1 Patient # 1 vomiting for three days. Diarrhea started yesterday and parents became concerned when the child s stool was bloody. The child started complaining about stomach pain three days ago and has not had an appetite since then. Prelimary Exam: Child seems irritable and has a slightly swollen abdomen. Temperature Blood Pressure 106/68 Additional Information: One week ago the family attended the NC State Fair. The child is a picky eater so they did not buy food. They did however go on the fair rides and visit two of the petting zoos._the mother started having abdominal pains last night. Other medications/treatments: Parents have treated the stomach pain and vomiting with over the counter medications.

2 Patient # 2 tired and lethargic for 7 days. Parents suspect the flu due to the extreme decrease in activity and the child is achy as well as weak. Stomach pain and diarrhea started three days ago. Prelimary Exam: Child has pain only in the upper right abdominal region. _Whites of eyes are slightly yellow. Temperature 99.8 Blood Pressure 115/74 Additional Information: The family always cooks and eats at home until recently. Because they are moving into a new house they have been eating out the last two weeks almost every night. Their favorite place to eat is a restaurant called Sweet Carrots. They have a huge salad bar that the family loves since they are all vegetarian. Other medications/treatments: Parents have treated the achy muscles and low grade fever with Tylenol. Child has been kept home from daycare for fear of giving the possible flu to the other children.

3 Patient # 3 coughing and wheezing for 1 week. Symptoms worsen at night. Parents have concerns due to the child s history with asthma. Stuffy nose and cough produce brownish mucous. Prelimary Exam: Child has chest pain when taking deep breaths. Appears extremely tired. Temperature Blood Pressure 115/74 Additional Information: Child has not been active for over a week. The last time the parents remember the child being active or outside was when they had a family yard clean up day. They were outside all day raking and gathering leaves that had fallen this fall. Even though the leaf piles were wet, the children still jumped in them to play. The patient s siblings are not sick but they do not suffer from asthma. Other medications/treatments: Child takes prescription fluticasone and albuterol for asthma. Parents have been using the nebulizer each night. Tylenol to treat the low grade fever.

4 Patient # 4 Symptoms/Concerns (include timeframe of symptoms when possible): Severe fatigue for a week. Child has had a fever as well for almost a week and complaining of aches and pains. Prelimary Exam: Child has slightly swollen knees that are tender and sore. There is also a erythema migrans at the base of the child s neck that was hidden by hair. Temperature Blood Pressure 112/70 Additional Information: Child has not been active for over a week due to fatigue and pain. The family returned from a camping trip last week before the child became ill. Other medications/treatments: Tylenol to treat the fever and aches.

5 Patient # 5 tired and lethargic for 3 days. The patient is suffering from aches, fever, sore throat, and extreme fatigue. Prelimary Exam: Child looks tired. Feels warm to the touch and says they have chills. Temperature Blood Pressure 118/76 Additional Information: Child has only been back to daycare for two weeks since the Christmas holiday. Both parents are elementary school teachers so they have also been around other sick children. Other medications/treatments: Parents have treated the achy muscles and fever with Tylenol.

6 Patient # 6 tired and lethargic for approximately 2 weeks.. The patient is suffering from body aches, low-grade fever, and headaches. Prelimary Exam: Child looks tired. Feels warm to the touch and lymph nodes are swollen. Temperature 99.1 Blood Pressure 116/80 Additional Information: Child has stayed home from day care on and off for 2 weeks. Parents have adjusted diet, but no changes noticed. Because the child has not been feeling well, they haven t been able to do their normal household chores which include: straightening their room, picking up toys, and cleaning the cat s litter box. Other medications/treatments: Parents have treated the achy muscles and fever with Children s Motrin,

7 Patient # 7 complaining of a sore throat for two days. One day ago a the child started to run a fever that has been difficult to control with over the counter medications such as Children s Motrin or Tylenol. Child has been lethargic and has no appetite. Prelimary Exam: Lymph nodes are slightly swollen in neck. Throat is red and has small blisters. Faint red rash on the child s body. Temperature 102 Blood Pressure 118/80 Additional Information: Child just started at a daycare program a week ago when their Dad transitioned from being a stay at home Dad to working as a computer programmer full time. Other medications/treatments: Parents have treated the fever with Children s Motrin but nothing for the sore throat (were afraid child would choke on a lozenge.

8 Patient # 8 experiencing headache, diarrhea, and abdominal pain for the past 24 hours. Prelimary Exam: Child seems lethargic and has a slightly swollen abdomen. Temperature Blood Pressure 90/68 Additional Information: One week ago the family returned from a visit to some relatives who have a goat farm. Yesterday the child s mother received a call from the cousin with whom they visited. Both of her children and husband are sick with the same symptoms. Mom thinks this is a stomach bug that the kids picked up from her cousin s children. Past medical history indicates that the family have city water in the home, but the goat farm has well water. Other medications/treatments: Parents have treated the stomach pain and vomiting with over the counter medications.

9 Patient # 9 experiencing shortness of breath and nausea for the past 24 hours. Prelimary Exam: Child seems air hungry and agitated. Has a hard time laying down and prefers to sit bolt upright. Temperature Blood Pressure 115/68 Additional Information: 4 months ago the family returned from a trip to Washington state where they camped. No other trips were reported. The child is home-schooled and therefore doesn t really contact many other people outside of the family. Other medications/treatments: Parents have treated the breathing issues by putting a vaporizer in the child s room. They also propped the child up on pillows to sleep.
