3 How to prevent outbreaks How to prevent outbreaks We know protecting your staff and guests is your number one priority and that an outbreak of infection in your establishment will seriously damage your reputation and cost your business significantly. It is difficult to predict instances of illness or outbreak, especially when large numbers of people gather in close proximity to one another. The important factor is to be prepared. Precautionary measures and ensuring you have the right resources on site to enable a speedy response to illness will be the difference between a few isolated instances and a full blown outbreak. Ultimately outbreaks are more difficult to control and costly to business. This is why prevention is better than cure. What is an infection? Infections are caused by pathogens ( bugs ) such as bacteria, viruses, yeasts or fungi that enter into the body. It can take some time before the microbes multiply enough to trigger the symptoms of an illness, which means an infected person may unwittingly spread the disease during this incubation period. Instances of transmission can rapidly escalate into larger scale outbreaks which are often difficult to control and extremely damaging to health and business alike. It is the responsibility of employers to provide a safe workplace for their employees and guests, which includes the provision of adequate infection control procedures. There are however precautions you can take to reduce and control the incidence of outbreaks at your sites. How are infections transmitted? Pathogens can spread in a variety of ways and understanding these different modes of transmission will help your staff to adopt good infection control practices. Airborne: Coughs or sneezes release airborne pathogens which are then inhaled by others. If infected people sneeze or cough they can spread germs through tiny airborne droplets. These droplets can land on surfaces. Hands and surfaces soiled with nasal and throat discharges can then aid the spread of the disease. Some of the infections that are spread in this way include: The common cold Parvovirus Chickenpox Diphtheria Influenza Measles Norovirus (Viral gastroenteritis) Contaminated objects or food: Soiled hands carry pathogens from other surfaces and even human feces. These can be spread to food or other objects. If a person is unwell their feces could carry viruses, bacteria or parasites. Remember, a person does not have to seem unwell to be carrying a pathogen. When ensuring pathogens are not transmitted, special attention should be paid to hand and surface hygiene. Frequently touched surfaces pose the greatest risk. These objects and surfaces can include: Taps Toilet flush handles Table tops Telephones Remote controls Some of these pathogens cause diarrhea such as: Hepatitis A, Salmonella Shigella Foot and mouth 2
4 How to prevent outbreaks Skin-to-skin contact: The transfer of some pathogens can occur through touch, or by sharing contaminated items such as clothing. These infections include: Impetigo Head lice Herpes (cold sores) Ringworm Scabies Contact with body fluids: Pathogens in saliva, urine, feces or blood can be passed to another person s body via cuts or abrasions, or through the mucus membranes of the mouth and eyes. AIDS (caused by the human immunodeficiency virus - HIV) or Hepatitis B and C are spread through blood or blood products. Transmission can occur through broken skin, sexual contact, sharing needles or needle stick injuries. Bites: Although rare in the UK, some viruses such as Malaria are spread by blood-to-blood contact through mosquito bites. Hand washing: To wash your hands effectively wet them, apply soap, lather it fully and rub your hands together for at least 20 seconds. Then rinse all the soap off and dry them fully with a paper towel. To make sure you have washed every part of your hands we recommend you follow this illustration. How to Wash your hands Wet hands with Apply enough 1 water. 2 soap to cover 3 all hand surfaces Palm to palm Backs of 5 with fingers 6 fingers to 7 interlaced opposing palms with fingers interlocked; Rinse hands Dry hands 9 with water 10 thoroughly with 11 a single use towel Rub hands palm to palm Rotational rubbing of left thumb clasped in right palm and vice versa; Use towel to turn off faucet; 8 12 Right palm over left dorsum with interlaced fingers and vice versa Rotational rubbing, backwards and forwards with clasped fingers of right hand in left palm and vice versa Your hands are now safe How can you control infections? Infections can be prevented or controlled by reducing the opportunities for infection transmission. This can be achieved by adopting basic infection control practices Sealed Air Corporation. All Rights Reserved en08/1 Check SDS for detailed information Basic infection control practice Good infection control begins with assuming everyone is potentially infectious and following proper procedures at all times. The following provides guidelines to reducing transmission of infection: Hand Hygiene: Effective hand hygiene is the greatest single measure that you can take to prevent the spread of pathogens. The first decision you should make is whether to wash or disinfect your hands. You should wash your hands if you: Can see soiling on your hands Think you have been in contact with a spore based illness Have just used the toilet Are about to prepare food Have just completed a dirty task (emptied the bins) At all other times sanitising them should be sufficient. You can download a copy of this illustration to place on your wall from the download section of our site It is available in fithteen different languages. For hand washing Diversey Care has a great range of products including Soft Care Sensisept. Hand Hygiene: Effectively reduces the number of microbes on the hands. It is quick and easy and requires no water or hand towels. To sanitise your hands apply 3mls of alcohol gel and rub them for 30 seconds. To ensure you cover the entire hand you can use the same method as seen in step 3 for hand washing. Do not rinse or dry the hands. The alcohol gel will evaporate. 5 Wound dressing: Broken skin wounds provide an opportunity for pathogens to penetrate the body. Individuals with cuts, burns, sores or other forms of open wounds must have the wounds covered using waterproof dressings. Personal items: Discourage individuals from sharing towels, clothing, razors, toothbrushes, shavers or other personal items. Infectious waste: To dispose of infectious waste that has been contaminated with blood or other body fluids, you should: Wear heavy duty gloves. Place waste in yellow clinical waste bags marked infectious waste. Dispose of waste in accordance with EPA regulations. Occupational exposure: If an individual is exposed to blood/body fluid: Flush the area with running water. Wash the area with plenty of warm water and soap. Report the incident to the appropriate staff member. Record the incident via your reporting procedure. Seek medical advice. Employers and occupational health and safety representatives should investigate all incidents involving contact with blood or body fluids. Prevention measures Strategic cleaning and disinfecting will reduce the opportunity for pathogenic outbreak. 1. Clean all areas daily. 2. Maintain excellent hand hygiene. 3. Use a disinfectant for targeted disinfection of frequently touched surfaces.. Deal with body spillages immediately. Diversey Care has a great range of disinfectants, but we recommend Oxivir Plus. Oxivir Plus is highly effective against a broad range of pathogens and yet suitable for use on a most surfaces and can even be used on soft furnishings such as carpets. It is not suitable for marble and acid sensitive surfaces however, and you should test in an inconspicuous area before using any disinfectant. So that you have everything in place to respond quickly and effectively. General hard surface cleaning & disinfection 1. Put on disposable gloves and apron. 2. Put out cleaning in progress sign and isolate area. 3. Pre-soak a disposable cloth with disinfectant and remove gross soil (if necessary), then place in plastic bag for disposal.. Clean surface with a disposable cloth pre-soaked with disinfectant, then dispose in a plastic bag. 5. Spray the disinfectant liberally onto surface and spread with a disposable cloth so not such that the entire surface is wet with the disinfectant. Dispose of cloth into plastic bag. 6. If the disinfectant does not require rinsing allow the surface time. 7. All soiled materials and protective clothing must be deposited into a yellow clinical waste bag and disposed as contaminated material. 8. Wash hands thoroughly for at least 20 seconds using hand soap. High risk areas Certain environments offer greater opportunity for spread of pathogens from one individual to another. These include areas where public traffic is high, body spills are frequent or a high level of hygiene is critical. Areas to be considered as high risk include: Public washrooms Baby changing areas Gymnasiums Telephone boxes High risk infection areas need to be cleaned on a regular and daily basis to create a barrier against pathogen spread.
6 How to prevent outbreaks Cleaning a high risk area Strategic cleaning and disinfecting will reduce the opportunity for pathogenic outbreak. 1. Prevent access to the public by attaching a closed for cleaning sign to the door. 2. Open windows to ventilate the area and accelerate drying. 3. Wear appropriate disposable protective clothing and primarily target any touch surfaces with a disinfectant. These include: Door handles Locks Light switches Telephones Toilet flush handles Taps. Always work in a clean to dirty manner; begin with windows and mirrors followed by fixtures/ fittings, wash basins, shower/bath and then toilet area. 5. Discard the used cloths and personal protective equipment into a clinical waste bag. Dispose as contaminated waste. 6. Wash hands for at least 20 seconds with a hand wash. 7. Leave the area closed for 5 minutes to allow the door to dry then re-open. Soft Care Des E - Hand Disinfectant Soft Care Des E is an alcohol based hand disinfectant for waterless skin disinfection. Soft Care Des E is suitable for use in all areas, including healthcare and food processing environments. Soft Care Des E is a hand disinfectant based on ethanol (71.5%). Ethanol is a disinfectant which is very effective against resident and transient flora such as bacteria (e.g. MRSA) and yeast. It is also effective against enveloped viruses such as influenza and certain non-enveloped viruses such as Norovirus and Adenovirus. 7 Hand Washing Soft Care Sensisept H3 - Hand Sanitising Wash Soft Care Sensisept is a very mild one stage disinfection hand wash, with patented technology. It offers a high level of disinfection and yet independent laboratories have proved that Soft Care Sensisept has an extremely low skin irritation potential and will not affect the natural skin barrier making it mild and pleasant to use.
