICD-10 code or description. If you are not aware of an ICD-10 code, you can also type in the ICD-9 code or description. A list of ICD-10 codes will be populated to select from. 12 Figure 10: Configuring ICD-10 in Superbill CREATING A CHARGE THROUGH SUPERBILL Once the ICD-10 is configured at the practice and insurance levels and a charge is created using Superbill, the Superbill screen will allow you to select ICD-9 or ICD-10 codes to create a charge. When you proceed further for creating a charge using Superbill, the Superbill Integration screen will display both the ICD-9 and the ICD-10 codes. The ICD-10 codes will be displayed with bold fonts to separate them with ICD-9 codes. To create a charge from Superbill: On the top menu bar, click Scheduler. Click the Superbill icon. On the Superbill screen, scroll down and select the required ICD-9 or ICD-10 codes. Note: You cannot select both ICD-9 and ICD-10 codes at one time. Click Save. Click Go To Charge Capture. On the Integration screen, locate the ICD-9 and ICD-10 codes. Select ICD-10 code(s) and click Import service(s). Call for customer service or us at insyncsupport@mdol.com. Page 12 of 14
