1 Children s Diabetes Patient Information Leaflet High HbA1c the Surgical Assessment Unit (SAU) You have been given this leaflet because you have a high HbA1c. This leaflet tells you: what HbA1c is what help you can get What is HbA1c? HbA1c is what is tested when you have a blood test. This tells doctors and nurses how well your diabetes has been controlled for the last 8 to 12 weeks. What does the HbA1c test measure? The HbA1c blood test measures the amount of glucose that is being carried by the red blood cells in the body. The more glucose in the blood over time the higher the HbA1c will be. How is HbA1c measured? A blood test is used to measure HbA1c. The test can be done at any time of the day. HbA1c levels used to be measured in percentage. This has changed to mmol/mol. For example, 8% is now reported as 64mmol/mol. Diabetes UK has an online converter (this will work it out for you).
2 What should my HbA1c level be? You should try to have a level of 48mml/mol or lower. Lower HbA1c levels show blood glucose is being controlled well. This helps lower the risk of getting complications from high blood glucose. What are the risks of a high HbA1c level? Having a high HbA1c means your blood glucose is not controlled well. This is not good for your health. If you have a high HbA1c for a long time you may get problems with your eyes, kidneys, heart and strokes. How can I lower my HbA1c level? You will be given support from the Children s Diabetes Team to lower your HbA1c levels. We will give you things to do at home and meet with you and your family. Because your HbA1c is more than 80mml/mol at the moment, you will have an appointment in the next 2 weeks. After this you will have an appointment every 6 weeks. This is as well as your 3 month appointment with your consultant. We also want you to call or us every 2 weeks so we can see how you are getting on. We may ask you to come in to see us if you are having problems. You will have another HbA1c test after 3 months to see how you are doing.
3 If your HbA1c is 80mmol or below you will get an appointment every 6 weeks with the team until it is below 69mmol. If your HbA1c is more than 80mmol but has reduced by more than 10mmol you will have six week appointments. We will ask you to call or us every two weeks. If your HbA1c is still more than 80mmol and has not gone down by more 10mmol we will talk to your consultant. We will keep seeing you every six weeks and ask you to call or us every two weeks. We would like patients and their families to call the team when they need support on We are open Monday to Friday 9am 5pm. If you need urgent advice call the children s ward on You can us at paediatric.diabetes@dgc.nhs.uk
4 What can I do to help reach my target? Try to keep your blood level between 4 and 7mmol. Test your blood glucose 6 times a day. Be very accurate when you are counting carbohydrates. If your blood glucose is more than 7mmol before your meal always add a correction dose to your meal time insulin. If you are on the pump put your glucose reading through the bolus wizard and use the insulin dose. Review your blood glucose diary every 2 weeks and adjust your insulin if you need to. Be active for at least 60 minutes every day.
