1 Treatment of Guillain-Barre syndrome: a cost-effectiveness analysis Nagpal S, Benstead T, Shumak K, Rock G, Brown M, Anderson D R Record Status This is a critical abstract of an economic evaluation that meets the criteria for inclusion on NHS EED. Each abstract contains a brief summary of the methods, the results and conclusions followed by a detailed critical assessment on the reliability of the study and the conclusions drawn. Health technology Plasma exchange or intravenous immune globulin (IV IgG) for treatment of acute Guillain-Barre syndrome. Type of intervention Treatment. Economic study type Cost-effectiveness analysis. Study population The study population consisted of patients diagnosed with acute Guillain-Barre who were unable to walk without assistance and who presented within two weeks of the onset of symptoms. Setting The setting was secondary care. No specific setting was outlined for the effectiveness data. The cost data were based on the Queen Elizabeth II Health Science Centre, Halifax, Nova Scotia, Canada. Source of effectiveness data The effectiveness data were based on a review of the literature. Modelling A decision model was used to estimate the costs of the treatments. Outcomes assessed in the review The review aimed to compare effectiveness in terms of improvement in disability grade of patients treated for Guillain- Barre syndrome with plasma exchange or IV IgG. Study designs and other criteria for inclusion in the review The review included only randomised-controlled trials which directly compared plasma exchange with IV IgG for the treatment of acute Guillain-Barre syndrome. 2 Sources searched to identify primary studies MEDLINE and the bibliographies of the retrieved studies were searched. No dates for the search or for the studies retrieved were specified. Criteria used to ensure the validity of primary studies To be included studies had to: be based on random allocation to an intervention group; report objective outcome measures evaluated by investigators blinded to the treatment type; report follow-up of at least 95% of patients and of at least 90 days duration; and provide a clear description of the intervention. Methods used to judge relevance and validity, and for extracting data A single reviewer assessed the titles and abstracts of all studies retrieved. Number of primary studies included Two primary studies were included in the review. Methods of combining primary studies The narrative method was used. Investigation of differences between primary studies The authors investigated the differences between the primary studies but did not investigate their effects on the estimates of effectiveness. No tests of homogeneity were performed. Results of the review The primary outcome of the reviewed studies was the improvement in disability grades. The first study reported significantly more IV IgG patients achieving the primary outcome at 4 weeks, but this difference was not statistically significant at 3 months. The study also reported that patients in the plasma exchange group experienced more treatmentrelated complications. The second, larger study reported no significant differences at 4 weeks or 48 weeks of follow-up. The authors concluded that there was insufficient evidence to support the contention that one treatment was more effective or safer than the other was and therefore made an assumption of equal effectiveness. Measure of benefits used in the economic analysis This was not applicable, given the cost-minimisation nature of the study. Direct costs The quantities and unit costs for the treatment of a cohort of 1,000 patients were incorporated into a decision tree model. The discounting of cost was not relevant as the costs were evaluated over a period of less than one year (48 weeks). Quantities and costs were analysed separately. The quantities and costs of supplies (IV IgG, replacement fluids, insertion supplies, etc.), professional costs (nurses' time and cost, physicians' reimbursement claims), overhead and hotel costs (costs common for both treatments were excluded, plasma exchange unit costs were included based on the unit area), capital costs (plasma exchange machines), imputed costs (rent of the plasma exchange unit), were included in the analysis. The estimates of quantities were based on protocols of treatment (from the effectiveness studies) and three Page: 2 / 5

3 experienced nurses (quantities of supplies, nursing time). Data for prices of IV IgG, 5% albumin and pentastarch were obtained from the manufacturers (USA). Other unit cost data were obtained from the Queen Elizabeth II Health Centre. The type of treatment received by the patient was based on the decision tree model. All costs were reported in 1997 US dollars. Statistical analysis of costs No statistical analysis of costs was performed due to the modelling approach adopted. Indirect Costs Indirect costs were not included. Currency US dollars ($). No rate was reported for conversion from Canadian to US dollars. Sensitivity analysis One-way sensitivity analyses were performed for the cost of replacement fluid in plasma exchange, nurses' time for administering treatments, rent, discounting rate of equipment and the cost of IV IgG. Estimated benefits used in the economic analysis This was not applicable as the analysis of benefits was based on the therapeutic equivalence of the treatment alternatives. Cost results The total cost for treatment with IV IgG was $10,165 per patient, 98% of which was the cost of IV IgG. The total cost for a 5-day treatment with plasma exchange was $6,024 most of which was the cost of replacement fluid. The results were shown to be sensitive to the costs of IV IgG and replacement fluid. The average cost saving was almost $4,000 per patient using plasma exchange. Synthesis of costs and benefits This was not applicable as the economic analysis was conducted on a cost-minimisation basis. Authors' conclusions The authors concluded that, although both plasma exchange and IV IgG are effective treatments for acute Guillain- Barre syndrome, the use of plasma generates cost savings to the health care system. CRD COMMENTARY - Selection of comparators No explicit justification was given for the treatments compared. The treatments represented the practice for management of acute Guillain-Barre syndrome, current at the time of the study. The combined treatment was not considered even though it was analysed in the main effectiveness study reviewed. You, as a user of this database, should consider the relevance of these alternatives to your own setting. Validity of estimate of measure of effectiveness A systematic review with clear selection criteria was carried out in order to identify available, good quality effectiveness evidence. The methods and conduct of the review were satisfactorily reported. However, only two studies were identified, and their conclusions differed with respect to treatment-related side effects and outcomes at 4 weeks of Page: 3 / 5

4 follow-up. The authors also used data from one of the retrieved studies selectively to inform their conclusions and the parameters of the decision model. The validity of the effectiveness measures would be improved if supported by additional evidence. Validity of estimate of measure of benefit The analysis of benefits was based on the therapeutic equivalence of the treatment alternatives. The economic analysis therefore included only costs. The analysis of patients' quality of life was considered to be an important research question, but may be informative in future studies of this nature. Validity of estimate of costs Only protocol driven costs were included in the analysis. The costs of managing the side-effects of the two therapies were not analysed, but would be relevant. Costs considered to be common to both treatments were (justifiably) excluded from the analysis. Within these limitations, all relevant costs seem to have been taken into account from the perspective of the health services. Quantities and costs were reported separately. Resource use quantities were taken from treatment protocols and the expert opinion of experienced nurses. One-way sensitivity analyses were performed over appropriate ranges. Unit costs were taken from the authors' setting and from the manufacturers. Physicians' charges were used to approximate physicians' costs, which tends to limit the generalisability of the results. Other issues The study did not differentiate between different patient groups (young adults, elderly), and the question of generalisability was not addressed. The cost finding might be driven by the specificity of the setting and adjustments would be necessary before the finding could be generalised to a different setting. Although a combined treatment was suggested by one of the effectiveness studies, no cost analysis of such an alternative was performed. Implications of the study The analysis concluded that the plasma exchange therapy is less costly compared with IV IgG for treatment of acute Guillain-Barre syndrome. Further research to determine the patients' quality of life and its impact on this economic analysis needs to be performed. Source of funding None stated.
