2 Preface Dental Fees 2016 Preface DENTAL FEES SURVEY AUSTRALIA 2016 This report presents the findings from the survey carried out by ACA Research between July and September 2016 for the Australian Dental Association Inc. The survey relates to the dental fees being charged by Association Members in Private Practice. The present publication provides a comprehensive summary of 2016 dental fee levels for General Practitioners and Specialists by State, and for differences in fee levels between State Capital and Rest of State areas, as at 1 July, The findings in this report will be made available to Association Members on their website ( A summary of the principal findings will be published in the December issue of the News Bulletin. The samples used represent ADA Members who are active in the private practice sector. A total of 10,397 dentists were invited to participate in the survey online. All the dentists were also informed that if they were using dental4windows or PracticeWorks practice management systems, they could use the software facility De-Identified Data Capture (DeIDC) which automates responses to the annual Fees Survey. A total of 4,180 dentists responded to the invitation to participate in the online survey however only 2,811 dentists, who completed the survey with valid responses, were included in the analysis of the report. Dentists were asked to list the fee they most commonly charged for each of 119 of items of service. The items of service included the types of services commonly performed by dentists in Australia, such as oral examinations, calculus removal and various restorative services. There were also some items that represented less frequently performed, but more specialised services, including orthodontic items. In order to eliminate extreme outliers, detailed data was calculated for each item of service in this report including minimum, maximum, median and mean fees using the central 90% of fees charged.

5 Introduction 5 Dental Fees Introduction The Association has conducted Dental Fees surveys of its members seeking the usual fees charged for selected items of service. The results from these surveys provide information on the landscape of the profession and are used as a strong information base for the Association when undertaking advocacy activities with the Federal Government. Dental Fees Surveys of ADA Members, active in private practice, have been carried out since 1961 and standardised from Since 1995, the surveys have been carried out each year by an independent market research firm, ACA Research. Items to be surveyed each year have been approved by the ADA Federal Executive. Australian dentists in private practice are free to negotiate the fees that will be charged for the services that they render to patients. The majority of dentists charge for services on a fee-for-service basis and have their own level of fees for the services they usually perform. They may vary a fee depending on the unexpected difficulty encountered, ease of execution, the patient s ability to pay, patient compliance, or when the service is carried out concurrently with other items of service. For some items the final fee charged depends in part on the dental laboratory charge and/or the cost of materials. Few dentists charge an hourly or sessional rate (with/without additional laboratory charges) but many take the time factor into account when they set their fee for some items of service. 6 General Practitioners 6 Dental Fees Method 2.1 Data Collection The design, implementation and analysis of the ADA Dental Fees Survey has been standardised since 1980 and the survey has a high level of acceptance by Australia Dental Association Members. The survey was conducted by means of an online questionnaire directed to active members of the ADA in the private practice sector. An invitation letter was ed out to all 10,397 dentists in private practice as listed on the ADA member database. The invitation letter contained the survey link that the dentist could follow to complete the survey online. This year communication to the dentists has been updated. At invitation, the dentists were presented with three options. The first option was to complete the survey, the second option was for dentists who were not sure if they would qualify and the third option was for dentists to decline participation in this year s study. This has led to a positive increase in terms of dentists responding to the survey. For those dentists who declined to participate in the survey, they were also given the opportunity to voice their feedback regarding the survey. The dentists were also informed that if they were using Centaur Software s dental4windows or Carestream Dental s PracticeWorks practice management systems, they could use the software facility De-Identified Data Capture or DeIDC which automates responses to the survey. A total of 10,397 dentists were ed the survey with a breakdown as below: State Number of Dentists NSW 3085 VIC 2733 QLD 2130 SA 697 WA 1371 TAS 152 ACT 168 NT 61 Total 10,397 The information sought related to usual fees charged for a selected list of items of service, as at 1 July Confidentiality was maintained as the individual results are not linked to either the dentist or location of practice. For those dentists who agreed to have their previous year answers prepopulated, their information was displayed to review, edit and submit the results on a single screen saving both time and effort. The first invitation was sent out to all General Practitioners and Specialists in late July Then there were three follow-up s sent out to those dentists who had not completed the survey during the 8 week period in August and September. The first follow-up was sent on the 18 th of August, the second on the 5 th of September and final follow-up was sent on the 15 th of September A total of 4,180 dentists responded to the invitation to participate in the online survey. The total response rate is 40%. However only 2,811 dentists completed the survey with valid responses and only the responses from these dentists were included in the analysis of the report. In comparison to 2015, it is positive to note there has been an increase of 458 responses by dentists included in the analysis this year.

The valid survey response rate is 27%. The results were then weighted to reflect the actual proportion of dentists in each state, as well as weighting for each dentist in the practise charging the same fees. The total number of respondents is adequate to determine reliable estimates of fee levels. The complete and valid response covered 2,811 members including 222 respondents submitting the results using the DeIDC software facility. This response has enabled valid use of average fees charged for comparisons between States, from 2015 to 2016, and for supportive evidence in the negotiation of dental fee levels with other organisations. The total responses enable valid inferences to be made concerning fees charged by dentists in Australia. The large sample enables valid comparisons to be made interstate and between State Capital and Rest of State. The overall response rate and the valid number of responses included in the analysis by State and Type of Practice for 2016 is detailed below.

9 General Practitioners 9 Dental Fees Analysis of the results Dentists submitted the data directly via an online survey. As in previous years, a number of very high or very low values were checked and it would be expected that any significant dentist errors would be eliminated by using only the central 90% of data. Reports are generated for the General Practitioners group at State, State Capital and Rest of State level, and for the Specialist group at the State level. They contain the sample size numbers and statistical numbers, including the range (minimum 5% and maximum 95%), mean, standard deviation and median 50%. It should be noted that the classifications for General Practitioner and Specialists were selfassigned by the dentists in all states. Postcodes were used to determine State Capitals and Rest of State. For ease of analysis in this report we later refer to State Capital as Metro and Rest of State as Regional. The appendix tables in this present report show the mean fees for each item of service for the central "90% - 0's" responses by State. It should also be noted that annual fee variations are often larger amongst the smaller states due to reduced sample sizes. A total of 119 items of service were selected and approved by the ADA Federal Executive for the 2016 survey. Two items were removed from the 2015 list for research this year including Item 281 Course of nonsurgical periodontal treatment and Item 282 Continuation of periodontal treatment or maintenance subsequent to item 281.

10 General Practitioners 10 Dental Fees Dental Fees - General Practitioners 3.1 Response Approximately 89% of the active private practice sector dentists, in Australia, are General Practitioners approved by the Dental Board of Australia. The General Practitioners responses have been separated from Specialists and the summary in this section comments on all items included on the questionnaire. A comparison is made for State Capital and Rest of State General Practitioners. In 2016 a total of 3,387 General Practitioners responded to the survey including 2,552 who completed and returned the survey. This represents a 36% response rate, from the 9,322 questionnaires ed to members. In comparison to the 2015, it is positive to note that an additional 450 GPs completed this survey. It is noted that a third of General Practitioners also provided some Orthodontic item services. 11 General Practitioners 11 Dental Fees 2016 The table indicates, for individual items of service, the percentage of respondents who indicated that: they did not provide the service; provided the service at $0 fee; or provided the service at an hourly rate. It is possible that some $0 fees were taken in conjunction with other items or some $0 fees were recorded in error. The last two columns of the table show the number and the percentage of respondents who provided an item of service and charged a fee. These latter responses have been analysed to determine the mean fees. Similar to previous years, very few General Practitioners indicated that they used an hourly rate for the different items of service. A few items such as Item 018 Written report, Item 061 Pulp testing per visit, Item 072 Photographic records, Item 131 Dietary advice, Item 141 Oral hygiene instruction, Item 221 Clinical periodontal analysis and recording, Item 411 Direct pulp capping and Item 741 Adjustment of a denture were more likely to have a $0 fee. 15 3.3 Variation of Fees - General Practitioners The following table provides a summary of General Practitioner details for individual items of service that are contained in Appendices 1-3 and 5. Firstly, the table indicates the low (minimum 5%) and high (maximum 95%) $ fees for the Australian sample. The next column shows the mean fees for 2015 and 2016 with the % yearly increase. The state variation is shown by listing those with the lowest and highest mean fees for each item. A comparison of fees charged by State Capital versus Rest of State General Practitioners is also included. 21 Discussion 21 Dental Fees Discussion - General Practitioner Fees State Differences As for previous surveys, there were considerable variations in the levels of fees charged both within and between states. Overall, General Practitioners in South Australia were found to have charged the lowest fees. The ACT and Tasmania were reported to charge higher fees compared to other states. The following table displays the proportion of the highest and lowest fees as charged by individual states for the various services surveyed. At an overall level, General Practitioners in State Capital have charged higher fees in comparison to their counterparts in the Rest of State In over half of the 119 items surveyed (n=72), General Practitioners in State Capital have charged higher fees in comparison to their counterparts in the Rest of State with differences ranging from 2% higher (e.g. for Item 161 Fissure and /or tooth surface sealing per tooth) to 31% higher (e.g. for Item 666 Prosthesis with metal frame attached to implants fixed per arch). There are 47 items that were charged at a slightly higher fee in Rest of State areas with the differences ranging from 2% to 10%. Please note the small sample sizes for Rest of State areas of Northern Territory in the Appendices A3.2 and A3.3.

22 Discussion 22 Dental Fees Change in Fees The fees charged by General Practitioners have increased by 3% from 1 July 2015 to 1 July 2016 across 119 items surveyed. Over the last 10 years the change in fees has shown a steady trend upwards, in line with the CPI. The accumulative year-on-year increase of fees over the last 5 years is 11%. The increases in fees by categories, in past surveys, over the last 10 years (since 2006) are summarised in the table below. On average, General Practitioners in Tasmania charged the highest mean hourly rates at $571/hr, whilst their counterparts in South Australia charged the lowest mean hourly rate of $399/hr.

23 Discussion 23 Dental Fees 2016 Diagnostic Services Almost all General Practitioners provided Diagnostic services for most items. The two items that are least provided by General Practitioners are Item 037 X-Ray panoramic (OPG) and Item 047 Saliva screening tests. Diagnostic Service items had an average fee increase of 1.8% since In the majority of the items, General Practitioners in the State Capital tend to charge less than their counterparts in the Rest of State areas. Preventive Prophylactic and Bleaching (PPB) Services Over 80% of General Practitioners provided Preventive services and almost half of them provided Item 131 Dietary Advice and Item 141 Oral hygiene instruction for a $0 fee. Preventive Service items had an average fee increase of 2.2% since In general, General Practitioners in State Capital charged higher fees than their Rest of State counterparts. Compared to other states, General Practitioners in South Australia charged the lowest fees for the majority of these services. Periodontics Approximately three quarters (78%) of General Practitioners provided Item 213 Treatment of acute periodontal infection per visit and charged a fee while only 31% of General Practitioners offered and charged a fee for Item 232 Periodontal flap surgery services. There has been no differences to the fees charged for Periodontics services since July 2015 except for Item 222 Root planning and curettage per tooth reported a decline of 11% in fees. For the majority of Periodontics items, General Practitioners in State Capital have charged higher fees in comparison to Rest of State GPs. Compared to other states, GPs in Western Australia charged higher fees for Periodontics services. Oral Surgery Almost eight in ten General Practitioners provided and charged a fee for Oral Surgery services. Fees for Oral surgery services have increased by 3% since July South Australia General Practitioners charged the lowest fee for all items in Oral Surgery. Except for Item 386 Splinting of displaced tooth/teeth per tooth and Item 392 Drainage of abscess, General Practitioners in State Capitals charges slightly higher fees for Oral Surgery than those in Rest of State areas. Endodontics With the exception of Item 411 Direct pulp capping, Item 414 Pulpotomy, Item 451 Removal of root filling Per canal and for Item 455 Additional visit irrigate/dressing root canal sys per tooth, at least 80% of General Practitioners provide Endodontic services that incur a fee. There has been an increase in fees by an average of 2.8% for Endodontic services compared to General Practitioners in ACT generally charged the highest fees for Endodontic services whilst fees were lower in South Australia.

24 Discussion 24 Dental Fees 2016 On average General Practitioners in State Capital areas charged more for Endodontic services than those in Rest of State areas. Restorative Services Eight out of ten General Practitioners indicated that they performed Restorative services. Fees decreased by an average of 2.3% for restorative services compared to There has been not much differences in fees charged (Less than 2%) between General Practitioners located in the State Capital versus Rest of State areas. Metallic Restorations On average fees for Metallic Restoration services have increased by 3.2% compared to General Practitioners in South Australia and Western Australia charged the lowest fees for Metallic Restoration services while dentists in ACT and Tasmania charged higher fees. Approximately half of all General Practitioners provided Metallic Restoration services for a fee. Adhesive Restorations There has been an average of 2.4% increase in fees charged for Adhesive Restorations since July Overall, General Practitioners in South Australia and Western Australia charged the lowest fees for Adhesive Restorations, whilst Tasmania and the ACT charged higher fees. Approximately 80% of all General Practitioners provided Adhesive Restoration services for a fee. Tooth-Coloured Restorations Fees for Tooth-Coloured Adhesive Restoration services have slightly decreased by an average of 2.2% since Approximately 40% of all General Practitioners provided Tooth-Coloured Adhesive Restoration services for a fee. General Practitioners in Tasmania are charging higher fees than other states whilst those in South Australia, and Western Australia charged lower fees. General Practitioners in Rest of State charge slightly higher fees than State Capital, when performing Tooth-Coloured Restoration for the first 2 surfaces. However, for 3 surfaces and more the fees became cheaper in State Capital areas. Other Restorations Fees increased by an average of 2% across all Other Restoration services within this section. General Practitioners in South Australia and Western Australia generally tended to charge lower fees for Other Restorative services than other states. General Practitioners in Rest of State tended to charge less for Other Restoration services than their State Capital counterparts. Except for Item 576 Metallic crown-preformed and Item 579 Bonding of tooth fragment, more than two thirds of General Practitioners (70%) provided the service with a fee charged for Item 582 Veneer direct, Item 577 Cusp capping, Item 597 Post direct and Item 578 Restoration incisal per corner.

25 Discussion 25 Dental Fees 2016 Prosthodontics Eight in ten General Practitioners (81%) indicated that they provided Prosthodontics services. Fees within the Prosthodontics services have slightly increased by an average of 1.5%. Fees for Prosthodontics services category were usually higher in the State Capital compared to Rest of State. Crown and Bridge and Implants Fees within the Crown and Bridge services have increased by an average of 4%. Almost three quarters (75%) of General Practitioners reported providing Crown and Bridge services for a fee. Close to 30% of General Practitioners provided and charged a fee for Implant services. Other Prosthodontics Overall, fees within this section have slightly increased by an average of 1.7% since Fees for these Other Prosthodontic services category were usually higher in the ACT and Tasmania and lower in Western Australia and South Australia. Orthodontics Only less than one third of all General Practitioners provided Orthodontic services. Overall, fees for Orthodontics services have increased in comparison to 2015 ranging from 2% for Item 823 Functional orthopaedic appliance to 22% for Item 825 Sequential plastic aligners. Fees for Orthodontic services category were usually higher in the State Capital compared to Rest of State. Higher fees for these services were generally charged in Western Australia and lower fees in South Australia. General Services Fees have slightly increased by 1.3% for General Services compared to For the majority of these items, General Practitioners located in State Capital charged a higher fee than their Rest of State counterparts. More than half of General Practitioners provided General Services, with 75% providing Item 965 Occlusal splints with a fee and 60% providing Item 926 Individually made tray medicaments and Item 981 Splinting and stabilisation direct per tooth Only 9% of General Practitioners offer Item 942 Sedation intravenous Per 30 minutes whereas 36% offer Item 943 Sedation - inhalation - per 30 minutes.

26 Specialists 26 Dental Fees Dental Fees Specialists 4.1 Response Approximately 10% of the active private practice sector dentists in Australia are Specialists. This survey also includes responses for those who restrict their practice to a specialty and classify themselves as Specialists. The responses of Specialists have been separated from General Practitioners and the summary in this section comments on items included on the questionnaire that are performed by Specialists. No comparison is made for State Capital and Rest of State Specialists. A total of 793 specialists responded to the survey including 34% of Specialists stated that they were not qualified for the study, not working in a fee charging environment (government, research, etc.) and a further 22% of Specialists refused to participate in the survey. Only 259 specialists provided valid responses which are included in the analysis. 27 Specialists 27 Dental Fees Frequency provision of services Instead of having Specialists provide answers to all 119 items, as in previous years, in order to save them time they were asked to specify their area of expertise and also indicate which Service Categories they provide. They were then asked to provide their answers for items in their relevant Service Categories. It is clearly noted that the Specialists in the areas of Orthodontics, Endodontics, Oral and Maxillofacial surgery only perform in their areas of expertise, whilst other Specialists like Periodontics, Paediatrics have provided services across a few Service Categories. 35 Discussion 35 Dental Fees Discussion - Specialists Fees Hourly Rate One third (31%) Specialists charged an hourly rate and, similar to 2015, the rate per hour is $548. Specialists in Queensland charged the highest average hourly rate of $600, while South Australia charged the lowest rate at $400. Diagnostic Services Almost half of Specialists provided Diagnostic services for the majority of the items. Very minimal number of Specialists provided and charged a fee for item 047 Saliva screening test. One third of Specialists (33%) provided Item 72 intraoral Photographic records without charging the patients. In comparison to 2015, fees have increased for half of the items in the Diagnostic Services category with the highest increases for Item 13 Oral Exam limited by 13%. Preventive Services Less than 20% of all Specialists provided Preventive services and charged a fee. Prosthodontics, Periodontics, Paediatrics and Orthodontics were more likely to provide Preventive services than other type of Specialists. In comparison to 2015, there has been a variation of fees charged across the Preventive services. Due to the low sample, of less than 30 Specialists who provided responses this year, a comparison of fees charged between the 2 years is not made. Almost half of Specialists who provided Item 141 Oral hygiene instruction and Item 131 Dietary advice did it free of charge. Periodontics Periodontic services are mainly provided and charged a fee by Periodontists and Paedodontists. Some specialists in the area of Oral Surgery also provided these services. Overall, fees for most Periodontics services have varied depending on items and due to a low sample size of less than 30, comparison between 2015 and 2016 is not conducted. Specialists in Victoria, compared to other States, tended to charge higher fees for Periodontics services.

36 Discussion 36 Dental Fees 2016 Oral Surgery Close to 20% of all Specialists provided and charged a fee for Oral Surgery items. They are mainly performed by Specialists in Oral Surgery and Paediatrics. Some Periodontic and Prosthodontic specialists also provided Oral Surgery. In comparison to 2015, there has been a variation of fees charged across the Oral Surgery services ranging from 2% for Item 323 Surgical removal tooth or fragment with bone removal to 24% for Item 386 Splinting of displaced tooth. Endodontics Approximately 2 out of 10 of all Specialists provided and charged a fee for Endodontic services. They are mainly provided by Endodontists and Paedodontists. Due to a very low sample of Specialists provided responses this year a comparison of fees charged between the 2 years is not made. Restorative Services On average less than 10% of all Specialists provided and charged a fee for various Restoration services. Less than 5% of all Specialists provided Metallic Restoration services and charged a fee. Approximately 10% of all Specialists provided the Adhesive Restoration services and charged a fee. Only 1% of all Specialists provided the Tooth-coloured Restorations and charged a fee. Due to the very low sample of Specialists who provided responses this year, a comparison of fees charged between the 2 years is not made. Prosthodontics Prosthodontic services are mainly provided by Prosthodontics. Almost of half of Paedodontists and some Periodontists also provided the Prosthodontics service items. Due to the very low sample of less than 30 Specialists who provided responses this year, a comparison of fees charged between the 2 years is not made. Specialists in NSW tended to charge higher fees for Prosthodontic services.

37 Discussion 37 Dental Fees 2016 Orthodontics Just under half of specialists (45%) provided Orthodontic services and charged a fee. The services are mainly performed by Orthodontists. Almost two thirds of Paedodontists and some Prosthodontists also provided the Orthodontic service items.
