8 U S E R G U I D E About the MiPCT Project MiPCT is the largest PCMH project in the country. It comprises 37 physician organizations (POs) throughout Michigan as well as their affiliated practice units totaling approximately 350 practices and 1800 physicians. Expected outcomes include an incentive model and resources for reducing trips to the emergency room, stabilizing (or reducing) the costs associated with patient care, and increasing positive patient experiences through more patient involvement in decisions surrounding their own care. About the MDC MiPCT Dashboard Throughout the duration of the MiPCT project, the Michigan Data Collaborative (MDC) is providing POs with an online, dynamic dashboard for analyzing data collected from payers both commercial and government. MDC is acting as a clearinghouse for the data, making it available to POs they can use the data to assess their various subpos, practice units, and providers. The goal is to provide data that will enable POs to help their care managers be more effective. ABOUT THE DATA The Dashboard contains data from Medicare, Medicaid, Blue Cross Blue Shield of Michigan (BCBSM), Blue Care Network (BCN), and Priority Health. Updates to the data are scheduled to occur regularly. Watch for announcements about data refresh dates in the MiPCT PO FLASH newsletter and on the MDC Website. To learn more about the data displayed in the MDC MiPCT Dashboard Project, see Understanding the Data on page 63. A third-party partner Truven Health Analytics assisted MDC in aggregating the data reported in the MDC Dashboard. 9 U S E R G U I D E Overview of the Pages in the Dashboard MDC has created a set of pages in the Dashboard for presenting the multi-payer claims data. Three core areas of concentration for data analysis include membership/populations, quality of care, and utilization. From the pages and the reports, individual POs can assess their own performance against published benchmarks at an MiPCT project level, which includes all of the multi-payer claims data, and at an individual PO level. The following pages are available on the MiPCT Dashboard: Page Name Population Quality Utilization Trends PO Comparisons PO Reports Description Use the Population page to review your PO s attributed membership, risk, and chronic condition information. (You also can view this data for the entire MiPCT population.) The data on this page can help provide an overview of the overall health and risk status for the members of your population. Use the Quality page to review your PO s rates for quality measures. (You also can view this data for the entire MiPCT population.) This page displays overall rates and counts for each measure and also includes benchmark data for each measure when that data is available. Use the Quality page to review your PO s rates for quality measures. Quality measures include Diabetes, Adult Preventive, Pediatric Preventive, and benchmark rates to gauge if your PO s members are receiving the appropriate care for their conditions. Use the Utilization page to review the following: Emergency Department (ED) visit information, including primary care sensitive visit rates and visit rates for patients with a previous asthma diagnosis. Inpatient admission information, including admission rates for patients with a previous asthma diagnosis. This information is available for your PO (including SubPOs, practice units, and/or practices) and the MiPCT population. Use the Trends page to view patient per member per month (PMPM) cost changes, diabetes quality measures, ED utilization measures, and IP utilization measures over the trend time periods for your PO (including SubPOs, practice units and/or practices) and the MiPCT population. Use the PO Comparisons page to compare emergency department (ED) and inpatient (IP) utilization rates and quality measure rates for each PO. The tables and graphs on this page also include benchmark rates and MiPCT overall rates for each measure. Use the PO Reports page to: Download reports that are specific to your PO. Access the Report Writer, which enables you to select individual components from each Dashboard page to view in one report. For more information, see the Report Writer User Guide. 10 U S E R G U I D E Overview of the Report Writer The report writer is accessible from the PO Reports tab. It enables you to select individual components from each Dashboard page to view in one report. The components that you can select reflect the table column and row headings in each section of each Dashboard page. Only the data available in the current release is available in the Report Writer. Instructions about using the Report Writer are included in the Report Writer User Guide, which can be found on the MiPCT Support page of the MDC Website. Overview of the Reports The Download PO Reports page in the MDC MiPCT Dashboard contains the following reports: Report Name ED Summary ED Detail IP Summary IP Detail PO Quality Measure Comparison Report for All POs PO Quality Measure Comparison Report by Practice PO ED Utilization Comparison Report for All POs PO ED Utilization Comparison Report by Practice PO IP Utilization Comparison Report for All POs PO IP Utilization Comparison Report by Practice G-Code Aggregated Totals G-Code Medicare Practice Feedback All Payer Patient List Description Provide ED visit information for patients who have recently had one or more Emergency Department (ED) visits. Provide IP admission information for patients who have recently had one or more inpatient admissions. Provide a comparison of quality measure rates. One report contains the rates for all POs; the other report contains rates for all of the Practices within your PO. Provide a comparison of ED utilization measure rates. One report contains the rates for all POs; the other report contains rates for all of the Practices within your PO. Provide a comparison of inpatient (IP) utilization measure rates. One report contains the rates for all POs; the other report contains rates for all of the Practices within your PO. Provide monthly and to date data to help monitor and improve the billing and reimbursement process for BCBSM, BCN, and Priority Health G-code claims. These reports provide the RTI-derived reports and measures provided to CMS for the national demonstration project. The reports contain measure rate comparisons at a Practice level for Medicare patients only. These reports provide a current list of MiPCT patients attributed to each practice. 12 U S E R G U I D E Logging In For instruction on how to access the new MiPCT Dashboard, see Accessing the MiPCT Dashboard document on the MDC Website s Support page. After you log in to the dashboard, the Welcome page displays, similar to the following: Logging Out When you are done using the Dashboard, you can log out by closing the web page and then closing the browser. 13 U S E R G U I D E Components of the Dashboard The MiPCT Dashboard contains the following components: Dashboard Component Navigation Menu Pages Sections Selection Fields See Detail Links Export Links Description A menu bar that appears at the top of the Dashboard. Use these to navigate to a specific section or page for each data category (Population, Quality, Utilization, Trends, PO Comparisons, and PO Reports). There is one page for each data category (Population, Quality, Utilization, Trends, PO Comparisons, and PO Reports). If you select View All from a navigation menu, you will see all sections on one page. Each page displays sections that contain unique data or data views. You can select to view one section at a time or all sections for a particular page. Use the Selection Fields at the top of each page to limit the data by PO, Sub PO (if applicable), Practice, or Physician. If you want to drill down into the data for a table, click the See Detail link. If a table and graph can be exported, Export links display above that section. NOTE: After you export a table or graph to another program, you can then print it. Data Attribution Dates and Timeframes On the Welcome page, you can find the current timeframes for the data used in the MiPCT Dashboard. For more detailed timeframe information, see the MiPCT Data Timeline document on the MDC Website s MiPCT Support page. Filtering Data Using the Selection Fields By default, the pages display data for the entire MiPCT project. You can use the Selection Fields to view data for only your PO, a SubPO (if applicable), Practice Unit, or Provider. Each time you change one of these Selection Fields, the data displayed on a page automatically updates. Revised: MiPCT_Dashboard_UG_v20_00.docx Page 13 of 95

14 U S E R G U I D E EXAMPLES Following are two examples for how to use the Selection Fields: To look at data for one provider in the PO: 1. In the Select PO drop-down list, select the appropriate PO. 2. From the Select Provider drop-down list, select the appropriate provider. To look at data for all the providers in a particular practice: 1. In the Select PO drop-down list, selected the appropriate PO. 2. Bypass the Select Sub PO drop-down list, leaving Select Sub PO visible in that field. 3. From the Select Practice drop-down list, select the appropriate practice. RESETTING THE SELECTION FIELD VALUES To clear a value in a selection field, scroll to the top of the list and select PO Sub PO Practice Provider. This selection clears all fields. Refreshing a Section You can refresh a section to revert it back to its original default format and data display. To refresh a section, click the refresh icon. This also updates the page data back to the default select all options, which displays data for the entire MiPCT project. Revised: MiPCT_Dashboard_UG_v20_00.docx Page 14 of 95

15 U S E R G U I D E Keep Only and Exclude Options KEEP ONLY If you want to view the data for only one column or chart element in a table or graph, you can select to Keep Only that data. To do so, hover your cursor over the column title or chart element until a pop-up displays. Then select Keep Only. EXCLUDE If want to view everything except a particular column or chart element in a table or graph, you can select to Exclude that data. 16 U S E R G U I D E Accessing Help To view help for a section, click the help icon. The User Guide PDF opens to the appropriate section. From there, you can search the PDF for additional help topics as applicable. Drilling Down to Detailed Member Lists On most tables, you can drill down and view a member list with detailed information about that member related to the type of data you are viewing. If you are viewing population and risk data, the member list contains additional details about a member s risk levels and chronic conditions. If you are viewing quality measures, the member list contains details about a member s quality measure rates. If you are viewing a utilization section, the member list contains details about a member s ED and IP utilization. PROCEDURE To drill down to the detailed list level: 1. In the table for which you wish to see more detail, click the See Detail link. Revised: MiPCT_Dashboard_UG_v20_00.docx Page 16 of 95

17 U S E R G U I D E A new table displays with the highest level information displayed. In the example below, the top level of information is the risk group. 2. For the tables on the Quality and Utilization pages, select the Measure by which you wish to sort the data. 3. Place your cursor over the first column title. A plus sign displays. 4. Click the plus sign to open the next level of detail. 5. Continue to place your cursor over the column titles and click the plus signs to navigate to the detail level you want to view. NOTE: To go back a level, hover the mouse over the column title until the minus sign displays, and then click it. Continue doing so until you are at the desired level. 6. 19 U S E R G U I D E In the Quality and Utilization tables, you can click the rate, the denominator, or the numerator to drill down. 7. In the pop-up that displays, click the View Data icon ( ). 20 U S E R G U I D E 8. Click the Underlying tab. 9. Select the Show all columns check box. 10. You can view detailed data for each patient attributed to the doctor you selected. IMPORTANT: The detailed patient lists are limited to 200 entries on screen. If there are more than 200 entries, click the Download all rows as text file link, which enables you to view the data for all rows in an Excel spreadsheet. 11. If you want to sort lists or copy/paste the data, click the Download all rows as text file link, which enables you to view and save the data in an Excel spreadsheet. Sorting Tables by Columns When you view a table onscreen, you can click the sort icons next to most column headings to sort the list based on that column heading. 21 U S E R G U I D E Null Data in Drill-Down Lists Some fields in the drill-down lists contain Null. This means that there wasn t enough claims information to determine the information or the patient didn t qualify for the measure. Exporting and Printing On each page, you can export the sections or the entire page to PDF or Excel. You can also export an image to an image editing program. You can then print the sections/pages/images as you like. The Export links are included above each section, and there is an Export All to PDF link at the top of each page. The following example indicates where the Export links display above the table in the Attributed Members by Payer section of the Population page. IMPORTANT: If you are exporting a table that also has a graph in the section, you must first highlight the table before you export it. Otherwise, the system only exports the data from the graph. 22 U S E R G U I D E EXPORTING TO PDF To export to PDF: 1. Do one of the following: A. To export only the current section, select the Export to PDF link. B. To export the entire page, select the Export All to PDF link. When the page updates, click the Export to PDF link. The Export PDF window displays, similar to the following: 2. In the Export PDF window, do one of the following: A. If you are exporting all from one of the PO Comparison pages, select A3 from the Paper Size list and keep the defaults for all other selections. B. For all other export options, keep the defaults for all selections. NOTE: Your selection choices are highlighted in blue. 3. Click Export. 4. Click Download. 5. To open the PDF in Acrobat, select the Open with Adobe Reader option, and then click OK. NOTE: You can also save the file if you do not wish to view it first. 6. Print the PDF as needed. 23 U S E R G U I D E EXPORTING TO EXCEL To export to Excel: 1. Highlight the table or graphic that you want to export. 2. Click the Export to Excel link above the section that you want to export. 3. In the Export Crosstab window, click Download. 4. Click OK to open the data in Excel. NOTE: You can also save the file if you don t wish to view it first. 5. Print the spreadsheet as needed. EXPORTING A GRAPHIC On each page, you can export a section to a graphics program for additional editing on your computer. To export a graphic: 1. Above the section that you want to export, select the Export as Image link. 2. In the Export Image window, click Download. 3. In the Open with list, select the program you want to use. 4. Use this page to review attributed membership, risk, and chronic condition information for your entire PO, subpo (if applicable), practice unit, and/or provider. The different sections of this page include the following information: The relative number and percentages of attributed members, by payer Risk information, including: - Number/percent of members by risk group (Very High, High, Medium, etc.) - Average risk score per risk group (uses concurrent risk score data) - Standard cost per member per month (PM PM) by risk group (excludes pharmacy costs) For each chronic condition: - Number/percent of members who have this condition - Standard cost per member per month (PM PM) (excludes pharmacy costs) A bar chart for comparing patient count totals for chronic conditions Risk sores by payer NOTE: Members with claims for which standard cost could not be assigned are not reflected in standard costs on this page. Selecting the Criteria You Want to View In the Selection fields, you can view data for a specific PO (if you have access to more than one), sub PO (if applicable), Practice, or Provider. For more information, see Filtering Data Using the Selection Fields on page 13. Attributed Members by Payer Section This section displays the following: A pie chart illustrating the distribution of attributed members by payer. A table listing the number of attributed members by payer. The table separates the attributed members based on whether they are adults or pediatric patients. It also provides the average Standard Cost PM PM (excluding pharmacy costs) for each payer. Members with claims for which standard cost could not be assigned are not reflected in standard costs on this page. Standard Cost PM PM is blank if a member does not have sufficient claims history to produce a standard cost and PM PM calculation. For information about how member attribution has been determined for the data in the Dashboard, see the Attribution section on page 65 of the Understanding the Data chapter. Member Risk Levels Section This section provides concurrent risk information for all payers participating in the MDC MiPCT project. This section displays the following: A pie chart illustrating the distribution of attributed members according to risk group. A table listing the number of members in each risk group, along with the average risk score and the standard cost per member per month (PM PM) for each risk group (excluding pharmacy costs). Chronic Conditions Section This section of the Population page displays a graph and table showing the number and percentage of attributed patients for each of the following chronic conditions: Attention Deficit Hyperactivity Disorder (ADHD) Asthma Coronary Artery Disease (CAD) Congestive Heart Failure (CHF) aka: Heart Failure Chronic Kidney Disease (CKD) Chronic Obstructive Pulmonary Disease (COPD) Diabetes Hypertension (HTN) Obesity NOTES: Patients with co-morbidities show up in each applicable chronic condition. Members with claims for which standard cost could not be assigned are not reflected in standard costs on this page. Standard Cost PM PM is blank if a member does not have sufficient claims history to produce a standard cost and PM PM calculation. After you navigate to the Download PO Reports page, do one of the following: - If you have access to one PO, verify that the correct PO displays. - If you have access to multiple POs, select the PO for which you want to view reports. 2. Click a report link. Depending on the version of Excel you are using, you may see an error message similar to the following when you attempt to open the Excel file containing the report after extracting it from the Zip file: 3. Click Yes to open the file. NOTE: This message displays if you are using a current version of Excel. In order to accommodate all users, the downloadable PO reports are provided in Excel version Newer versions can still open this file format successfully after clicking Yes to the Excel prompt.
