1 INSTRUCTIONS FOR USE ZINBRYTA (zin-bry-tuh) (daclizumab) Injectin, fr Subcutaneus Use Single-Dse Prefilled Syringe 150 mg Read this Instructins fr Use befre yu start using ZINBRYTA and each time yu get a refill. There may be new infrmatin. This infrmatin des nt take the place f talking t yur healthcare prvider abut yur medical cnditin r yur treatment. Imprtant infrmatin: Befre yu use ZINBRYTA fr the first time, yur healthcare prvider shuld shw yu r yur caregiver hw t prepare and inject yur ZINBRYTA prefilled syringe the right way. ZINBRYTA is fr use under the skin nly (subcutaneus). ZINBRYTA prefilled syringe is fr 1 time use nly. D nt share yur ZINBRYTA prefilled syringe with ther peple. Yu may give an infectin t them r get an infectin frm them. Thrw away (dispse f) the ZINBRYTA prefilled syringe right away after use. See Dispsing f ZINBRYTA prefilled syringes at the end f this Instructins fr Use. Supplies needed fr yur ZINBRYTA prefilled syringe injectin (See Figure A): a ZINBRYTA 150 mg prefilled syringe Additinal supplies yu will need fr yur injectin (See Figure B): alchl wipe gauze pad adhesive bandage 1 sharps cntainer fr thrwing away ZINBRYTA prefilled syringes. See Dispsing f ZINBRYTA prefilled syringes at the end f these instructins. a well-lit area and a clean, flat surface t wrk n, like a table

2 Parts f yur ZINBRYTA prefilled syringe (See Figure C): Befre yu prepare yur injectin, take yur ZINBRYTA prefilled syringe ut f the refrigeratr and let it cme t rm temperature fr at least 30 minutes. D nt use external heat surces such as ht water t warm the ZINBRYTA prefilled syringe. D nt use mre than 1 prefilled syringe each mnth. The finger flange will allw yu t hld the prefilled syringe mre firmly. D nt remve the finger flange. Preparing fr yur injectin: Step 1: Cllect yur supplies and wash yur hands Find a well-lit and clean, flat surface, like a table and cllect all the supplies yu will need t give yurself r t receive an injectin. Wash yur hands with sap and water and dry yur hands well.

3 Step 2: Check yur ZINBRYTA prefilled syringe Check the expiratin date printed n yur ZINBRYTA prefilled syringe (See Figure D). D nt use ZINBRYTA prefilled syringe past the expiratin date. Check that yur ZINBRYTA medicine is clear and clrless t slightly yellw (See Figure E). D nt use ZINBRYTA prefilled syringe if the liquid is cludy, r has flating particles in it. Yu might see air bubbles in yur ZINBRYTA medicine. This is nrmal and will nt affect yur dse. Giving yur injectin: Step 3: Chse and clean yur injectin site ZINBRYTA prefilled syringe is used t inject ZINBRYTA under the skin nly (subcutaneus). ZINBRYTA prefilled syringe shuld be injected int the thigh, abdmen, r the back f the upper arm (See Figure F). D nt inject directly int yur belly buttn. D nt inject int an area f the bdy where the skin is irritated, tender, red, bruised, infected, scarred, r tatted. Chse an injectin site and wipe the skin with an alchl wipe. Let the injectin site dry befre injecting the dse. D nt tuch this area again befre giving the injectin. D nt take ff the ZINBRYTA prefilled

4 syringe needle cver until yu are ready t inject. Step 4: Remve the ZINBRYTA prefilled syringe needle cver With 1 hand, hld yur ZINBRYTA prefilled syringe as shwn. D nt push n the finger flange. With yur ther hand, firmly hld the needle cver and pull the needle cver straight ff the needle (See Figure G). Thrw away the needle cver after yur injectin. D nt recap yur ZINBRYTA prefilled syringe, yu culd get a needle stick injury. D nt tuch the needle. Step 5: Give yur ZINBRYTA prefilled syringe injectin With 1 hand, use yur thumb and first finger and pinch the skin arund the cleaned injectin site (See Figure H). With yur ther hand, hld yur ZINBRYTA prefilled syringe like a pencil. Use a quick dart-like mtin and insert the needle at a 90 angle int yur skin (See Figure I). After the needle is in, let g f yur skin Slwly push the plunger dwn until the syringe is empty (See Figure J).

5 D nt pull back n the plunger. D nt take yur ZINBRYTA prefilled syringe ut f yur skin until yu have slwly pushed the plunger all the way dwn. Step 6: Remve the ZINBRYTA prefilled syringe frm yur injectin site Pull the needle straight ut f yur skin (See Figure K). If yu see bld at yur injectin site, wipe it ff with gauze and apply an adhesive bandage. D nt recap the ZINBRYTA prefilled syringe. Recapping the prefilled syringe can lead t a needle stick injury. D nt reuse the ZINBRYTA prefilled syringe. After yur injectin: Step 7: Dispsing f ZINBRYTA prefilled syringes Put yur used ZINBRYTA prefilled syringes in a FDA-cleared sharps dispsal cntainer right away after use (see Figure L). D nt thrw away (dispse f) lse needles and ZINBRYTA prefilled syringes in yur husehld trash. If yu d nt have a FDA-cleared sharps dispsal cntainer, yu may use a husehld cntainer that is: made f a heavy-duty plastic, can be clsed with a tight-fitting, puncture-resistant lid, withut sharps

6 being able t cme ut, upright and stable during use, leak resistant, and prperly labeled t warn f hazardus waste inside the cntainer. When yur sharps dispsal cntainer is almst full, yu will need t fllw yur cmmunity guidelines fr the right way t dispse f yur sharps dispsal cntainer. There may be state r lcal laws abut hw yu shuld thrw away used needles and syringes. Fr mre infrmatin abut safe sharps dispsal, and fr specific infrmatin abut sharps dispsal in the state that yu live in, g t the FDA s website at: D nt dispse f yur used sharps dispsal cntainer in yur husehld trash unless yur cmmunity guidelines permit this. D nt recycle yur used sharps dispsal cntainer. Hw shuld I stre ZINBRYTA Prefilled Syringe? Stre ZINBRYTA prefilled syringe in the refrigeratr between 36 F t 46 F (2 C t 8 C ). D nt freeze ZINBRYTA prefilled syringe r expse t temperatures abve 86 F (30 C). D nt use ZINBRYTA prefilled syringe that has been frzen. Keep ZINBRYTA prefilled syringe in the riginal cartn t prtect it frm light. If yu cannt refrigerate ZINBRYTA prefilled syringe, yu can stre ZINBRYTA at rm temperature fr up t 30 days. If ZINBRYTA prefille syringe has reached rm temperature, d nt put it back in the refrigeratr. Keep ZINBRYTA prefilled syringe and all medicines ut f the reach f children. Questins?
