1 Portfolio Guide PORTFOLIO POINTS GUIDE The Continuing Professional Development () Portfolio Guide provides an overview of the three (3) compulsory categories and the fourth category and how the required can be achieved. Please note the activities provided are examples only. ADEA welcomes other experiential learning, provided these activities can be substantiated. Other activities may be added to these lists in the future as technologies and evidence base changes. Please contact the ADEA Professional Services Manager if you have any queries or suggestions. The Portfolio categories have been developed to address the ADEA National Core Competencies for Credentialled Diabetes Educators: Category 1 Category 2 Category 3 Category 4 Clinical Practice / Diabetes Education / Counselling Management / Administration / Leadership Research Professional Contribution How to use this Guide 1. Read through the examples of activities in each category. 2. Record the specific details of your activities on the Portfolio template. Remember the same activity cannot be claimed in more than one category. 3. Record in your Portfolio the relevant points for each activity, as specified in this Guide. Allocation Generally, one point is allocated per hour of, unless otherwise specified in this guide. Some activities are capped. 3 PORTFOLIO POINTS GUIDE Continuing Professional Development () To complete your portfolio each year, you need to have a minimum of 20 points in total across all categories within the 12 month period. If you are applying for initial credentialling or being audited, you will also need to upload evidence of the activities you have undertaken. The number of points required in each category will depend on which current Role and Scope of Practice you choose below. Categories 1, 2 and 3 are compulsory. You will be required to achieve a minimum of 15 points in the category you choose as your primary role. You will also be required to have a minimum of 1 point in each of the other compulsory categories. The remaining points can be accrued in any of the categories. 4 PORTFOLIO POINTS GUIDE Category One - Clinical Practice ADEA National Conference 3 days. ADEA State conference 1 day. 24 () 8 () Related to ADEA core competency point per hour for attendance at the ADS/ADEA ASM can be claimed. A maximum 24 points can be claimed for the 3 days. This does not include the breakfast symposiums- these can be claimed additionally. Individual workshops at the ASM can t be claimed in additional to the 24 points point per hour for attendance at an ADEA State Conference. A maximum 8 hours can be claimed per day. Claim a 2-day conference by adding the activity on two separate days in the activity box. Evidence required for initial credentialling or if being audited Certificate of attendance or receipt of registration As above Workshops, seminars or conferences relevant to diabetes educators work practice. Grand-rounds or case studies. () Any workshop/seminar/conference that is related to diabetes. 1 point per hour can be claimed. E.g. Roche diabetes educators day; breakfast symposium at the ASM; international diabetes conference (IDF, AADE); other diabetes related conferences (ADIPS); insulin pump workshop; medication/insulin seminars; diabetes management development days; carb counting workshop; seminars on aspects of diabetes complications; seminars on aspects of motivational interviewing/counselling/behavior change Attending case studies discussion in a formal presentation format with members of the medical/multidisciplinary team. Must be related to diabetes. 1 point per hour can be claimed. 5 Category One - Clinical Practice Video or Audio recordings of conferences or seminars Post-graduate study relating to clinical care (per unit successfully completed). * e-learning with clinical care or education topic. Providing clinical support at a diabetes camp. Developing clinical diabetes education tools as appropriate. Delivering education, training and workshops for clinical staff or carers. Related to ADEA core competency PORTFOLIO POINTS GUIDE Watching/listening to recordings of conferences or seminars via DVD, podcasts, webinar recordings etc. 1 point per hour can be claimed. 8/unit Claim 8 points per unit completed Study must be relevant to work practice. *The Graduate Certificate in diabetes education and management is excluded from this post-graduate study claim point per hour can be claimed. Courses from ADEA Learning Management System or other online facilities. Evidence required for initial credentialling or if being audited Certificate or brief summary of the lecture and content (minimum 2 paragraphs) Copy of transcript Certificate of attendance or receipt of registration 5/day 1.1, points per day attending the camp. Letter from Manager/Employer or camp organiser point per hour can be claimed, with a maximum point claim for this activity of 5 per year. Developing patient resources, flyers, group education sessions/ presentations, clinical assessment or recording tools. 2/course points per course delivered can be claimed. Delivering workshops, education sessions or training to clinical staff in Medical centres, hospitals, health centres, schools, group homes, aged care facilities, etc. Letter from Manager/Employer or brief summary of the need and process of development Letter from Manager/Employer or brief summary of the need and target group of the presentation Australian Diabetes Educators Association Credentialling Program 2015 Page 5
6 Category One - Clinical Practice Participating in ward rounds or clinical case review, case conference. Providing clinical placement work experience. Self-directed learning with clinical care or education topic. Related to ADEA core competency PORTFOLIO POINTS GUIDE 1.4.3, 1.5.5, point per hour. Hospital ward rounds, group clinical case review or case conference involving the medical or multidisciplinary team. Review of patient records etc. Evidence of meeting notes (identification details removed for client), diary notes or calendar events point per hour of diabetes related student supervision can be claimed. Providing clinical placement work experience for students DE students or other students. Must be diabetes related point per hour can be claimed. Any other learning not covered above, must be diabetes clinical practice related, e.g. reading evidence based guidelines, text books, undertaking literature review about a certain topic of interest. Evidence required for initial credentialling or if being audited De-identified meeting notes or diary entry/calendar event Letter from Manager/Employer or signed university agreement Certificate or receipt of registration or brief summary of reading and how it relates to your practice as a diabetes educator (minimum 2 paragraphs) Australian Diabetes Educators Association Credentialling Program 2015 Page 6

Development and review of service policies and procedures. Development of clinical pathways incorporating diabetes related management. Related to ADEA core competency , 4.2.3, points per appraisal/review can be claimed. Staff performance appraisal by supervisor/manager on applicant OR Staff performance appraisal where the applicant is in the supervisor/manager role OR Peer review process between colleagues who are both diabetes educators. Applicant can be the reviewer or the person being reviewed point per hour can be claimed, with a maximum of 5 points per year. Involvement in the development of an annual business plan for a diabetes service point per hour can be claimed, with a maximum of 5 points per year. Involvement in the development of a diabetes related policy or procedure point per hour can be claimed, with a maximum of 5 points per year. Involvement in the development of diabetes related clinical pathways and resources that assist. Evidence required for initial credentialling or if being audited Copy of appraisal/peer review including how the issues raised will be addressed. Any s/ meeting minutes relevant to show involvement OR a summary of the process (minimum 2 paragraphs) OR a copy of the business plan showing the date and your name as being involved. Any s/ meeting minutes relevant to show involvement OR a summary of the process (minimum 2 paragraphs) OR a copy of the policy/procedure showing the date and your name as being involved. Any s/meeting minutes relevant to show involvement OR a summary of the process (minimum 2 paragraphs) OR a copy of the pathway showing the date and your name as being involved. 8 Category Two - Management / Administration / Leadership Participation in a committee or working party for diabetes related service issues. 1/ meeting Related to ADEA core competency point per meeting can be claimed. Meeting may be with other diabetes educators or may be multidisciplinary. Service related issues may be referral pathways, triage for high risk patients, setting up a new clinic for a specific group (e.g. young adults / GDM), service promotion within the community, etc. PORTFOLIO POINTS GUIDE Evidence required for initial credentialling or if being audited Any s/meeting minutes relevant to show involvement OR a summary of the process (minimum 2 paragraphs). Management of diabetes related health service budgets. Planning and coordination of a diabetes camp. Self-directed learning with management or administration focus e.g. e-learning, seminars , point per hour can be claimed, with a maximum of 5 points per year. Management and planning of a diabetes related service budget. 5/camp 3.1, 3.2, 3.3, points per camp can be claimed. Involvement in organising, planning, coordinating a diabetes camp point per hour can be claimed. Any other learning not covered above, must be management, leadership or administration focussed e.g. e-learning, seminar, workshop, undertaking literature review about a certain topic of interest - admin/management related. Any s/meeting notes relevant to show involvement OR a summary of the process (minimum 2 paragraphs). Any s/meeting minutes relevant to show involvement OR a summary of the process (minimum 2 paragraphs). 9 Category Two - Management / Administration / Leadership Involvement in Mentoring Partnership or Clinical/Peer Supervision Completing ADEA online Mentoring modules Development of strategic partnerships to enhance diabetes services. Development of diabetes related service business case. Post-graduate study relating to management (per unit successfully completed). * 5/ Partnership / year Related to ADEA core competency 3.2.2, 3.2.3, 3.5.1, 3.5.2, , 3.2.3, 3.5.1, 3.5.2, points can be claimed per partnership per year. These points not available to mentees working towards initial credentialling. 1 point per hour can be claimed. Complete the ADEA online E-Learning Mentoring Modules. These points not available to mentees working towards initial credentialling , points per partnership can be claimed. Partnership and meetings may be with other diabetes educators or may be multidisciplinary point per hour can be claimed, with a maximum of 5 points per year. Management and planning of a diabetes related service business case to enhance diabetes related services points per completed unit can be claimed Study must be relevant to work practice in management, leadership and/or administration. PORTFOLIO POINTS GUIDE Evidence required for initial credentialling or if being audited Provide copy of mentoring agreement OR any s/meeting notes relevant to show involvement. Certificate of completion. Any s/meeting minutes relevant to show involvement OR a summary of the process (minimum 2 paragraphs) including relevance to the diabetes service. Any s/meeting notes relevant to show involvement OR a summary of the process (minimum 2 paragraphs) OR a copy of the business case showing the date and your name as being involved. Copy of transcript *The Graduate Certificate in diabetes education and management is excluded from this post-graduate study claim. 10 PORTFOLIO POINTS GUIDE Category Three - Research Literature review / review of journal articles Conducting a quality improvement activity Participation in a quality improvement activity Author or coordinator of diabetes research Member of a research team Related to ADEA core competency point per hour may be claimed, with a maximum of 5 points per year. Undertake a literature review about a chosen topic points can be claimed per QI activity conducted Leading, managing, planning and implementing a quality improvement project/activity points can be claimed per QI activity. Participating/assisting in a quality improvement project/activity points can be claimed per research project conducted. Author or coordinator of a research project relating to diabetes points can be claimed per research project. Member of the team conducting the research, may not necessarily be an author or coordinator, but have regular involvement in the research and are considered part of the team. Evidence required for initial credentialling or if being audited Provide an (A4) 1 page outline of the literature review including why you chose the articles and how you will use the information in your diabetes education role. Any s/meeting notes relevant to show involvement OR an (A4) 1 page summary of the process, including aims, methods, and outcomes. Any s/meeting notes relevant to show involvement OR a summary of the process, including aims, methods, and outcomes. Provide a copy of the final research report with authors names listed OR any s/meeting notes relevant to show involvement OR a summary of the process, including aims, methods, and outcomes. 11 Category Three - Research Supporting a diabetes research trial (recruitment/ protocols) Publication of peerreviewed papers in a journal Implementation of evidence based changes to practice using literature Development of evidence based resources or policies using literature Application for a research grant. 1/contact Related to ADEA core competency point can be claimed per recruitment or protocol review, with a maximum of 5 points per year. Involvement in a research project/trial, including assisting with recruitment and ensuring protocols meet relevant target group requirements points can be claimed per published article. A research project/activity has been accepted for publishing in a peer-reviewed journal points can be claimed per change to practice Use evidence based guidelines and literature to make changes to practice involving diabetes. 3.3, point per hour can be claimed, with a maximum of 5 points per year. Use evidence based guidelines and literature to develop resources or policies involving diabetes points per application can be claimed Writing and submitting an application for a research grant. PORTFOLIO POINTS GUIDE Evidence required for initial credentialling or if being audited Provide any s/meeting notes relevant to show involvement OR a letter from the research team confirming involvement. Provide a copy of the published article, noting the author/s and the journal it has been published in. Provide an (A4) 1 page outline of the literature review including why you chose the particular topic (the need) and how you have used the information to make changes to practice. Describe the aim, methods and results. Provide an (A4) 1 page outline of the literature review including why you chose the particular resource/policy (the need) and how you have used the information to develop the resource/policy. Describe the aim, methods and results. Any s/meeting notes relevant to show involvement in application submission OR a summary of the process, including need, aims, application process, and relevance to diabetes and/or your role in diabetes education. Post-graduate study relating to research (per unit successfully completed). * Related to ADEA core competency point per hour can be claimed. Study must be relevant to research. *The Graduate Certificate in diabetes education and management is excluded from this postgraduate study claim.
