Articulate the dental profession s consensus statement regarding infection control education 2. List the overarching core competencies and broad learning objectives for dental infection control education 3. Describe how to obtain the list of essential elements for an infection control curriculum that is based on federal standards, regulations and evidence-based guidelines. 4. Recognize how the work of the Infection Control Consortium proactively addresses the national trend toward competency-based care as described in the 2018 OSAP plenary session, Accreditation in an Increased Environment of Scrutiny Implications for Dentistry. 2 Brief History 2013 High profile infection control breaches in a handful of dental offices Awareness (by state dental boards and CDC) of non-compliance with federal guidelines and regulations Passage of legislation in several states requiring hospital-level central sterilization processing (CSP) certification for Federally Qualified Health Center (FQHC) employees Dental equipment not addressed in certification Law may apply to dental personnel Infection Control Consortium 2014 National Dental Infection Control Consortium (IC Consortium) established CDC + 7 national organizations agreed to have a broader discussion around issue 20 participants (staff and SME s) from U.S. Centers for Disease Control and Prevention (CDC) plus: IC Consortium s Goal and Charge Goal: To enhance the protection of the public and the dental team Charge: To provide a framework for infection control education of oral healthcare professionals responsible for performing and/or overseeing infection control procedures in oral healthcare settings Conference 2

3 Outcomes of the IC Consortium After many planning calls, IC Consortium convened in Chicago, August 6, 2014 Meeting outcomes included agreement by all participating organizations plus CDC: 1. The importance of patient and provider safety 2. in infection prevention and control that is based on federal standards, regulations and evidence-based guidelines should be required for all dental healthcare personnel 3. Defined educational components related to dental infection control, including a. Overarching core competencies b. Broad learning objectives c. Suggested list of essential elements of an infection control curriculum Core Competencies The IC Consortium identified the following overarching core competencies: Knowledge-based competency in an infection control curriculum will be measured by the ability to demonstrate: Knowledge of disease transmission and principles of infection prevention and control in oral healthcare settings Application of relevant infection control laws, regulations, guidelines, standards and best practices Use of quality assurance measures (e.g., direct observation and feedback) to help to ensure accurate implementation of recommended infection control practices Learning Objectives Based on these overarching core competencies, the IC Consortium outlined the following high-level learning objectives: Upon successful completion of the course, participants will be able to demonstrate understanding of: The chain of infection Differences between standard and transmission-based precautions Infection control practices and precautions that prevent transmission of infectious agents Identification of and response to breaches in infection control Relevant infection control laws, regulations, guidelines and standards and their integration into best practices Conference 3

4 Master Curriculum Elements To provide a curriculum framework for the infection control education of professionals responsible for infection control procedures in oral healthcare settings and those who supervise them, the IC Consortium followed: The outline of topics in CDC Guidelines for Infection Control in Dental Settings (2003) and CDC Summary of Infection Prevention Practices in Dental Settings: Basic Expectations for Safe Care (2016) OSHA Bloodborne Pathogens Standard (1910: 1030) and OSHA Hazard Communications Standard Other relevant federal standards from EPA FDA CDC Other agencies IC Consortium s dental infection control framework = nearly 20 primary curriculum elements Master Curriculum Elements Transmission and Prevention of Infectious Diseases Overview of Laws and Guidelines Applicable to Oral Healthcare Settings Personnel Health Elements of an Infection Control Program Preventing Transmission of Bloodborne Pathogens Hand Hygiene Personal Protective Equipment Master Curriculum Elements, Cont. Respiratory Hygiene/Cough Etiquette Contact Dermatitis and Latex Hypersensitivity Sharps Safety and the Needlestick Prevention Act Safe Injection Practices Sterilization and Disinfection of Patient-Care items Environmental Infection Control Conference 4

Dental Unit Waterlines, Biofilm and Water Quality Special Considerations Written Procedures Policies, Procedures and Recordkeeping Hazard Communication The Message The IC Consortium s participating organizations considered this work to be a significant step forward in promoting a unified message from the oral healthcare professions: That the protection of the public and the dental team is paramount and That a basic science-based curriculum is necessary for the safe delivery of oral healthcare The Message Other organizations asked to be kept apprised including but not limited to: AAPHD ADHA APIC ASTDD CODA NNOHA Conference 5

6 Publication of CDC Summary (2016) Work of the IC Consortium to finalize the outcomes extended 2 years beyond August 2014 in-person meeting In mid-2015, OSAP was informed that CDC would soon introduce a Summary of Infection Prevention and Control Practices in Dental Settings: Basic Expectations for Safe Care ( Summary ) Describes basic infection prevention recommendations and includes a checklist Does not replace CDC s Guidelines for Infection Control in Dental Healthcare Settings (2003) The IC Consortium expressed its support for this Summary and, agreed to wait until CDC published its Summary, in order to include its support as part of IC Consortium Proceedings publication CDC Summary March 2016 CDC introduced a Summary of Infection Prevention Practices in Dental Settings: Basic Expectations for Safe Care ( Summary ) IC Consortium Master Curriculum Elements - consistent with CDC Guidelines (2003), CDC Summary (2016) and OSHA Bloodborne Pathogens Standard (1910:1030) IC Consortium s Master Curriculum Elements - resource to develop education for dental and medical health care personnel working in dental settings, as well as educators, consultants, regulatory agencies and dental manufacturers and distributors IC Consortium: Conclusion and Impact IC Consortium participants consider the outcomes of their work: a significant step forward in promoting a unified message from the profession that the protection of the public and the dental team is paramount basic evidence-based curriculum is necessary for the safe delivery of oral healthcare. 7 Advancing Knowledge-Based Competence What is OSAP doing to advance knowledge-based competence in dental infection prevention and control? In 2016, OSAP, the Dental Assisting National Board (DANB) and its official affiliate, the Dental Auxiliary Learning and Foundation (the DALE Foundation) formed a multi-year collaborative relationship with these areas of expertise: OSAP (dental infection prevention and control education) DANB (professional credentialing, including certification) The DALE Foundation (infrastructure for delivery of online education) Advancing Knowledge-Based Competence The OSAP-DANB-DALE Foundation Collaboration focuses on the development, implementation and administration of 4 pillars of lifelong learning, applying federal infection prevention and control standards and guidelines in dental settings multi-faceted, vetted education offerings 2 types: a certificate program = education and exam; PLUS ongoing OSAP CE programs and OSAP-DALE Foundation online modules) knowledge-based competence (2 OSAP-DANB credentials) industry specialist credential professional certification Advancing Knowledge-Based Competence Who should obtain education in dental infection prevention and control (IPC)? 1. Individuals responsible for IPC in dental settings and those who supervise them 2. Educators of individuals responsible for IPC in dental settings, and IPC consultants to dental settings 3. State investigators and inspectors of dental settings 4. Sales representatives and dental IPC educators employed by manufacturers or distributors of IPC products and services Conference 7
