1 Crown Lengthening
2 CROWN LENGTHENING (CL) GAUGE: The Crown Lengthening Gauge has the Biologic Periogauge (BLPG) tip on one end and the Papilla tip on the opposite end. The BLPG tip is used to achieve the proper mid-facial clinical crown and biologic crown (osseous crest to incisal edge position) length during surgical crown lengthening procedures. The Papilla tip follows the use of the BLPG tip to establish the correct aesthetic position of the interdental papilla from the incisal edge before the flap is closed and sutured. BLPG TIP All measurements are noted in millimeters. Longer Vertical Arm: Biological Crown Length Measurement Shorter Vertical Arm: Clinical Crown Length Measurement Incisal Guide: Starting Point for Measurements The BLPG Tip is designed to measure the mid-facial length of the newly restored clinical crown and the length of the biologic crown (bone crest to the incisal edge), simultaneously. The instrument is color coded with a preset mid-facial dentogingival complex measurement of 3mm. This is based on the ideal 3mm difference recommended between the clinical length and the biologic length of the crown. (See above.) The color coded marks on the shorter arm represents the clinical crown length and the corresponding color markings on the longer arm represents the biologic crown length. Note: The short arm of the BLPG tip is of the same length and measurement as the long arm of the T-bar and the Inline tip. BLPG In Use: Incisal edge position should be determined before the instrument is used. Hold the CL Gauge in a pen grasp with the BLPG tip facing up. Rest the incisal stop against the incisal edge of the tooth to be measured. Center the tip along the long axis of the tooth. On the central incisor the clinical crown length should be at the outer margin of the red band on the short arm (same as the long arm of the T-bar and Inline tip) and the biologic crown should be at the outer margin of the red band on the long arm to achieve the desired 3mm difference. Similarly lateral incisor measurements should be at the outer margin of the blue bands on both arms and the canine measurements should be at the outer margin of the yellow band below the red band on both arms.

4 EXCEPTIONS IN TOOTH SIZE: Small Teeth: Measurements move downward one full band from the normal measurements on both arms due to the smaller size. Central incisor measurements should be at the inner margin of the red band on both arms representing the clinical crown length and biologic crown length, respectively. Lateral incisor measurements should be the inner margin of the blue band and the canine measurements should be at the inner margin of the yellow band below the red band on both arms Large Teeth: Measurements move upward one full band from the normal measurement on both arms due to the larger size. Central incisor measurements should be at the outer margin of the yellow band above the red band on both arms. Lateral incisor measurements should be at the inner margin of the red band and the canine measurements should be at the outer margin of the red band.
5 Extra-Large Teeth: Measurements move upward two full bands from the normal measurement on both arms. Central incisor now should be at the outer margin of the black band on both arms. Lateral incisor should be at the inner margin of the yellow band, and canine measurements should be at the outer margin of the yellow band below the black band on both arms. PAPILLA TIP The Papilla tip is used in conjunction with the BLPG tip during the crown lengthening procedure to determine the ideal aesthetic position of the interdental papilla when the flap is closed and sutured. The tip has a long arm positioned behind the short arm. The long arm is color coded to determine the interdental biologic crown length and the short arm helps determine the clinical interdental papilla position corresponding to the teeth being measured. Longer Vertical Arm: Interdental Biological Crown Length Measurement Shorter Vertical Arm: Clinical Interdental Papilla Measurement Incisal Guide Measurements in mm Considering the facts of the clinical research, the ideal aesthetic position of the interdental papilla should be at about 60% the length of the clinical crown measured from the incisal edge position. The interdental biologic crown (interproximal bone crest to the incisal edge) is 4 mm greater than the clinical interdental papilla position. Hence the tip is color coded with preset interproximal dentogingival complex measurements of 4mm. For in-
6 stance the measurement on the outer margin of the red band on the long arm is 10.5mm and the one on the outer margin of the red band on the inner arm is 6.5 mm, giving a 4mm variance. When the interproximal biologic crown is determined with a particular color band on the long arm, the interdental papilla is positioned at the corresponding color band on the short arm. PAPILLA TIP IN USE: Hold the CL Gauge using a pen grasp with the Papilla tip facing up. Rest the incisal stop against the interproximal incisal edge of the tooth to be measured. Start from the central incisor. The interproximal biologic crown length on a central incisor should be at the upper margin of the red band on the long arm and the interdental papilla position between the two centrals should be at the upper margin of the red band on the short arm. Align the outer RED markings Similarly, interproximal measurements between central and lateral incisors should be at the upper margin of the blue band on both arms. 9 Align the outer BLUE markings 5
7 PAPILLA TIP IN USE CONTINUED: Interproximal measurements between lateral incisor and canine should be at the upper margin of the yellow band (located between the red and blue bands). 9.5 Align the outer YELLOW markings 5.5 VARIATIONS IN TOOTH SIZE Small Teeth: Measurements move downward on both arms one full band from the normal measurement due to the smaller size. Interproximal measurements between two central incisors should be at the lower margin of the red band on both arms. Interproximal measurements between the central and the lateral should be at the lower margin of the blue band on both arms. The interproximal measurements between the lateral incisor and canine should be at the lower margin of the yellow band (below the red band) on both arms.
8 Large Teeth: The measurements move upwards one full band from the normal measurement due to the larger size. Interproximal measurements between two central incisors should be at the upper margin of the yellow band on both arms just below the black band. Interproximal measurements between the central and the lateral should be at the lower margin of the red band on both arms. The interproximal measurements between the lateral incisor and canine should be at the upper margin of the red band on both arms. Extra-Large Teeth: Measurements move upward two full bands from the normal measurement on both arms. Interproximal measurements between two central incisors should be at the upper margin of the black band on both arms. Interproximal measurements between the central and the lateral should be at the lower margin of the yellow band on both arms. The interproximal measurements between the lateral incisor and canine should be at the upper margin of the yellow band on both arms just below the black band.
9 CLEANING AND STERILIZATION Color bands will wear with normal use; simply dispose the tip. Tips last for approximately 5-10 sterilization cycles. Replacement tips are available in packs of 3. For intended use only by dental professionals. Inspect, clean and sterilize before each use. Clean using an ultrasonic cleaner or thermo disinfector. Steam sterilize for at least 5 minutes at 273 F (134 C) or 20 minutes at 250 F (121 C). Do not heat above 302 F/150 C. Use recommended chemicals only. Do not disinfect with phenols. Do not use dry heat or chemiclave. Replacement Parts BLPG tips Papilla tips Part Codes BLPGREF PAPREF PROPORTION GAUGE: The Proportion Gauge has the T-bar tip on one end and an In-line tip on the opposite end. The T-bar tip is used on teeth with normal alignment and the In-line tip is used for crooked or crowded teeth where the horizontal arm of the T-bar is obstructed by the rotated teeth. The instrument is used to measure the width and length of the maxillary anterior teeth. The tip measurements have a predetermined ratio of about 78% (based on the study results on the average width:length ratio of the maxillary anterior teeth.) The gauge assists the clinician in diagnosing tooth size discrepancies that require attention to correct and enhance patient aesthetics. T-bar Tip In-line Tip SOUNDING GAUGE: Helps provide quick and simple analysis of the osseous crest location both mid-facially and interdentally. It is used in aesthetic and periodontal crown lengthening procedures to determine the bone level before reflecting the flap. The present armamentarium for this purpose is a periodontal probe. Sounding Gauge is designed to make sounding easier faster and more predictable.
