Surveillance of Streptococcus pneumoniae. Current situation in Slovakia. Klement C., 1 Maďarová L., 1 Bottková E., 1 Hupková H., 2 Piačková V., 2 Avdičová M. 1 1 National Reference Centre for Invasive Pneumococcal Diseases, Regional Authority of Public Health Banská Bystrica 2 Institute of Microbiology, Faculty of Medicine, Comenius University, Bratislava
National Reference Centre for Invasive Pneumococcal Diseases the NRC has been established since 1 st january 2011 as the result of efforts of many public health and university experts established in Regional Authority of Public Health (RAPH) (RAPH) operates under Ministry of Health of Slovak Republic head of NRC: Assoc. Prof. Cyril Klement MD., PhD. (NMFP ECDC) employees: 6 great cooperation with Institute of Microbiology of Faculty of Medicine, Comenius University
Main tasks and objectives of NRC diagnostics of S. pneumoniae isolated from invasive pneumococcal diseases serotyping collection and storage of pneumococcal strains collecting of epidemiological data related to the vaccination introduction of new diagnostic procedures publications and education
Diagnostic procedures Commonly used laboratory methods of proof Culture (gold standard) PCR (conventional) Serotyping (Quellung reaction- in cooperation with Institute of Microbiology of Faculty of Medicine, Comenius University) mesacc.edu New diagnostic procedures in the validation process PFGE (after culture) Serotyping using the multiplex PCR (according to the Brito et al. 2003, Pai et al. 2006, Carvahlo et al. 2010, Pimenta et al. 2009)
Vaccination in Slovakia since valent conjugate vaccine was approved in Slovakia since 1 st january 2009 part of the routine infant immunization (firstly 7PCV; today 13PCV, 10PCV ); m.; m.; m. in the after approval the 13-valent conjugate vaccine replaced the 7PCV 23-valent pneumococcal polysaccharide vaccine (PPSV 23) mainly recommended for all adults aged 65 years nursing-medicine.blogspot.com
Results of Laboratory Diagnostics, 2011 during the strains isolated from severe invasive pneumococcal infections, delivered to NRC for further diagnostics (serotyping, PCR, PFGE) Examined clinical materials, % Blood culture Examined samples according to age groups, % 64% Cerebrospinal fluid Others (BAL; punctates) 49% 29% 22% Children under 2 years Adults aged 65 years Other age groups
Serotype Distribution, 2011 Serotype Distribution of S. pneumoniae Isolated from Invasive Diseases, Výskyt sérotypov S. pneumoniae v SR z invazívnych ochorení za rok 2011 Slovak Republic, Number of cases A 6B 14 19F 19C 4 15A 15B 15C 18C 18F 35B 6A 9L Sérotyp Serotype
EPIS all confirmed cases (case deffinition) of invasive pneumococcal diseases have to be reported in the SK- Epidemiological Information System (SK- EPIS) total number of reported cases is available on the website
Antibiotic Susceptibility Testing development of resistant strains respectively antibiotic susceptibility is monitored by NRC for monitoring resistance of microorganisms to antibiotics Slovak National Antimicrobial Resistance System (
To consider the progress that has made with the commitments from the 4 th Pneumo Surveillance Workshop During The Fourth Pneumo Surveillance Workshop, Slovakia made these commitment statements to address throughout 2011: Meeting objectives: 1. The technical guidelines of Ministry of Health for diagnosis and surveillance of invasive pneumococcal disease was developed (approved and valid since 1 st september 2011). 2. We have actively participated in some conferences and published several papers on this issue; introducing of new diagnostic methods (PFGE, multiplex PCR for serotyping).
Results & Conclusions very short period (1 year) of NRC activity it is not yet possible to make statistical analysis of obtained data is necessary to strengthen the system for reporting individual cases to Epidemiological Information System (EPIS) effectiveness of transmission of isolated strains (from invasive pneumococcal disease) in to NRC for further typing and analysis Autor: Shutterstock
