1 Last Updated: Version 3.3 NQF-ENDORSED VOLUNTARY CONSENSUS STANDARDS FOR HOSPITAL CARE Measure Set: Pneumonia (PN) Set Measure ID #: Measure Information Form Performance Measure Name: Pneumococcal Vaccination Description: Pneumonia patients, age 65 and older, who were screened for pneumococcal vaccine status and were administered the vaccine prior to discharge, if indicated. Rationale: Pneumococcal vaccination is indicated for persons 65 years of age and older, because it is up to 75% effective in preventing pneumococcal bacteremia and meningitis. It is also an important vaccine due to increasing antibiotic resistance among pneumococci. In the United States today, vaccine coverage is suboptimal. Although inpatient vaccine screening and administration are recommended, hospitalization is an underutilized opportunity for adult vaccination. Some hospitals may prefer to gather data concurrently by identifying patients in the population of interest. This approach provides opportunities for improvement at the point of care/service. However, complete documentation includes the principal and other ICD-9-CM diagnosis and procedure codes, which require retrospective data entry. Data Accuracy: Variation may exist in the assignment of ICD-9-CM codes; therefore, coding practices may require evaluation to ensure consistency. Run cases that are included in the Pneumonia (PN) Initial Patient Population and pass the edits defined in the Transmission Data Processing Flow: Clinical through this measure. 2. Check Chest -Ray a. If Chest -Ray is missing, the case will proceed to a Measure Category Assignment of and will be rejected. Stop processing. b. If Chest -Ray equals 2 or 3, the case will proceed to a Measure Category Assignment of and will not be in the Measure Population. Stop processing. c. If Chest -Ray equals 1, continue processing and proceed to Comfort Measures Only. 3. Check Comfort Measures Only a. If Comfort Measures Only is missing, the case will proceed to a Measure Category Assignment of and will be rejected. Stop processing. b. If Comfort Measures Only equals 1, 2, or 3, the case will proceed to a Measure Category Assignment of and will not be in the Measure Population. Stop processing. c. If Comfort Measures Only equals 4, continue processing and proceed to Clinical Trial. 4. Check Clinical Trial a. If Clinical Trial is missing, the case will proceed to a Measure Category Assignment of and will be rejected. Stop processing. b. 7 5. Check Patient Age The Patient Age is calculated from Admission Date minus irthdate as part of the ICD Population logic. a. If Patient Age is greater than or equal to 18 and less than 65, the case will proceed to a Measure Category Assignment of and will not be in the Measure Population. Stop processing. b. If Patient Age is greater than or equal to 65, continue processing and proceed to Discharge Disposition. 6. Check Discharge Disposition a. If Discharge Disposition equals 2, 3, 4, 6, 7, the case will proceed to a Measure Category Assignment of and will not be in the Measure Population. Stop processing. b. If Discharge Disposition equals 1, 5, 8, continue processing and proceed to Pneumococcal Vaccination Status. 7. Check Pneumococcal Vaccination Status a. If Pneumococcal Vaccination Status is missing, the case will proceed to a Measure Category Assignment of and will be rejected. Stop processing. b. If Pneumococcal Vaccination Status equals 5, the case will proceed to a Measure Category Assignment of D and will be in the Measure Population. Stop processing. c.
