1 * All CMA s, MA s, LPN s, and RN s are authorized by this practice to complete each standing order listed below. It is policy of this practice to only complete the standing order for patients who have had a medical exam within 12 months. These standing orders are reviewed on an annual basis and approved by the supervising physician. PREVENTIVE SCREENING Breast Cancer Screening Women who are aged Exclusions: Women who have had a bilateral mastectomy. Mammogram: If most recent mammogram result is more than 24 months, schedule a mammogram for the patient. Chlamydia Screening Women who are aged and are sexually active Chlamydia Screening: If most recent Chlamydia screening is more than 12 months, order the Chlamydia, DNA probe urine test. Colorectal Cancer Screening Men and Women aged Exclusions: Patients who have had colorectal cancer or a total colectomy. Colonoscopy: If most recent Colonoscopy result is more than 10 years, schedule a colonoscopy screening. or Fecal Occult Blood Test: If most recent FOBT result is more than 12 months, order/perform a FOBT, and Sigmoidoscopy: If most recent sigmoidoscopy result is more than 5 years, schedule a sigmoidoscopy. Lead Testing All children, birth to 2 nd Birthday Lead Test: If the child has never had a lead test and is not yet 2 years of age, order/perform a lead test. Lipid Profile Men ages 35 and older; Women ages 45 and older Lipid Panel: If most recent lipid panel is more than 12 months, order a fasting lipid panel.

These standing orders are reviewed on an annual basis and approved by the supervising physician. DISEASE MANAGEMENT Coronary Artery Disease Patients age with a Dx of CAD 1. Complete Metabolic Panel: If most recent CMP is more than 12 months, order a complete 2. Lipid Panel: If most recent lipid panel is more than 12 months, order a fasting lipid panel. Congestive Heart Failure Patients age 18 years of age or older with a Dx CHF 1. CBC: If most recent CBC is more than 12 months, order a CBC. 2. Complete Metabolic Panel: If most recent CMP is more than 12 months, order a complete Chronic Kidney Disease Patients aged with a Dx of CKD 1. Complete Metabolic Panel: If most recent CMP is more than 12 months, order a complete 2. Lipid Panel: If most recent lipid panel is more than 12 months, order a fasting lipid panel. Diabetes Mellitus Patients age with a Dx of Diabetes type 1 or type 2 1. Complete Metabolic Panel: If the most recent complete metabolic panel is greater than 12 months, order a complete 2. HbA1c Testing: If most recent A1C result is more than 3 months, order a HbA1C test. 4. Retinal Eye Exam: If the patient does not have a retinal eye exam result within the last 12 months, refer the patient to an eye care provider for a retinal eye exam. 5. Urine Microalbumin: If the most recent urine microalbumin test is more than 12 months, order a microalbumin test.
3 Disease Management (page 2) Hyperlipdemia Patients aged 18 years of age or older with a Dx of HLD 1. Lipid Panel: If most recent lipid panel is more than 12 months, order a fasting lipid panel. Hypertension Patients aged 18 years of age or older with a Dx of HTN 1. Basic Metabolic Panel: If the most recent basic metabolic panel is greater than 12 months, order a basic 2. CBC: If most recent CBC is more than 12 months, order a CBC. Obesity Patients aged 18 years of age or older with a Dx of Obesity 1. Lipid Panel: If most recent lipid panel is more than 12 months, order a fasting lipid panel.

These standing orders are reviewed on an annual basis and approved by the supervising physician. ACUTE HEALTH EXAM Initial Depo-Provera Injections 1. Following the provider examination, the patient will be given a pregnancy test. 2. If the pregnancy test is negative, then the protocol is to give the patient one (1) dose of the Depo- Provera Injection (150mg). Subsequent Depo-Provera Injections 1. Confirm the last date the patient received a Depo-Provera Injection. 2. Reference Depo-Provera Perpetual Calendar to ensure date range falls between the appropriate time frame of 11 to 13 weeks. 3. If it is determined that it has been 11 to 13 weeks since the last Depo-Provera Injection, a subsequent dose (150mg) shall be given. If outside of date range, a pregnancy test must be obtained and the result must be negative prior to injection. Influenza Screening to perform an influenza screening. Fever above 100F Muscle Aches (predominantly in back, arms, and legs) Chills Sweats Headache Cough Fatigue and/or Weakness Nasal Congestion Pregnancy Test If any of the following symptoms are present during an acute health exam of a female between the ages of years with no history of hysterectomy/oophorectomy, the protocol is to perform a pregnancy test. Amenorrhea - The absence of menstrual periods. Abdominal Pain
5 ACUTE HEALTH EXAM (page 2) Strep Screening to perform a strep swab. If the rapid screen is negative, then perform culture. Throat pain Difficulty swallowing Fever Headache Rash Stomach ache (occasional vomiting in children) Fatigue Known recent exposure to strep Urinalysis to perform a urinalysis. Exclusions: Male patients with complaint of penile discharge. A strong, persistent urge to urinate A burning sensation when urinating Passing frequent, small amounts of urine Urine that appears cloudy, red, bright pink, or cola colored Strong smelling urine Lower back and side (flank) pain Fever Pelvic pressure/lower abdomen discomfort
