ROS: no skin rash, joint pain, UR sx, other GI/GU sx. PMH: HIV c/b PCP/TB/MAC/COPD(on home o2), lung empyema s/p R thoracotomy 12/2010, pulmonary AVM, pseudomonas vertebral OM 5/2012, IVDA. Meds: truvada, kaletra, methadone. Heavy ex-smoker No personal or family ho renal stones.
3 Exam: Vitals: T: bp: 127/85 p: 82 po2: 99 nc. Gen: NAD. Chest: right ribcage bandaged, healing granular tissue, left basilar rales. CV: regular, no murmurs, or rub Abd: soft. NT Ext: pitting edema 2+, no skin rash
