3 Lumbar Nerve Root Block What is a Lumbar Nerve Root Block? A lumbar nerve root block is an injection of local anaesthetic and steroid anti-inflammatory around the origin of a nerve as it exits the spinal cord in the small of your back on its way to your legs. This treatment is used when a particular nerve in the lower back is inflamed due to irritation from either a disc prolapse or narrowing of the spinal canal due to disc prolapse or narrowing of the spinal canal due to degenerative wear and tear. A nerve root block is not a cure, but aims to alleviate your symptoms by reducing the inflammation caused by the mechanical irritation of the nerve What is the aim of treatment? The aim of treatment is to deposit some local anaesthetic and anti-inflammatory drug close to the nerve root as it leaves the spine. This can help to decrease swelling and inflammation and may lead to a decrease in your leg pain. Before the procedure To be sure your general health is suitable to allow you to have this procedure you will first be assessed by the healthcare team. You will be asked to read the information leaflet and to Page 3
4 decide whether you wish to consent to having the procedure performed. You may wish to discuss this with your family. Normal medication should be taken prior to the procedure with the exception of medication to thin the blood or to inhibit blood clotting. Please tell your doctor if you take any substance of this nature. Your healthcare team will ensure that these are stopped at the appropriate time. How is the procedure performed? Your treatment will be performed as a day case procedure in the operating theatre. You will be positioned on your front to allow treatment of your back. The area will be washed with antiseptic solution to prevent infection. X ray will be used to see the bones and joints and to guide the doctor to position the needle. Local anaesthetic will be injected first to numb the skin. This stings a little at first. Using x-ray guidance, a fine needle is inserted into the exit hole of the nerve root suspected to be the cause of pain. A small amount of dye is injected to check the position of the needle (radiculogram) and then a small amount of local anaesthetic and anti-inflammatory steroids is injected around the nerve root. After the procedure, you may notice that your leg feels slightly warm and numb for a few hours afterwards. When you are able to stand unaccompanied you will be taken back to the ward area and after a period of observation on the Day Surgery Ward you will be allowed home. Page 4
5 You should arrange for a responsible adult to accompany you home and stay with you overnight after the treatment. Rarely you may be asked to stay in hospital overnight for observation. What is the potential benefit? Reduced leg pain and sensitivity allowing improved mobility. What are the potential risks? 1. You may feel a little bruised at the injection site - this is temporary and will be relieved by simple pain killers. 2. There may be a drop in your blood pressure - this is usually small and is treated by fluids given into a vein. 3. Procedures may cause an initial increase in pain but this is usually self-limiting. 4. Temporary limb weakness because of local anaesthetic being close to the nerve root. It is possible though rare for the nerve root to be bruised. 5. Bleeding may occur. 6. Infection may, rarely, develop but every care is taken to avoid this by carrying out the procedure under sterile conditions in theatre. 7. Failure or only partial success may occur. 8. Occasionally headaches may develop. This usually settles with drinking lots of water and painkillers. Page 5
6 Is there any alternative to this injection? Nerve Root Pain is most effectively treated by using more than one treatment at the same time. Physiotherapy, exercise, painkillers, and TENS therapy are some other options. WHAT If I have any FURTHER questions? Before treatment You should ask your doctor before signing the consent form. After treatment (on Day Ward) Ask a member of staff, they will answer your questions or will contact your doctor.
