1 STATUTORY REQUIREMENTS FOR GENERAL ANESTHESIA /DEEP SEDATION PERMIT # Cases Hours of AL 1 yr advanced anesthesiology training consistent with ADA Guidelines; ABOMS Diplomate / Diplomate eligible or AAOMS member. At the Board s discretion At least 3 trained individuals required: 1, the operating dentist; 2, a patient observer/monitor; 3, an assistant to the operating dentist. AK ABOMS Diplomate; AAOMS Member; 36 month OMS advanced education approved by ADA; certified member of an ADA Specialty Board and meets the minimum requirement in anesthesiology as recommended by ABOMS; 1 yr. Accredited advanced anesthesiology program. Team capable of handling emergencies; permit required to supervise CRNA A dentist administering a general anesthetic may not perform any operative procedures for the duration of the anesthesia. AZ ACLS ; 1 yr advanced anesthesiology training in a program described in regulations, offered by a JCAHO accredited hospital, or sponsored by a university accredited by CODA; ABOMS Diplomate / Diplomate eligible, AAOMS Fellow, ADSA Fellow / Fellow eligible. 12 / 3yrs. All personnel involved in administering and monitoring general anesthesia or semiconscious sedation must hold current certification in CPR;

Operative supervision is required for Deep Sedation and General. 3 individuals required: treating practitioner who may direct the sedation; individual to assist with observation and monitoring and who may administer drugs if appropriately licensed; and other personnel to assist the operator as necessary. Staff monitoring patients at this level must hold a permit as a Sedation Assistant. CA 1 yr. residency program in general anesthesia approved by ADSA; graduate program in OMS approved by CODA. 15 CO BLS & ACLS; 2 yr. residency program in general anesthesia approved by ADSA; post-doctoral training program (e.g., OMS) commensurate with ADA Guidelines. 3 individuals required: supervising dentist qualified to administer anesthesia and 2 individuals, at least one of whom is trained in patient monitoring. CT ACLS; Completion of either ADA approved post-doc course in anesthesiology or OMS, certification from OMS board or designation of practice limited to OMS. Entire staff must be BLCS certified.

At Board s discretion. 6 Auxiliary personnel shall maintain current certification in BLS and be capable of administering BLS. KS Approved course and residency program, advanced OMS program, or 1 year advanced anesth. training consistent with ADA Guidelines. Dentist must provide evidence of having performed minimum of 48 Level II procedures during the 2 yrs preceding renewal.

ME BCLS & ACS or appropriate emergency mgt. course. Appropriate CODA accredited advanced education program commensurate w/ Part IV. C of ADA Guidelines Must employ & document use of continuous ECG monitoring. Must not have more than one patient under deep sedation/general anesthesia and/or moderate sedation simultaneously. MD 1 yr. advanced anesthesiology training; ABOMS Diplomate, ABOMS exam eligible, AAOMS Member, CODA OMS residency completion, ADSA Fellow. MA 1 yr. advanced anesthesiology training; meeting ABOMS Staff certified in CPR
6 requirements or ADSA Fellow. MI ACLS; 1 year advanced anesthesiology training. MN ACLS; CODA program resulting in the dentist becoming clinically competent in the administration of general anesthesia. Program must be equivalent to a program for advanced specialty education in OMS; or 1 yr. residency in general anesthesia at an ASA, AMA, or JCHA certified institution resulting in the dentist becoming clinically competent in the administration of general anesthesia. MS ACLS; Advanced anesthesiology program consistent with ADA Guidelines. Auxiliary personnel must be certified in CPR. MO ACLS; Post-doctoral training program in anesthesia consistent with ADA Guidelines, CODA OMS program, anesthesiology program approved by ACGME or AOA. team members must be certified in CPR, BLS or ACLS. MT ACLS 20 per 3 yrs. With respect to a full general anesthesia facility, in addition to the dentist and dental assistant,
7 there must be at least one person present to monitor vital signs. That person must be either: (i) an anesthesiologist licensed to practice medicine in the state of Montana; or (ii) a certified registered nurse anesthetist recognized in that specialty by the Montana board of nursing; or (iii) a trained health professional who has received at least one year of postgraduate training in the administration of general anesthesia. With respect to light general anesthesia, in addition to the dentist and dental assistant, there must be one person present whose duties are to monitor vital signs. This person must be trained in BLS and their task dedicated to monitoring. NE 1 yr advanced anesthesiology training; ABOMS Diplomat, ABOMS exam eligible, ADSA Fellow. Assistant shall be BLS certified. NV Advanced training in anesthesiology consistent with ADA Guidelines; CODA OMS program. 3 per 2 yrs. Auxiliary personnel must be certified in CPR. NH Advanced anesthesiology training Staff/ as set forth in
8 consistent with ADA Guidelines; advanced anesthesiology training as described in the CODA requirements for each advanced program AAOMS Manual or 2007 ADA Use Guidelines. NJ 3 yrs. postdoctoral training in oral surgery, or a minimum one-year training course in anesthesiology; or Diplomate in oral surgery or is Board-eligible in oral surgery; or ADSA Fellow or AAOMS Member or NJOMS Member. 20 per 2 yrs. Must employ no fewer than two persons who must be present in the office, at least one of whom assists in monitoring. Such personnel shall be certified by the permit holder as being trained in and capable of monitoring vital signs, and of assisting in emergency procedures. NM ACLS; Advanced anesthesiology program consistent with ADA Guidelines; ADA accredited postdoctoral training program (e.g. oral and maxillofacial surgery), affording appropriate anesthesiology training. Any dentist holding enteral anxiolysis (minimal sedation), CSI, CSII, deep sedation and permit at large (AAL) are required to have a minimum of 5 hrs of CE for the permit renewal (every 6 yrs) in medical emergencies, air way management, pharmacology, Auxiliary personnel must have BLS certification.

ND Dental Examiners must be satisfied the applicant is in compliance with ADA policy statement: The Use of Conscious Sedation, Deep Sedation and General for Dentists. PALS is required for administration of general anesthesia and conscious sedation on patients age 8 and under. This requirement is effective immediately for new applicants and effective Dec for current ND dentists as part of licensure renewal. This requirement is found in the ND anesthesia/sedation application. OH ACLS or its age-appropriate equivalent; Accredited postdoctoral training program which affords appropriate anesthesia training; approved ACGME accredited post-doctoral training program in anesthesiology; 1 yr 6 per 2 yrs. Permit holder must agree to have available adequate personnel as recommended in the ADA s Guidelines and/or AAOMS s "Office Evaluation Manual.
10 advanced anesthesiology training from a JCAHO accredited institution that meets the objectives provided in the ADA Guidelines. OK ACLS or PALS; 2 yrs. years advanced anesthesiology training consistent with ADA Guidelines; Specialty certification in OMS; ADSA Fellow. 6 per 3 yrs. 2 assistants involved in the procedure currently certified in BLS. OR ACLS or PALS; Advanced with ADA Guidelines; Completion of an ADA accredited postdoctoral training program which affords comprehensive anesthesiology training; In lieu of these requirements, Board may accept equivalent training or experience in general anesthesia. 14 per 2 yrs. Persons serving as anesthesia monitors shall maintain current certification in BLS/CPR or equivalent. An anesthesia monitor and 1 other BLS certified person shall be present in the operatory in addition to the dentist performing the dental procedures. PA ACLS for adult patients and PALS for pediatric patients; 2 years advanced anesthesiology training consistent with ADA Guidelines; ABOMS Diplomate or ABOMS exam eligible; AAOMS Fellow; ADSA Fellow. 15 Auxiliary personnel who assist the permit holder must be BLS certified. RI ACLS; Advanced anesthesiology training consistent with ADA Guidelines or CODA training At the Board s Discretion. Auxiliary personnel shall hold certificate in BLS.

Dental auxiliaries must be trained and capable of administering BLS. Dentist may not administer general anesthesia or monitor a patient without the presence and assistance of qualified auxiliary personnel. TN ACLS or PALS; 1 yr advanced with ADA Guidelines; CODA OMS residency; ADSA Fellow or 1 yr. General anesthesia residency program approved by ADSA. During deep sedation/general anesthesia at least two (2) persons (certified in Health Care Provider Life Support), in addition to the operating dentist, must be present. Dentists who provide deep sedation/general anesthesia for children must provide evidence of adequate training in pediatric sedation techniques, in general When the same individual administering the deep sedation/general anesthesia is performing the dental procedure, there must be a second (2nd)

WA ACLS; Dentists who began their anesthesia training prior to 2008: 2 yrs continuous anesthesiology training as outlined in part 2 of ADA Guidelines (last revised October 2005). Dentists who begin their anesthesia training in January 2008 or after: CODA dental anesthesiology program; DQAC approved anesthesiology program; or 2 yr ACGME accredited. New OMS applicants must be: ABOMS diplomate, AAOMS fellow or CODA accredited OMS program grad. 18 per 3 yrs. AED or defibrillator must be within reach within 60 seconds from any area where general anesthesia/deep sedation is delivered. Person administering anesthesia and person monitoring patient may not leave immediate area during deep sedation/general anesthesia. Recovery: patient must be continually observed by anesthesia provider or credentialed personnel. WV ACLS; Advanced anesthesiology training consistent with ADA Guidelines; ADA or AMA accredited postdoctoral training program which affords comprehensive and appropriate anesthesiology training; Board may also accept documented evidence of equivalent training or experience in general anesthesia. 16 hrs. The dentist must use an assistant qualified as a monitor to monitor and record the patient's condition. The trained personnel must have a certificate showing successful completion in the last two years of BLS/CPR training and the AAAOMS Office Assistant certification or an equivalent. WI 1 yr advanced anesthesiology training in a program approved by the council on dental education of
