1 ASSESSMENT AND MANAGEMENT OF THE DENTAL PATIENT UTILIZING SEDATION AND ALTERNATIVE ANALGESIA STRATEGIES SATURDAY, APRIL 14, 2018 Sponsored by the University of North Carolina Department Oral and Maxillofacial Surgery LOCATION: The UNC School of Dentistry

2 Course Description This course is designed for every member of the team providing sedation for the dental patient. The history of anesthesia will be reviewed with emphasis on dental office anesthesia and recent morbidity and mortality information. The North Carolina State Board of Dentistry sedation regulations will be discussed with emphasis on how they affect the role of each sedation team member. The anatomy and physiology of the respiratory, cardiovascular, neurologic, endocrine, gastrointestinal and musculoskeletal systems will be described. The pre-treatment assessment of each patient will be reviewed with emphasis on determining the appropriate patient for a safe and predictable sedation in the dental office setting. The fact that the pediatric dental patient posses several challenges including different anatomy, physiology and psychological responses, will be highlighted. The appropriate equipment and staff required when providing each of the levels of the anesthesia will be described, including how the required monitors function and how to interpret the information they provide. Current medications used during dental office sedation will be described in detail including their routes of administration, dosage, potential interactions, risks and side effects. Complications that can arise during office sedation will be discussed, with emphasis on early recognition and management as an efficient team. Various complication scenarios will be reviewed. Post-treatment monitoring and discharge criteria will be described. Currently available modalities designed to minimize or eliminate the need for opioid analgesia will be discussed including research projects being undertaken at the University of North Carolina department of Oral and Maxillofacial Surgery. The course will satisfy the 6 hour requirement for sedation related CE for a sedation permit holder and the 3 hour requirement for auxiliaries involved in providing sedation. This course will also fulfill the North Carolina General Assembly s requirement for one hour of CE related to opioids. Educational Objectives Upon completion of this course, participants should be able to: 1. Assess patients from a clinical and medical standpoint, so as to make a determination if a patient is a candidate for office sedation. 2. Recognize the anesthetic agents that are currently available from the standpoint of their pharmacology, mechanism of action, method(s) or routes of administration, interactions with other medications and potential side effects. 3. Develop an anesthetic plan based on the patient s status and the treatment being performed. 4. Determine the appropriate use and interpretation of various monitors utilized during the administration of office sedation. 5. Identify the roles each team member has when providing an efficient and safe office sedation. 6. Complete appropriate documentation pre-treatment, during the time sedation is provided and during the post-treatment phase. 7. Manage complications that may arise during or after sedation. 8. Provide patients with adequate alternative analgesia to opioids.

3 About the Speakers Barry D. Kendell, DMD, MS is currently Adjunct Professor in the Department of Oral and Maxillofacial Surgery at the University of North Carolina at Chapel Hill. He is a native of Canada, where he received his undergraduate dental training at the University of Manitoba. Dr. Kendell served in the Canadian Armed Forces as a Dental Officer for 5 years and then received his Oral Surgery diploma and master s degree at UNC. Dr. Kendell is board certified, a member of the American Association of Oral and Maxillofacial Surgery, the North Carolina and Southeastern Society of Oral and Maxillofacial Surgeons and was in the private practice of OMFS for 27 years in the Chapel Hill, Durham and Sanford area. He has been an examiner and section chair for the American Board of Oral and Maxillofacial Surgery for 10 years. He has published 5 book chapters and several journal articles as well as lectured in British Guiana, Argentina, England, Canada and the United States. His current focus of interest is with residents in OMFS in the area of outpatient anesthesia, pain control and office emergencies. Glenn J. Reside, DMD, MS is a Clinical Associate Professor in the UNC School of Dentistry Department of Oral and Maxillofacial Surgery and is the Director of Dental Student Oral and Maxillofacial Surgery education and the primary faculty for dental implantology in the Oral & Maxillofacial Surgery Residency training program. He earned a BS degree in Chemistry at Illinois College, a MS degree in Biochemistry at the University of Illinois, and a DMD degree at Washington University in St. Louis, Missouri. Immediately after college, he entered military service and became a Special Forces Officer assigned to Vietnam. Upon returning to the United States, he went to graduate school and dental school and then completed a General Practice Residency at Womack Army Hospital, Fort Bragg, NC and an Oral & Maxillofacial Surgery Residency at Brooke Army Medical Center, San Antonio, TX. He is a diplomat of the American Board of Oral and Maxillofacial Surgery and a fellow of the American Association of Oral and Maxillofacial Surgeons. Raymond P. White, DDS, PhD is the Dalton L. McMichael Distinguished Professor of Oral and Maxillofacial Surgery at the University of North Carolina. He served as Dean of the UNC School of Dentistry from 1974 to 1981 and Associate Dean of the UNC School of Medicine from 1981 to Before coming to UNC, Dr. White was Chairman of the Department of Oral and Maxillofacial Surgery at the University of Kentucky, and Professor of Oral and Maxillofacial Surgery and Assistant Dean for Administrative Affairs at the Medical College of Virginia, Virginia Commonwealth University. Dr. White completed his surgery residency, his D.D.S. and his Ph.D. at the Medical College of Virginia after finishing a pre-med curriculum at Washington and Lee University. Dr. White has received numerous awards and honors including: the Distinguished Service Scroll Award and the Meritorious Service Award from the North Carolina Dental Society, the John C. Brauer Award from the University of North Carolina Dental Alumni, the William Gies Award and the Distinguished Service Award from the American Association of Oral and Maxillofacial Surgeons, the Research Achievement Award and the Torch Award from the Oral and Maxillofacial Surgery Foundation, the W. Harry Archer Award from the American College of Oral and Maxillofacial Surgeons, and the Southeastern Society of Oral and Maxillofacial Surgeons Distinguished Educator Award and the Lifetime Achievement Award.

4 He was elected to the Institute of Medicine (now the National Academy of Medicine) of the National Academy of Sciences in Dr. White has been Assistant Editor of the Journal of Oral and Maxillofacial Surgery and Co-editor of the Journal of Adult Orthodontics and Orthognathic Surgery. His peer reviewed publications span his academic career of over 45 years. Dr. Ray White is an experienced clinical investigator who has focused for most of his career with Dr. William Proffit on treatment of dento-facial deformity. Currently Dr. White was principal investigator of a series of clinical trials designed to fill gaps in our knowledge of third molar management sponsored by the Oral and Maxillofacial Surgery Foundation, the American Association of Oral and Maxillofacial Surgeons, and UNC Oral and Maxillofacial Surgery. This project has led to 130 peer reviewed publications split evenly between abstracts and papers. Currently Dr. White is engaged in a series of clinical studies addressing innovations with local anesthesia drugs and clinical options to reducing the use of opioid drugs for post-procedure acute pain. Registration Fees The registration fee is $500 for dentists and $300 for team members. The registration fee includes course materials, CE credit, all meals included on the agenda and a parking voucher.
