2 ADVANCED GRADUATE EDUCATION PROGRAM IN ENDODONTICS PROGRAM OVERVIEW The postgraduate program in endodontics at the Medical University of South Carolina is designed to offer a balanced clinical and didactic curriculum in advanced adolescent and adult endodontic care. Administered by the Department of Oral Rehabilitation of the College of Dental Medicine; the postgraduate program in endodontics will be based in a dedicated area of the James B. Edwards Dental Clinics Building on the main MUSC campus. A Certificate in endodontics and Master of Science in Dentistry (M.S.D.) degree will be awarded upon successful completion of the twenty-six (26) month program. Completion of the program will qualify the postgraduate student for board examination by the American Board of Endodontics. The program has been granted the accreditation status of approval without reporting requirements by the Commission on Dental Accreditation of the American Dental Association (August 31, 2012). Recognizing the evolving nature of contemporary endodontic practice and the varied interests of postgraduate students, the MUSC postgraduate program in endodontics will strive to provide a full range of clinical experiences in endodontic management. The postgraduate student will receive clinical opportunities in endodontic diagnosis, treatment planning and care for adolescents and adults. The clinical resources of the College of Dental Medicine and the MUSC Medical Center will enhance program experiences to include the endodontic management of traumatic dental injuries, surgical care, and coordinated restorative-prosthetic treatments. The clinical experiences will be supported by a structured didactic and laboratory curriculum designed to provide an in-depth, critical, and scholarly appraisal of the specialty with evidenced-based knowledge. The program is designed to prepare graduates for successful entry into the contemporary practice of endodontics as well as for future growth in the discipline. In order to meet the goal of providing a wide range of clinical and didactic experiences grounded in a process of critical analysis, the program provides the following: 1. Clinical endodontic care involving the diagnosis and treatment of medically compromised patients. 2. Clinical endodontic care involving the diagnosis and treatment of patients in the young permanent dentition to include comprehensive vital pulp therapy management. 3. Clinical endodontic care involving the diagnosis and treatment of adults to include comprehensive management of retreatment protocols as well as surgical procedures and coordinated restorativeprosthetic management. 4. A structured curriculum of seminar and laboratory exercises supports the clinical experiences in addressing the parameters of endodontic care for immature and fully developed teeth. 5. All postgraduate students will prepare comprehensive case reviews that document clinical management of patients following format criteria of the American Board of Endodontics. Besides documenting performance of quality care, the case reviews will enhance the graduate's ability for critical analysis and patient documentation. 2

3 6. Teaching experience through serving as clinical and laboratory instructors in the predoctoral endodontic curriculum on a one half-day per week basis. 7. Interdepartmental seminars and courses provided through the Biomedical Sciences and Clinical Core M.S.D. curriculum will supplement the postgraduate student's knowledge base. 8. Each postgraduate student conducts a data collection research project and a comprehensive literature review of the topic which is of publishable quality. Besides providing informational service to the profession, the experience of conducting research will enhance the student's ability for critical analysis. 9. Exposure to practice and risk management and dental care delivery through seminars and visits to varied private and public clinic sites. 5 PROGRAM GENERAL OBJECTIVES The postgraduate program in endodontics at MUSC is a program in which postgraduate students become qualified and competent endodontic specialists through didactic and clinical training. This training occurs throughout the year in classroom, patient clinic and laboratory setting in which qualified faculty members instruct the residents in all aspects of endodontic care. The program has a rigorous training schedule which involves in-depth reading and comprehension of classic and current literature, and numerous presentations of clinical care and clinic activities. The general objectives of this program include, but are not limited to, the following: 1. Training dentists to become specialists in the field of endodontics to pursue a career in endodontics in private practice, academics, or health related fields. 2. Providing the postgraduate student with an in-depth background in the basic sciences as they relate to endodontics and to have the graduate integrate this background into the practice of endodontics. 3. Providing advanced education and training in the field of endodontics that is consistent with the standards provided by the American Dental Association Commission on Dental Accreditation. 4. Providing an environment that encourages and prepares students to communicate, share and work with professionals beyond the discipline of dentistry (IPE). 5. Providing an environment that will prepare students to design, conduct, analyze, and discuss scientific research investigation. 6. Providing the postgraduate student an atmosphere that fosters the appreciation of the importance and advantages of membership and participation in organized dentistry. 7. Preparing postgraduate students to become Diplomates of the American Board of Endodontics. 8. Providing an environment conducive to evidence-based practice and lifelong learning. 5

6 PROGRAM GOALS In meeting the overall goal of producing a proficient specialist in endodontics, the specific goals of the postgraduate program will encompass the following aspects of patient care: 1. Proficiency in the treatment of dental emergencies required to properly manage pain and acute infection of endodontic origin. 2. Proficiency in the treatment of traumatic dental injuries to properly manage the pulp, tooth and adjacent periradicular tissues. 3. Proficiency in the treatment of young permanent dentition required to manage the development of the pulp and tooth in adolescents to include vital pulp therapy. 4. Proficiency in the management of adult dentition required to address pulp and periapical considerations of teeth to include treatment, retreatment, surgery, bleaching, and placement of coronal and intraradicular restorations. 5. Ability to educate and guide patients to accept and practice oral health care with preventive concepts a foundation of clinical practice. 6. Understanding the physical and chemical properties of dental materials used in endodontic treatment and patient responses to these agents. 7. Knowledge of oral pathology and management procedures applicable to patient care. 8. Ability to diagnose traumatized and carious primary and permanent teeth to maintain the teeth, pulp tissues, and periodontium in a healthy state. 9. Understanding the growth and development of the pulp and tooth, and the ability to provide treatment aimed at allowing optimal maturation. 10. Familiarity with published literature pertinent to endodontics, and the motivation to remain abreast with and critically evaluate the literature. 11. Commitment to the implementation of community based programs aimed at educating the public on oral health, dental and endodontic issues. 12. Preparation for managing a contemporary endodontic practice relative to practice administration, efficient auxiliary utilization, and marketing. 13. Coordination of all objectives such that diagnosis, case assessment, treatment planning, and clinical management of endodontic problems that occur in adolescence and adulthood are grounded in a sound application of scientific evidence-based knowledge through development of an attitude towards lifelong inquiry and study. 6

7 PROGRAM ACQUIRED SKILLS AND KNOWLEDGE Postgraduate students who complete the entire twenty-six (26) month postgraduate endodontic program at MUSC are expected to have developed their skills and knowledge to the level of a specialist in endodontics as approved by the ADA Commission on Dental Accreditation. Completion of the program will enhance specialty skills of each postgraduate student beyond that of their predoctoral education with significant advances in the following major areas of practice: 1. Diagnostic Skill The postgraduate student will have had considerable didactic background and direct contact with patients exhibiting a variety of symptoms and case scenarios. Recognition of differential diagnosis parameters of facial symptoms and treatment modification for medically compromised patients is expected. Familiarity with specific radiographic bone appearance from pathologies and their consequences is mandatory. Knowledge of physical, mental, and emotional development is essential. Common and rare defects of the teeth, oral tissues, and skeletal structures should be recognized. Students should be especially skilled in obtaining and interpreting data from diagnostic aids assessing pulp and periradicular tissues. These aids would include in-depth clinical examinations, intraoral radiographs and assessment of restorability. The postgraduate student should be able to make correlations between basic sciences and clinical applications. 2. Treatment Planning The postgraduate student should be able to prepare and present treatment plans which utilize the diagnostic training received. The treatment plans should be comprehensive with treatment provided in an efficient sequence. Postgraduate students should have an accurate assessment of their ability to deliver treatment by critically evaluating outcomes of cases at follow-up. 3. Patient Management The postgraduate student should have superior abilities to achieve the cooperation of adolescents and adults. The student should possess good communication skills and an understanding of behavioral principles. Application of these skills with motivational tools will enable the graduate to modify patient behavior, especially in the management of orofacial pain and anxiety. 4. Provision of Endodontic Treatment The postgraduate student will be able to provide high quality endodontic care for adolescent and adult patients. Clinical expertise will be attained in diagnosis and treatment planning, use of digital radiographic imaging and magnification technologies, familiarity with varied conventional treatment modalities, management of retreatment cases, bleaching techniques, surgical treatment, traumatic dental injuries, emergency care of dental pain and acute infection, placement of coronal and intraradicular restorations, outcome assessment of treated cases, treatment of the medically compromised, and management of medical emergencies. 5. Collaboration The postgraduate student should develop considerable skill in establishing rapport and cooperation with dental and medical colleagues. Referrals to appropriate professionals or from other professionals occur frequently and the postgraduate student should be familiar with the courtesy and importance of these activities. The postgraduate student should be able to make, or respond to, all appropriate consultation requests. 7

8 6. Research The postgraduate student will be able to evaluate original dental research articles for methodology, results, statistical interpretation, conclusions, and implications. Ability to conduct research may be developed from the required research project; however, the major research goal is an understanding and appreciation of published research. 7. Teaching The postgraduate student will have developed considerable teaching skills by preparing and presenting lectures as well as from predoctoral clinical supervision. These skills should provide the postgraduate student with a strong foundation for patient education and professional presentations. 8. Practice Management and Auxiliary Utilization The postgraduate student should be well prepared for most aspects of practice administration and efficient auxiliary utilization. 9. Critical Thinking It is expected the postgraduate student will establish an approach to learning which utilizes aspects of continual inquiry and critical thinking. The development of a lifelong attitude of study and advancement is anticipated. FACILITIES AND RESOURCES - Postgraduate Endodontic Clinic The primary postgraduate endodontic clinical facility is located on the first floor of the James B. Edwards Dental Clinics Building on the main campus of the MUSC complex and serves as the primary care site for endodontic services affiliated with the College of Dental Medicine. As a major teaching service component of the Medical University, the postgraduate endodontic clinic is directed toward providing comprehensive endodontic specialty care for adolescents and adults, with in-house sterilization for instruments. The postgraduate endodontic clinic includes six patient operatories that are wheelchair accessible, incorporating a closed-bay format completely equipped for the delivery of comprehensive endodontic care, including ceiling mounted Seiler surgical operating microscopes. Computers with dental software programs (axium and MiPACS Dental Enterprise Viewer) for entry and storage of patient data and digital images are in each operatory and student s study cubicle. Each operatory has wall mounted units for intraoral radiography (Instrumentarium, tube type: D-07115, 70 kv, 7 ma), digital CCD docking stations, adjacent digital phosphor plate imaging processing (Soredex OpTime scanners). Operatories are shielded according to the requirements of the South Carolina Department of Health and Environmental Control (DHEC), the National Council on Radiation Protection and Measurements, and the MUSC Radiation Safety Manual. Radiographic equipment is routinely inspected and exposure parameters are posted by all control boxes. The exposure timers are located outside of each room. Lead aprons with collars are utilized for all patients. A log is kept on the maintenance and repair of the equipment. Quality assurance is monitored by clinical staff under the lead of the clinic director using QA Indicators. Personnel with significant responsibility in radiography wear dosimeters. Operators adhere to radiation safety policies as listed in the Ionizing Radiation Control Policy document of the Dental Clinic Manual. Adjacent to the postgraduate endodontic clinical operatories is a dedicated conebeam radiography room which contains a Carestream D unit, facilitating efficient supplemental diagnostic data. 8

9 The clinic floor has a full reception/business office and ample patient waiting areas. Patient education cubicle areas with comfortable seating and patient information materials are located within the main clinic waiting area. These areas serve as a consultation site for case presentations and other patient discussions requiring privacy. Adjacent to the treatment area are storage rooms, a faculty / student office, a staff office, and a dedicated postgraduate endodontic student room with personal computers at each student s study cubicle. A simulation laboratory facility is located in the Basic Science Building adjacent to the James B. Edwards Dental Clinics Building. Each station has a lab bench format with stools, mannequins, electric high and low speed handpiece adaptors, a torque control gear reduction handpiece, suction, and a storeroom with a full supplement of endodontic materials and supplies. Unlimited access to the facility allows students to examine and test new materials, instruments, equipment and techniques. The facility has eight wall mounted radiographic units with digital processing capacity available for postgraduate student use. 9
