6 Wound healing a three-phase process Physiological wound healing is a complex and dynamic process which always runs the same course, regardless of the type of wound involved. This process typically includes three mutually dependent phases, with smooth, continuous transitions in between. 1. Exudation phase The exudation or cleansing phase begins immediately after the injury. The clinical picture shows a local inflammation, which is followed by high amounts of exudate. This phase involves blood coagulation, wound cleansing, and the body s natural defence against infection. Pathogens and destroyed tissues are removed through the process of phagocytosis. In the exudation phase, moist wound management promotes and enhances the natural wound cleansing process in the best way possible, thus creating a clean wound environment. If the wound has coatings or necrotic tissues, they are softened and removed. Exudate, cell debris and pathogens are absorbed by the dressing. A perfect wound dressing keeps the wound moist and protects it against infection the ideal basis for quick and efficient healing. Dressing change intervals are determined by the condition of the wound and the amount of exudate. Thus, dressings may have to be changed several times a day. If necessary, necrotic tissues and coatings will also be removed in the process. 1 EXUDATION Days

7 2. Granulation phase In the granulation or proliferation phase, new cells are formed. The wound space is gradually filled in with fresh granulation tissue. There is an increased migration of fibroblasts into the wound, and collagen synthesis sets in. Capillaries grow towards the centre of the wound, while exudation decreases. The clinical picture now shows a reddish tissue with good blood perfusion. In the granulation phase, a moist wound dressing keeps the wound from drying out, while efficiently absorbing any excess exudate. It provides safety from infection and mechanical damage and protects the fresh tissue. In this phase as well, it is the best possible wound management modality in terms of an ideal moist wound environment. The individual condition of the wound determines how often the dressing has to be changed. As a general rule, dressing change intervals tend to increase over the course of wound treatment. A distinct advantage is the fact that a moist wound dressing facilitates an atraumatic change of dressings, which means less pain for the patient and better healing. 3. Epithelization phase The epithelization or regeneration phase is characterized by a progression in tissue differentiation and epithelization. The wound edges move in towards each other; the wound is gradually covered by epithelial tissue growing inwards from the edge. Wound contraction and scar formation completes the process of wound healing. In this phase, moist wound management promotes the final stage of healing and the formation of fresh epithelial tissue. The dressing maintains an optimum level of moisture and provides protection from outside influences to keep the wound undisturbed. The individual condition of the wound determines how often the moist wound dressing needs to be changed. As exudation keeps decreasing, the dressing may remain in place for up to 7 days, which is a benefit to users as well as patients. 2 GRANULATION 3 EPITHELIZATION

8 Suprasorb professional care for each wound Wound with necrotic tissue Wound with fibrinous coatings Epithelizing wound Granulating wound In an effort to facilitate the phase-oriented use of individual wound dressings, wounds are classified into four stages on the basis of their visual appearance, as follows: wounds with necrotic tissue / wounds with fibrinous coatings / granulating wounds / epithelizing wounds. For practical purposes, these four stages are commonly identified by colour codes black, yellow, red, and pink, respectively. The amount of exudate in a wound (light, moderate, or high) serves as another criterion for choosing the appropriate wound dressing. Furthermore, the possibility of an infection must always be taken into account. A clinical infection is characterized by the classic signs of inflammation (swelling, redness, heat, pain, and possibly impaired function), plus such signs of infection, as wound odour and suppuration. These must be distinguished from colonization and contamination of the skin or the wound. Colonization occurs when pathogens invade the skin or wound, while contamination refers to a colonization of the wound with pathogens which does not result in a host reaction. 8

10 PHMB antimicrobially active and well tolerated Polyhexamethylene biguanide (or PHMB for short) is an active substance with a broad antimicrobial spectrum, a favourable toxicological profile, and a low allergenic potential. PHMB has long been used in a wide variety of everyday products, for instance as a preservative in cosmetics and solutions for contact lenses, or in the manufacture of surface disinfectants. Its positive effects on wound healing make PHMB the agent of choice in the treatment of poorly healing chronic wounds. 1-3 PHMB a tiny agent with a big effect. Consequently, L&R uses PHMB in combination with the biosynthetic Suprasorb X HydroBalance wound dressing for treatment of infected wounds or wounds at risk of infection, in various phases of wound healing, for lightly to moderately exuding wounds. Even with methicillin-resistant Staphylococcus aureus (MRSA) and vancomycinresitant Enterococcus (VRE), the antimicrobial effects of PHMB have been proven in studies

11 It fights bacterial cells but protects human cells A common complication associated with chronic wounds is colonization with Staphylococcus aureus, one of the dreaded causes of infections acquired in a clinical environment. Due to the specific propensity of Staphylococcus aureus in resistance to antibiotics, and in particular to methicillin (MRSA), an additional treatment by use of antimicrobial wound dressings is of essential importance for a successful outcome of therapy. The use of Suprasorb X + PHMB has a significant antibacterial effect on infected cells, while skin cells are protected against the pathogen. 7 No development of resistances with repeated inoculation Quick release for good success A successful and sustainable treatment is not only contingent upon the efficacy of PHMB as such. The release of PHMB from the wound dressing is also an essential factor with regard to therapy success. The Suprasorb X + PHMB HydroBalance wound dressing has produced good results in this regard. Within a short period of time, PHMB is released from the wound dressing. 6 Thus, even substantial bioburdens in a wound can be fought efficiently. Likewise, the effectiveness of PHMB over a period of 7 days has been demonstrated. 4 Even with frequently repeated use of PHMB, no resistances have been observed. Specific tests with Staphylococcus aureus have shown that no resistances were developed even after 100 days of repeated incubation with PHMB. 8 Particularly effective when used in combination In the treatment of infected wounds or wounds at risk of infection, Suprasorb X + PHMB is an outstanding therapy option. PHMB has been proven to be antimicrobially effective against the pathogens involved in colonization and infection, without any harmful effects on healthy tissue. 12 Pain management enhancing patients quality of life Wound pain is a complex, multi-level phenomenon. As a general rule, there are two different types of pain nociceptive pain and neuropathic pain. Nociceptive pain can be triggered, for example, by injuries. Nociceptive pain is usually temporary. Neuropathic pain, by contrast, is the result of a malfunction of the nervous system, which may be due to an injury or infection. Neuropathic pain is the most common cause of chronic pain conditions. In patients with chronic pain, even the slightest touch, or a change in temperature, can trigger severe pain. Especially when pain is persistent, the patients morale will also suffer. They will often feel left out from everyday activities because their pain deprives them of the energy they need to live a normal life like a healthy person. As a general rule, each individual patient perceives his or her pain in a different way. Listening to the patient An optimum approach to pain management must give consideration to all individual aspects of a patient s perception of pain. Therefore the attending physician must be capable of accurately assessing the pain. Patients should be encouraged to describe their pain to the medical professionals to help them obtain a clearer picture of its nature and severity. Furthermore, various instruments are available for the assessment of pain. For instance, patients may use a pain scale to describe their pain, or a pain diary in which they note all relevant aspects of wound pain over time. 12

Experts estimate that about three or four million people in Germany (approx. 4 % of the population) are suffering from chronic wounds. In most of the professional literature there is a common consensus that wounds should be referred to as chronic if no tendency towards healing is apparent within a period of four to twelve weeks from their origin (the individual nature of the wound as well as other factors are essential aspects in this context). 9 The most common types of chronic wounds include pressure ulcers, leg ulcers, and Diabetic Foot ulcers. 13

18 Suprasorb always the right choice Non-infected wounds and wounds at risk of infection With non-infected wounds, the main objective consists of promoting granulation and epithelization. In order to ensure a successful outcome, appropriate therapies for any underlying conditions, as well as adequate wound bed preparation (removal of necrotic tissues, coatings etc.) must be incorporated in the wound management concept. The choice of a therapeutically appropriate wound dressing is contingent upon the individual condition of the wound and the amount of exudate. Wounds at risk of infection Critical colonization may occur on a wound which is at risk of infection, which may quickly develop into a local infection involving all the complications commonly associated with this situation. What kinds of wounds are at risk of infection?* Whether a wound is at risk of infection or not depends on a variety of factors. Patients suffering from concomitant conditions, such as diabetes or an immunodeficiency, have an increased risk of infection compared to healthy individuals. In general, hygienic conditions and the specific site of the wound are also relevant aspects with regard to its susceptibility to infection. Thus, for instance, a pet owner will have a higher risk of infection. Similarly, gunshot wounds, bite wounds, and contaminated wounds sustained in an outdoor environment also involve a general risk of infection. In an effort to prevent such infections, antimicrobial wound dressings such as Suprasorb X + PHMB and Suprasorb A + Ag should be used. In addition to promoting wound healing, the objective is to prevent critical colonization and subsequent infection. 20 Suprasorb always the right choice Infected wounds With infected wounds, in addition to promoting wound healing, the main objective is to reduce the bioburden of microorganisms. Polyhexamethylene biguanide (PHMB) and silver have antimicrobial effects and can actively fight micro organisms in the wound area. PHMB is appropriate for long-term use, as it has special cell-preserving properties. The choice of a therapeutically appropriate wound dressing is contingent upon the individual condition of the wound and the amount of exudate. What is an infected wound?* A wound which is at risk of infection may quickly develop a critical colonization, which then advances to infection. Infections with Staphylococcus aureus or Enterococcus are particularly critical in this context, as these specific pathogens have a tendency to develop resistances (MRSA and VRE, respectively). These two are the most common causes of nosocomial infections (i.e. infections acquired in a hospital setting). In any case, the treatment of infected wounds requires antimicrobial wound management. For this specific purpose, the antimicrobial wound dressings Suprasorb A + Ag and Suprasorb X + PHMB are particularly appropriate, and for malodorous wounds also Vliwaktiv Ag. Suprasorb Liquacel forms a cohesive gel upon contact with wound exudate and this gel traps exudate, debris and bacteria and transfers them to a secondary dressing, e.g. Suprasorb P. Thereby exudate is kept away from the wound edge, the risk of maceration is reduced and wound healing is assisted. Suprasorb Liquacel stands out by its uncomplicated application even when moist: the dressing can be removed fast and easily in one piece. This atraumatic dressing change promotes wound healing. The primary dressing can be cut to size and folded and may also be used as a wound packing. It coats the wound bed completely and is low-adherent. Applications Protection and safety for the wound and wound environment. Suprasorb Liquacel is suitable for exuding, superficial to deep wounds. For example for ulcers of various aetiology, acute wounds (e.g. lacerations, cuts and abrasions, 2 nd degree burns), postoperative wounds (e.g. skin donor sites), and traumatic wounds. Suprasorb A + Ag is the dressing for wounds with high amounts of exudate, as well as wounds which are infected or at risk of infection. Through the gel formation process of the alginate, the silver ions released by contact with the wound exudate can exert their effect immediately on the surface of the wound.
