3 Introduction It is a central aim of research and development at 3M ESPE to assist dentists in every way with materials tailored to the dental practice, simple handling methods and a product range that takes into account of the fact that users preferences differ. Twenty years ago, Express was already setting the standard for precision impression taking in the dental practice with the world s first automatically mixable light-bodied wash material from a dispenser. With Express Penta Putty, the researchers at 3M ESPE now present the first genuine 3M ESPE VPS Putty Material specifically developed for automatic handling in the Pentamix 2 Automatic Mixing Unit. The scientific and technical challenge posed by this exciting project consisted of combining the valued characteristics of the familiar malleable VPS with a completely new type of handling. The putty is complemented by Express, whose innovative formulation offers both excellent hydrophilisation and high tensile strength. This is the major distinction between Express and conventional VPS light-bodied wash materials. This booklet focuses on the evaluation of a major user study, in which more than 220 dentists in ten European countries participated. Express Penta Putty and Express provide the perfect combination of convenience and precision impressions, thus generating a high level of acceptance amongst practitioners, as the study results demonstrate. The results of comparative in-vitro studies, which are also presented, are relevant to daily dental practice. In addition, important questions on the method of use are answered. 6 1.1. Express Penta Putty 1 The putty material Express Penta Putty is the first genuine 3M ESPE VPS Putty specially developed for automatic mixing in the Pentamix 2 Mixing Unit from 3M ESPE. This innovative material is easy and clean to handle, permits dispensing of exactly the quantity required and offers the user all the benefits of a conventional putty material. Express Penta Putty Chemical background 1 The innovative formulation of Express Penta Putty, with reactive polysiloxanes and fillers, makes it easy to dispense and enables problem-free automatic mixing, in spite of its putty consistency. The special polymer combination also gives the material high ultimate hardness: the preliminary impression is quick and easy to carve, while, at the same time, the pressure required to press the light-bodied wash material perfectly into the sulcus is created in the preliminary impression. The highly reactive polymers contained in Express Penta Putty and Express also offer another important advantage for the two-step putty wash technique. The two-stage impression technique can also be carried out quickly because of the short working and setting times which is better for patient and dentist alike. New Penta Components The new Penta components are designed to suit the high viscosity of Express Penta Putty perfectly, offering maximum safety and reliability for Automatic mixing in the Pentamix 2 Automatic Mixing Unit. The Penta Cartridges are reinforced with a metal cylinder. The new design of the foil bag caps permits optimal dispensing of the paste. The Penta Mixing Tip Red has a new design which reduces the dispensing forces by up to 50 percent. 7

8 1.2. Express 1 The light-bodied wash material The low-viscosity Express is mixed automatically in the dispenser and is specially formulated for use with Express Penta Putty. It has outstanding flow properties, even in the moist oral environment, and offers high mechanical strength. 9 1.2. Express Chemical background 1 The increased molecular crosslinking density after setting gives Express greater mechanical strength than conventional light-bodied addition-cured silicones. It offers extremely high tensile strength both during removal from the patient s mouth and when fabricating the model in the laboratory. Very good hydrophilisation means excellent flow properties even in the moist oral environment. The small proportion of microfine fillers which are very weakly crosslinked with each other, increases the structural viscosity: even minimal pressure is sufficient to make Express flow perfectly, while without pressure, the material is stable. This also makes it ideal for syringing around the prepared teeth. Express flows finely, and is extremely resilient after setting, thanks to the strengthened molecular network. 9

Insertion Resistance of Putty Silicones 2 Comparison of insertion forces of conventional and new automixed putty impression materials Introduction To date it still has not been possible for a large number of dentists to take advantage of the benefits of automixing systems for their impression material for the tray. These dentists prefer to use putty silicones for taking precision impressions. Due to their very high viscosity levels, these materials have been typically mixed by hand, incurring the associated disadvantages such as nonhomogeneity and the presence of voids in the mix. In particular dentists value putty silicones because of their high resistance on insertion of the tray and the resulting high pressure that forces the low viscosity wash material deep into the sulcus. In addition, the mixed material can be distributed and shaped in the impression tray with the finger. With Express Penta Putty the objective is to also offer this group of dentists a suitable impression material with putty characteristics that can be automatically mixed in the Pentamix Automatic Mixing Unit. Objective A valuable property of putty silicone impression materials is high resistance on insertion of the tray. The aim of this study is to compare the insertion force of conventional handmix putty materials to the new automix VPS putty impression material Express Penta Putty. 12

Insertion Resistance of Putty Silicones Comparison of insertion forces of conventional and new automixed putty impression materials Materials and methods* 2 The new Express Penta Putty (3M ESPE, EXPR) was compared to handmixed products: Optosil Comfort Putty (Heraeus Kulzer, OPTO), President Putty Fast Soft (Coltene, PRES), Panasil Putty Soft (Kettenbach, PANA). To differentiate putties from heavy bodies, Dimension Penta H Quick (3M ESPE, DIME) was added to the comparison. For the automixed materials a new low force mixing tip was applied. All materials were mixed for exactly 30 seconds before testing. The test was conducted on the Zwick Z010 testing machine to guarantee reproducible results. Testing machine Equal quantities of each putty were placed between specially formed dies, which were moved together with the same velocity until a defined distance was reached. The insertion force of the automix VPS 3M ESPE Express Penta Putty is within the typical range of conventional handmix putties. The values of the heavy body impression material DIME were significantly below this range. 14

Insertion Resistance of Putty Silicones Comparison of insertion forces of conventional and new automixed putty impression materials Discussion 2 The high insertion force of the new automix VPS Express Penta Putty is possible due to the special rheological properties of the paste system: The structural viscosity of the material leads to lower viscosity while mixing (high shear rates) but putty consistency in the tray (low shear rates). In addition to that the mixing process is optimized by reinforced Penta Components (page 7). Conclusion With respect to the insertion resistance the automixed material 3M ESPE Express Penta Putty shows typical putty properties and offers characteristics similar to handmixed putties. Tensile Strength and Contact Angle of VPS 2 Correlation of wetting behaviour and mechanical properties of light-bodied (LB) VPS materials Introduction The clinical success of a precision impression material depends on many different factors. Important features are for example good mechanical properties of the elastomer during removal of the set impression from the mouth and good wetting behaviour in the moist environment and while pouring the impression. The hydrophilicity of VPS impression material can be increased by addition of surfactants. But these surfactants may interfere with the elastomer network formation and thus weaken the physical properties of set impression materials. This plasticizer effect can result in a reduction of the tensile strength of the cured silicone material. Thus hydrophilization of VPS impression materials may cause an increased risk of tearing during removal of the impression from the patient s mouth. Typical molecular structures of wetting agents for VPS impression materials Hydrophilicity of VPS impression materials normally is defined by a measurement of contact angles of drops of water on the surface of impression materials. Therefore an important goal for the development of new hydrophilic VPS impression materials is to combine highest possible tensile strength with lowest contact angels. To improve the elastomeric properties of a new hydrophilic VPS impression material the network of the chemical matrix was reinforced. Newly developed crosslinkers build up a higher crosslink density of the elastomeric network after setting to compensate the loss of tensile strength caused by the surfactant. 16

Tensile Strength and Contact Angle of VPS Correlation of wetting behaviour and mechanical properties of light-bodied (LB) VPS materials Objective 2 The aim of this study was to investigate the influence of the degree of hydrophilization on the elastomeric properties of VPS impression materials. A newly developed VPS impression material with a reinforced network was compared to several conventional products. Measurement of Tensile Strength Measurement of Contact Angle Materials and methods* Tensile strengths and hydrophilization of the following materials were compared: Provil Novo Light C. D. Fast set (Heraeus Kulzer, PN, LOT ), Express Light (3M ESPE, EL, LOT ), Panasil Contact Plus (Kettenbach, PC, LOT 30451) and the newly developed Express (3M ESPE, EU, experimental batch). To determine the degree of hydrophilization three specimens (38 mm x 32 mm x 3 mm) of each material were produced. One hour after mixing base and catalyst paste the 10 second contact angle (CA) of four drops of water on each specimen was determined using a standard goniometer. Tensile strength was determined by producing six l-shaped specimens with a central unit of 20 mm x 4 mm x 2 mm. Ten hours after mixing of base and catalyst paste tensile strength was measured using a Zwick machine. * See back page for trademarks information 17

Tensile Strength and Contact Angle of VPS Correlation of wetting behaviour and mechanical properties of light-bodied (LB) VPS materials Discussion 2 All products in this test are Type 3 VPS impression materials. EL, PN, PC show significantly different degrees of hydrophilization. For these products a decrease in contact angle seems to be connected to a decrease in tensile strength although tensile strength data for EL and PN are not significantly different. Only EU (3M ESPE Express ) does not fit into this series as it shows the highest hydrophilicity (lowest contact angels) and the highest tensile strength in this test. This may be due to the reinforcement of the VPS network which was realized by addition of newly designed crosslinkers. Those are believed to increase the crosslink density of EU and thus compensate the weakening effect of surfactants in this case. This may be supported by the fact that the consistency of EU (3M ESPE Express ) is on a comparable level as the other materials in this study. Therefore the higher tensile strength of EU (3M ESPE Express ) should mainly result from an increased crosslink density of the VPS network and not from a higher filler content. Conclusion EL, PN and PC show a tendency towards decreasing tensile strength with increasing degree of hydrophilicity indicated by decreasing contact angle. In contrast 3M ESPE Express shows the highest tensile strength and hydrophilicity in this test. 19

22 3.1. Participants / Result Categories Comprehensive assessment by more than 220 dentists 3 As part of a European dental practice study, in which more than 220 dentists from ten countries took part, over 3,000 impressions were taken with Express Penta Putty. The participating dentists then assessed Express Penta Putty and Express, and their automatic handling in the Pentamix 2 Automatic Mixing Unit, in a detailed survey. Their opinions about the impression quality were also included in the study. The sequence in which the results of this extensive evaluation are presented in the first subsections basically reflects the chronology of the impression-taking process. The last subsection focuses on the assessment of the system as a whole. Express Penta Putty and Express Vinyl Polysiloxane Impression Material 22

Impression Techniques, Materials, Indications Details about use 32 percent of the participating dentists used the two-step putty wash technique exclusively, while 31 percent used the one-step tray wash technique for all their impressions, and the remaining 37 percent used both techniques. Express Penta Putty and Express were used with the two-step putty wash technique for half of all the impressions taken. With the one-step tray wash technique, the same material combination was used for 37 percent of all the impressions. For the remaining 13 percent, Express Penta Putty was combined with another VPS light-bodied wash material from 3M ESPE. The indications for over 80 percent of all the impressions were crowns (56 percent) and bridges (27 percent). The majority of the dentists (65 percent) used metal fullarch trays (52 percent), and metal partial trays (13 percent) were used in addition. 3 Total number of impressions for each indication Number of impressions Crowns Bridges Inlays/Onlays Others 23

Furthermore, one in four dentists thought it worth special mention that the automatic mixing method for the putty required less effort and was more stress-free. 35

Assessment of the Complete System Assessments categorised according to material combination and dentist s preferred technique 3 The users were asked what they considered to be the two biggest advantages of the new impression system. The answers received were then categorised by preferred technique and material combination. Express Penta Putty combined with Express The following ranking is derived from the response of dentists who only use the two-step putty wash technique: Hygiene, ease of handling, high precision/accuracy of fit. Dentists who only use the one-step tray wash technique also put hygiene at the top of their list, followed by mixing quality and ease of handling. Hygiene was also the number one advantage in the view of dentists who use both techniques, depending on the particular indication. Simple usage and outstanding precision and accuracy of fit came in at numbers two and three. Express Penta Putty combined with another LB VPS material from 3M ESPE Dentists who only take one-step tray wash impressions and also use Express Penta Putty in combination with another addition-cured silicone light-bodied wash material from 3M ESPE regarded the mixing quality and specific consistency of the putty material as the two most important advantages. Participants who use both the one-step and two-step impression techniques with the same material combination particularly valued the fact that the material is suitable for both. Further advantages cited were the high precision and mixing quality. 36

Irrespective of the material combination or impression technique used, these dentists stated that the ease of handling was the most important argument for their recommendation. They were equally impressed by the hygiene, cleanliness and precision. Those who only use the one-step tray wash technique put the speed of use in second place. Dentists who only use the two-step putty wash technique describe the system as an ideal solution for the two-stage method in the dental practice. 37

The individual statements by the dentists covered a wide range of advantages for impression taking in the dental practice. The ease of handling, consistent mixing quality, reliable precision plus hygiene and cleanliness are quite evidently aspects to which the study participants personally attach importance. Numerous participants additionally singled out the advantages of the new Express Penta Putty in combination with their normal choice of addition-cured wash material from 3M ESPE. 44 5. Questions and Answers Can Express Penta Putty also be mixed in the Pentamix 1 Automatic Mixing Unit? 5 No, Express Penta Putty must be mixed in the Pentamix 2 Automatic Mixing Unit. Only this unit has the necessary mixing speed. Specific added components ensure reliable mixing of the heavy-bodied Express Penta Putty in the Pentamix 2: Special Penta Mixing Tips Red reduce the dispensing forces. The new design of the foil bag end and the metal-reinforced Penta Cartridge guarantee the required mechanical strength. What will happen if conventional mixing tips are fitted to the Pentamix 2 by mistake? The end of the Express Penta Putty foil bag is designed so that only the new (red) Penta mixing tips can be fitted. Accidental use of old (white) tips is therefore impossible. What types of tray are ideal for Express Penta Putty? Torsionally rigid edentulous or partial metal trays, with or without perforation, are the most suitable. For reliable adhesion of the impression material, the tray is pre-treated with a tray adhesive for addition-cured silicones, e.g. 3M ESPE VPS Tray Adhesive. Undercuts should be blocked out. What points require special attention if you want to mould Express Penta Putty in the tray with your finger? Just a few seconds after filling the tray from the Pentamix 2, the material in the tray can be moulded as required with your finger, with no sticking just like any malleable silicone. As with other addition-cured silicones for precision impressions, you should avoid direct contact with the material if you are wearing latex gloves. It is therefore recommended to use vinyl gloves. 44

45 5. Questions and Answers For which impression technique are Express Penta Putty and Express most suitable? Express Penta Putty and Express is the ideal combination for the two-step putty wash technique and the one-step tray wash technique. For the one-step putty wash technique Express Penta Putty can be also combined with other 3M ESPE wash materials (e.g. Express Light Body [blue] or Imprint II Garant Quick Step [orange]). Advantages for two-step putty wash impressions: The two-stage method is particularly suitable for the dental practice thanks to the agreeably short working and setting times of both viscosities. The putty material is easy to carve. Its high ultimate strength means that the preliminary impression exerts a high pressure, so that the wash material flows finely and can be pressed into the sulcus efficiently. 5 Advantages for one-step tray wash impressions: The automatic mixing of both consistencies and the short working and setting times make the single-stage method even quicker, as confirmed by numerous assessments by dental practices. What wash materials can Express Penta Putty be combined with? The low-viscosity Express was developed specially for use with Express Penta Putty. However, the automatically mixable Express Penta Putty can also be used in combination with other addition-silicone wash materials from 3M ESPE. Like all addition-cured silicones, Express Penta Putty and Express Ultra-Light Body are not compatible with condensation-cured silicones, alginates or polyethers. How should the area where the impression is to be taken be prepared? If a retraction cord is used, this should first of all be thoroughly removed. All the areas of which an impression is required must be dried. As from when can the model be cast? Just two hours after disinfection, the model can be produced with a standard dental stone. To prevent voids on the model, the impression is rinsed with water and blown dry shortly before casting. Alternatively, a silicone wetting agent can be used. 45
