Congratulations and Welcome This publication is designed to give a detailed overview of the Teeth on Implants process offered at Smile Artistry. We invite you to come in for a consultation and make an informed decision with the appropriate assessment of your individual circumstances. This booklet is intended to give you helpful information to help you understand this dental treatment, however it is no substitute for seeking a professional opinion from an appropriately Qualified Dental Practitioner. The results discussed in this publication will vary with different individuals and invasive procedures discussed in this publication do have risks and complications. Not everyone is suitable for treatment, so we do recommend you visit a Dental Practitioner to determine if this treatment is suitable to you, along with consideration for the other options that are available. Our Treatment Co-ordinators will be able to offer you further advice and answer your questions. Our Information Seminars are available for those who are considering the Teeth on Implants treatment and you will be able to get answers to your questions at these free seminars. 4 The Past For decades, dentures have been the first choice if you are unfortunate enough to lose many adult teeth. Some patients love their dentures, but some experience a distinct change in the quality of their life and enjoyment due to the lack of chewing, aesthetics and psychological changes. For patients who want the option of having fixed-in teeth again, Smile Artistry can create functioning teeth for you in days, giving you your mouth and smile back with modern day Dental Implants. The Future The future is now, aided with Australia s high standard of dental care, technology and techniques which use the latest in CAD-CAM technologies for safer, pain-free, beautiful and predictable results. At Smile Artistry, our Dentists are trained and qualified to give you a fully functional set of new teeth, both quickly and safely, following the loss of your adult teeth, allowing you to have a second chance at smiling with confidence* again. *Smiling with confidence refers to being able to smile with the confidence your dentures will not fall out of your mouth 2

Dental literature displays denture-related problems such as: Eating Chewing Problems Psychological Problems Pain and Discomfort Problems Loose, moving, food catching underneath are all often attributed to dentures. 1,7 With Teeth on Implants you will be able to eat and enjoy your foods again. Feeling unhappy and losing confidence has been linked with wearing dentures. 2 With Teeth on Implants you will have teeth that look natural, beautiful and compliment you and your personality, making you feel comfortable to smile, talk and laugh again. No teeth, missing teeth or unhealthy gums can cause pain and discomfort. With Teeth on Implants a healthy mouth will help reduce the pain and sensitivity caused by gum disease and gum recession. 3 4 Are you suitable? Do you relate to any of the listed problems above? Everyone is different and everyone faces different problems when it comes to wearing dentures or having no, missing or unhealthy teeth. However if you suffer or can relate to any of the problems listed, then seeking advice regarding Teeth on Implants could help you.

Gum Disease Problems Removable Denture Problems Bone Loss Problems Speech Problems Advanced gum disease results in unhealthy, loose and uncomfortable teeth and bone loss and ultimately could affect your general medical health. With Teeth on Implants your gum disease risk factors can be improved. Dentures need to be removed for cleaning, they get larger as your gums continue to shrink making them become looser. With Teeth on Implants the pressure from our teeth is better transferred to the jaw than a denture would allow, effectively assisting the jaw to remain the same in shape and size. 4,5 Missing teeth results in poor bone cell stimulation through under-use, resulting in bone loss. With Teeth on Implants the jaw is once again stimulated thus assisting in maintaining jaw shape and size. 88% of people who wear removable dentures complain of speech problems. 5,6 With Teeth on Implants the permanently fixed-in teeth will give you the most natural possible teeth contour, improving your speech and helping you talk normal and naturally again. 8 Your aesthetic result is customised to endless lengths to achieve what you consider you 6

Teeth on Implants can give you a functional set of fixed-in teeth in days. In most cases, teeth removal and implant placement happen on day one (after prior teeth impressions, and shape mock-up). Provisional teeth are placed on day two or three, giving you functional teeth during the healing phase. Your final porcelain teeth will be delayed until the healing period is complete when your jaw bone has osseo-integrated with your Dental Implants. This means the bone has fused and grown onto the surface of your Dental Implant. So, following the final teeth, you ll have beautiful natural looking teeth, allowing you to smile and eat normally again. The Benefits Not only do Teeth on Implants act like real teeth, but other benefits include: Maintenance of bone Maintenance of muscle tone in your face and jaw Improved aesthetics Improved speech Improved bite and bite sensation Improved your ability to eat and chew Your facial expressions improve, become more normal Your teeth become more stable Your teeth are not removable, they are a fixed-in replacement Your health and wellbeing are improved So not only are you getting an overall health and psychological improvement, but you are going back to the most teeth-like feel you can get following tooth loss. Whilst Teeth on Implants does vary slightly to that of regular teeth, your aesthetic result is customised to endless lengths to achieve those perfections and imperfections that you consider you. 7

What is a Dental Implant? A Dental Implant is a screw that is placed into the bone in your jaw that acts like a root of a tooth. It is the anchor that a porcelain crown is then placed upon. Teeth are securely fixed to your Dental Implant, and only your Dentist will remove your teeth for cleaning, repair or replacement. The Dental Implant is the foundation, the part you do not see. Dental Implants are stronger than natural teeth, made from hospital grade titanium, they are very efficient at transmitting force to your bone just like natural teeth roots do. Therefore, your chewing ability is equal to or greater than natural teeth. Some dentures produce as little as only 5.6 psi maximum chewing force, whereas teeth produce a force of 200 psi. 8 Dental Implants can produce a force similar too or greater than teeth, therefore you will be able to bite into that apple, or chew that steak again. Reference 8: Carr A, Laney WR: Maximum occlusal force levels in patients with osseointegrated oral implant prostheses and patients with complete dentures, Int J Oral Maxillofac Implants 2: ,1987miol 26: ,1998 The Dental Implant is the foundation 8

What is Teeth on Implants? The Teeth on Implants process involves placing a number of Dental Implants into your jaw then placing a permanent full mouth bridge onto these Dental Implants, creating a fully functional set of teeth on your upper, lower or both jaws. Giving you teeth again and giving you the ability to eat, chew and speak freely again! 9

Case Studies performed at Smile Artistry Case One This lady had no teeth and had been wearing dentures for many years. Unhappy with her smile and the problems she had associated with her dentures, she came to Smile Artistry in search of a fixed-in teeth solution. The Teeth on Implants process was used to give her a fully functional bridge of teeth on both her upper and lower jaws. 10

Case Two This middle-aged male had severe breakdown of his remaining teeth and was severely debilitated and unable to eat properly. The problem was compounded by a loose denture which held bacteria, plaque and food against his remaining teeth and gums. Removal of his unhealthy teeth and replacement with the Teeth on Implants process was used to give this gentleman a fully functional mouth and the confidence to smile without the fear of his teeth (denture) falling out. A second chance at smiling again 11

The Process The first step for your Teeth on Implants treatment is your personal consultation with our Implant-accredited Dentists. During this consult your Dentist will look at your mouth, assess your suitability for Teeth on Implants and ensure you require such treatment and that other options are not more appropriate. Initial photos will be taken of your mouth, scans and X-rays will be taken, all to ensure you are suitable and that this process is the best for you. Following your consent to proceed with treatment after cost and timeframes have been agreed, Impressions will be taken and a wax up of your teeth will be created to design your final teeth shapes and positions. Time frames During the Teeth on Implants process, you will not go without any teeth in your mouth (except the first day or two). On the first day, assuring you have enough bone in your jaw your Dental Implants will be placed into your mouth. The following days we will insert your temporary teeth into your mouth. These teeth will stay in your mouth until your Dental Implants have healed and osseo-integrated with your jaw bone. They will look and feel natural and act like normal teeth, however you will be advised to have a lighter, more softer diet than usual during the entire healing months. After a short few months (between three to six months), your Dentist will create your final porcelain teeth. At this point your temporary teeth will be removed, and your new porcelain teeth will be inserted. These teeth are designed just for you, their fit, shape and colour are all personally tailored to suit your mouth and your smile. They will be determined and duplicated from the temporary teeth, so it is important that the temporary teeth are made to be as accurate as possible. 12

Consultation Analysis Planning Implant Surgery During this consult your Dentist will look at your mouth, assess your suitability for Teeth on Implants and ensure you require such treatment. Initial photos will be taken of your mouth, scans and X-rays will be taken all to ensure you are suitable and that this process is best for you. Next, Impressions will be taken and a wax up of your teeth will be created to design your final teeth. Removal of your teeth, placement of the Dental Implants and impressions are taken along with matching the teeth position with your smile. Delivery of Final Teeth Healing of Jaw Bone Provisional Teeth Each individual step is essential for creating your perfect smile At this point your temporary teeth will be removed, and your new porcelain teeth will be inserted. These teeth are designed just for you, their fit, shape and colour are all personally tailored to suit your mouth and your smile. Healing is dependent on individual circumstances but usually ranges from three to six months. Computer milled supersmooth temporary teeth are delivered two days later and will be worn for the duration of healing (three to six months). These are fixed teeth. 13

Technology Having the right experience, team and technology ensures that your Teeth on Implants experience, is not only comfortable but you will end up with a smile that you want and looks natural. Advances in Technology IV Sedation Intravenous sedation (I.V. Sedation) allows for larger treatments to be performed efficiently in the one appointment. A Specialist Medical Anaesthetist will provide you with safe and deep I.V. sedation that removes all anxiety and allows you to feel relaxed but conscious enough for our team to not only place your Dental Implants, but perform all other procedures to make your express teeth. Computer Planning Dental Implants must be placed accurately and precisely into your jaw, and computerised implant placement allows our team to place your Dental Implants into the more suitable areas of bone which relate appropriately to the positions of your final teeth. This computer technology allows planning for any bone grafting procedures that may be required without surprises during surgery. Zirconia CAD-CAM Machines To ensure your teeth look, feel and function as natural as possible, Porcelain-Zirconia teeth and gums are our choice material. Using Porcelain-Zirconia allows teeth that look natural, have the added strength to withstand eating and chewing forces and are cost efficient over a longer period of time than traditional methods. Zirconia forms the base of the tooth due to its superior strength and extreme accuracy when milled by CAD-CAM technology, then porcelain is layered over the front surface of the teeth and gums (pink porcelain). This type of restoration, is a non-metal restoration. 14

Options with Costs Previously, full dental implant replacements took multiple stages and surgeries and became quite costly. With modern-day techniques, equipment and materials, fixed dental implant restorations can now be completed at significantly lower costs. Teeth on Implants is a long-term solution, having a life span of 20 years or more*. Replacing your dentures or broken teeth with beautiful, natural looking and fully functioning teeth may be more costly initially than a removable denture, but less costly than having individual dental implants for each tooth. Teeth on Implants is a staged treatment and cost can be spread over the treatment phase. An initial fee for dental implant placement and temporary teeth is required, then the next treatment cost is usually about six months later when your final Porcelain Zirconia bridge teeth are inserted. When comparing treatment costs, be sure to compare the Smile Artistry Teeth on Implants treatment, which uses Porcelain Zirconia Bridge Teeth to similar materials. Other materials may use alternatives such as welded metal bars with plastic-resin teeth, which will wear down and require updating more frequently. After your 40 minute Consultation with our Teeth on Implants Dentist, you will be given a personal plan with costs, time-frames and expected treatment outcomes. *Most Zirconia teeth have a lifespan of 20 or more years before revisiting them, though this is independent on individual cases and cannot be guaranteed. When we need to remake your Porcelain Zirconia teeth, a replacement cost of the bridge alone is required, this will be less than the upfront costs to commence treatment from the beginning. 16

Other Options Retaining Good Teeth and adding some Implant Teeth When some teeth are in a good condition and have good prognosis for years to come, they should be retained and only the missing teeth be replaced with Dental Implants. Dental Implant Retained Denture If you are happy with dentures, but just want improved retention and stability, then using a few Dental Implants with special attachments can stabilise and hold your denture in position. You will still have to take your denture out daily for cleaning, however you can relax that your denture won t fall out unnecessarily. The Dental Implants can often be upgraded to fix-in teeth at a later date when additional implants are placed. A Five Plus Year Warranty! see conditions in Disclaimer 17

Risks and How to Reduce Risks Placing implants comes with risks, many identified risks that have surfaced in the years since Dental Implant placement started have been: RISK 1: Spans of teeth with no implants under them will either stress the bone around the nearest implants causing bone loss, or the bridge teeth may flex and may break RISK 2: If too few implants are placed then, this will stress the bone around the existing implants which can cause bone loss and ultimately possible implant or teeth loss RISK 3: If the implants used are too small, the end result is that these smaller Dental Implants are not sufficient enough to create a large enough contact bone area to transmit forces, causing stress again on the bone, therefore leading to bone loss and implant/teeth loss. RISK 4: Wrongly positioned implants occur often due to non-computerised planning but in most cases can now be overcome with zirconia bridges Risk 1 Risk 2 Risk 3 Risk 4 18

RISK 5: Early infection and loading protocols in inappropriate situations can cause catastrophic failure in the first month or two and result in further surgery and replacement Dental Implants RISK 6: Dental Implants are damaged, break or parts are no longer available is overcome by using the best brands of Dental Implants which have undergone long-term review and proven outcomes. Our solutions at Smile Artistry to help overcome and minimise these risks is to create an appropriate and individualised plan using the right number of Dental Implants, using CAD-CAM technology to ensure accurate Dental Implant placement and using sinus grafting where needed to help increase bone to areas where there may be increased force in the upper jaw. For patients with minimal gums, we create their gum lines using pink porcelain to make their smile as natural and as beautiful as possible. 22 20 Rebuilding missing bone is an important step

Bone Grafts For a Dental Implant to be successful you need to consider the bone taking the implant in two regards: dimension and density. After a tooth is lost, the jaw bone around that area tends to fade away from a lack of stimulation. This stimulation occurs normally from the tooth when we chew and eat. Dentures can also pathologically overload areas of your mouth causing further bone loss in denture resting areas. Therefore, bone grafting can sometimes be needed when you have excessive bone loss, in particular when you have lost a tooth for a period of longer than six months. A bone graft is a process used to rebuild bone in deficient areas using a substitute bone. The aim of a bone graft is to rehabilitate bone to its intended profile and use creating new jaw bone alike its original form. The bone graft is placed in the deficient area, and then over time the natural bone adjacent will grow through the graft and become natural jaw bone. Different types of bone grafts can be performed depending on the deficient bone areas. These are: Socket bone grafts This is where the graft bone substitute material is placed into the tooth socket, where the teeth roots are removed from Sinus bone graft Used to build up the upper back tooth region when the sinus area pushes down upon the jaw following teeth removal Bone graft Block This type of graft is used when needing a large area of bone. Blocks are usually taken from the wisdom tooth region of the lower jaw and placed in the larger areas of missing bone Particulate Bone graft This type of bone graft is used to help build up smaller areas of bone. Particles of bone grafts are placed in defects to help build and repair areas of bone loss 21

Bone Mechanics In order to provide a long lasting, predictable result bone mechanics must be taken into consideration: Bone Depth Dental Implants with a greater length are more predictable and stable. If bone is not 12mm in depth, then additional bone grafting or additional implants may be required 5 Bone Stimulation It takes time for bone to attach and strengthen around Dental Implants, so your permanent teeth should not be placed initially. Bone responds to stimulation and the healing phase is a delicate balance of not overloading the bone around a Dental Implant. Therefore it is best to wait a duration of time, such as three to six months before final teeth are placed. Sometimes delaying or progressively creating the load placed upon the Dental Implants may help create a strong and healthy Dental Implant base 5 Upper Jaw Density The upper jaw is less dense in comparison to the lower jaw. Therefore when creating Teeth on Implants for the upper jaw, more implants are usually required to create a long lasting and predictable result 5 Bone Stress When we bite, chew or grind we cause stress in five major locations of our jaw: The right side molars, the right side canine, the incisors, the left side canine and the left side molars. So when we position the Dental Implants, these stress points must be addressed as key pressure areas 5 Denture wearers Wearing a denture can cause bone loading and accelerate bone loss over time. Dentures will eventually become loose and uncomfortable and relines may need to be performed annually with new dentures made every five years 5 Reference 5: Misch CE: Contemporary Implant Dentistry 3rd ed: Mosby Elsevier, 18-19,

Smile Aesthetics A beautiful smile can be described as a presentation of the lips, teeth and gums that are in perfect harmony with the surrounding facial features, lips and muscles. All smiles need to be personalised and individualised to ensure they are in perfect harmony with your facial features, lips and muscles. Smile Principles with Teeth on Implants: Lip support Ensuring lips have even support throughout your smile Teeth length Making sure the correct length of teeth are displayed while smiling, laughing, talking and opening your mouth Speech Teeth positioning is very important and vital for correct speech. Your smile will be created to ensure that your teeth are correctly spaced and positioned for accurate speech and language Determining the amount your gums show during smiling is an essential element in creating a beautiful smile. Hiding the junction between the bridge teeth, pink gums and the natural gums also creates a beautiful natural looking smile. Usually removing the need for a denture, also creates a more aesthetic appearance of the gums and lip support due to removing the flange of the denture, allowing the lip to sit naturally. Colour matching is an important aspect when creating a smile to match individuals. Therefore it is important that your Dentist works closely with the dental laboratory during the colour matching process this is critical to get the right match. 25
