Review clinical documentation templates associated with conditions frequently reported as unspecified Determine root causes Are EMR, dictation or paper based templates missing required documentation elements? Are physicians, coders, billers aware of the impact of unspecific codes? Is there an opportunity to intervene when unspecified codes are assigned prior to billing?
