Specializing in Preventive and Internal Medicine since 1978 Linus Pauling and Preventing Artery Plaque One afternoon 38 years ago all of us medical students were crowded into a small auditorium. Classes were cancelled for the day. Enter an 83 year old silver haired man with 2 pieces of chalk, and a glass of water: Linus Pauling, the only person in history to win the Nobel Prize twice! Over the next 3 hours he tells us that arteries block up because of inflammation. This causes cracks to form in the lining of the artery wall and LDL cholesterol moves under the lining through the cracks. He says that animals that cannot produce their own Ascorbic acid (vitamin C) will have blocked arteries and veins. Man cannot manufacture our own and so we must make vitamin C available at all times to insure healing of the artery wall. Not a word about what he presented is ever discussed or reviewed in my remaining 3 years of school. Fast forward to December I go to a lecture given by a prominent Cardiologist from Tulane University s Medical Center. He says the cardiac fellows and residents put into the computer full information about the 10 major factors of atherosclerosis (Hypertension, Chol, Diabetes, etc). What do they all have in common? Abnormalities of ascorbic acid metabolism. Pauling was right! Want to really protect your arteries against blockage? 3 Intravenous Treatments I.V. #3 - For Energy, Vitality, and Well-Being: TAKE EVERY 3-4 WEEKS We ve resurrected the Myer s Cocktail and improved on the original. We use it to provide energy, vitality and get-up-and-go. With my crazy schedule I take one every week. Most patients do well with one every 3-4 weeks. In the late 1960 s the first intensive care units used Myer s Cocktail an intravenous treatment for energy. This was given to heart patients. It worked fairly well. Over the years more specific I.V. treatments replaced the single cocktail. There was nothing wrong with it, they just developed newer products. Super-C I.V. - To Help Fight Colds, Flu, and Infections: When you feel yourself coming down with an infection, flu or cold don t run to the drugstore. Instead call us and ask for a Super-C I.V. Note: Extend One a better Centrum Silver! Centrum Silver is a hard, poorly absorbing tablet that is recommended for patients under the age of 45! It is actually a decent vitamin for a young person. Extend One is a single capsule that is better made and absorbs more easily. We recommend Extend One instead of Centrum products. Most single, all-in-one tablets are best left for patients under 45 years of age: They are just too difficult to fully digest. Program Two (Better) Extend Core (4/day) from VRP and Co-Q-1O An exceptional vitamin, mineral, and anti-oxidant program. These are not available at stores. Program Three (Best) Complete Life and Co-Q-1O and/or Carni-Q. Dr. H s personal program. Our best vitamin product: It replaces Ultra-Essentials. I personally take both the Co-Q-10 and 2 caps of the Carni-Q (Solinova) which contains both Acetyl-L-Carnitine plus additional Co- Q-10. A lot of pills, a lot of cost, and worth it. Awesome! 5 Insulin: Our Savior: Our Killer Learning the I Word Any animal on this earth with little or no insulin production ain t around for very long. That includes juvenile diabetics who produce little or no insulin and require regular insulin injections to live. Insulin distributes energy to muscle for utilization or to fat for storage. It s a great system. Its taken mankind through feast and (mostly) famine for the 4 million plus years we ve been on this earth. Once you understand how this wonderful system can work to your benefit (or against you) you ll see where the One Page Diet Book idea came from. You ll also be able to understand why adult Type II diabetes develops and why it has reached epidemic proportions. You ll comprehend the Metabolic Syndrome and learn how to break its cycle. Most importantly, by utilizing this God-given system of utilization/storage you ll become lean, strong, and have fantastic energy and vitality. The Parking Lot and the Safe Deposit Boxes Insulin is a hormone produced by the pancreas. It works like a taxicab moving energy trough the body. When energy producing food in absorbed into our body insulin will distribute the energy to either muscle cells for utilization (the Parking Lot ) or to fat cells for storage (the Safe Deposit Boxes ). In a healthy system at least 80% goes to muscle while 20% or less is stored in fat. Inflamed Arteries: The Killer of Modern Man All nutrition in our body flows through a highway system call arteries. Each artery wall has a protective liner called the endothelial liner. When artery walls become inflamed cracks and erosions occur in this protective endothelial liner. Dangerous cholesterols (usually LDL-Cholesterol) move inside the gaps. This starts the process of plaque formation that eventually leads to the blockage of heart or brain arteries. Heart attack or stroke is the end point of this process. You can measure the level of inflammation by having one of two blood tests: Homocysteine or Heartsensitive C - reactive protein (CRP). Another indicator is the calcium level calculations of the CT-Heart Scan (EBT). I would recommend having at least one of the two blood tests done regularly. In the following picture sequence we ll examine the steps leading to this plaque formation. The key idea here is: High levels of blood insulin (Hyperinsulinemia) are the leading initiators of this inflammation process. A Healthy System In healthy system energy-producing foods are consumed, absorbed, and 80%-plus parked in muscle cells. I think of muscle cells as shopping malls with parking spaces. These parking spaces are called insulin receptors. The insulin with the energy food attached to it parks in the receptors of the muscle cells and stays until the energy is utilized. It then shrivels-up and dies, leaving the body through the kidney.

6 The 20% or less of the energy-carrying insulin floats in the arteries over to fat cells for storage. Fat cells don t have anywhere the number of parking spaces (Insulin Receptors) a muscle cell has. Just like going to the safe deposit box at a bank, only a few insulins at a time can be let in to drop off their energy for storage. This forces the other insulins to stay in the arteries waiting their turn. This can cause trouble because these insulins may inflame the artery wall. In a healthy system there is very little insulin going to the safe-deposit boxes of the fat cells. Most goes to the muscle parking spaces. There is very little insulin back up in the blood. Whatever injury occurs to the artery liners should be easy to repair. By the way, the most popular safety-deposit boxes are found in the lowerabdominal fat. Anyone with a lot of abdominal fat (the old Beer Belly ) probably has a high blood insulin level no matter what they weigh. Their insulins are floating around waiting to deposit in the fat safety-deposit boxes, and causing serious inflammation to the arteries. 1. Insulin Resistance An Unhealthy System: The Metabolic Syndrome The Metabolic Syndrome (also called Syndrome X) begins when a patient eats insulin producing energy foods. The insulin takes the food to the muscle cells but when it gets there all the insulin receptor sites (parking spaces) are filled. In fact, the muscle cells have even put out a sign stating Sorry Lot Full! This is called Insulin Resistance. The insulins have nowhere to park. They stay in the blood floating around waiting to deposit in the belly fat safety-deposit boxes, and causing serious inflammation to the arteries. 2. Hyperinsulinemia When the insulins cannot park at the muscle cells they begin to accumulate in the blood. This is called hyperinsulinemia and means higher than normal levels. The situation is worsened by two further factors: a. Fat cells (where the safety-deposit boxes are) have very few parking spaces compared to muscle cells. Thus, just like a bank, only few customers at a time can drop off at the boxes. This backs the insulins up in the blood as they wait to make their deposit of energy for storage. This keeps the insulin level high (hyperinsulinemia) and causes further inflammation to the artery wall. b. While all this is going on our patient keeps eating more and more insulin producing foods. The parking lots at the muscle are filled and the blood insulin level really rises. 3. Abdominal Obesity The storage of energy does proceed at the fat cell safety deposit boxes in the lower abdomen. The belly fat grows: Central obesity. 3. Hypertriglyceridemia and Hyperglycemia The accumulation of energy products (mainly carbohydrates) and insulin in the arteries is accompanied by a rise in blood sugar and triglycerides. The blood sugars generally run in the range. When the insulins really begin to back-up in the blood, they cannot hold onto the sugars. The sugar levels begin to rise to 126 or more. This is called Ádult-Onset (Type II) Diabetes. Thus, the Metabolic Syndrome is really Pre-Diabetes. Anyone with the Metabolic Syndrome is well on his or her way to developing diabetes. Note: Hypertension is often part of the Metabolic Syndrome.

7 Fixing the Problem: Let the System Work for You The same system of utilization and storage that created the problem can be used to correct it. They key here is to get the body away from storage and into utilization. Here s how: 1. Don t Eat Insulin Producing Foods: The Famous One Page Diet Book is based on the simple idea of dramatically lowering your body s insulin production. It is based on the Glycemic Index. Although this index is widely available on the Internet, I would avoid using its information until you really have good control. Keep things simple: Stay with The Famous One Page Diet Book. Strict avoidance of the insulin producing foods is a must. You cannot make-up tomorrow for the insulin foods you eat today. Sorry, avoid bread, potatoes, rice, etc. means just that. 2. Daily Body Movement (and a little weightlifting): The parking spaces of the muscle cells (those insulin receptors ) are filled to the brim. You need to clear insulin out of there and free up those spaces. The muscle has to use the energy in the parked insulin. Those used-up insulins will then disappear through the kidney. No damage will be done. Daily body movement totaling 30 minutes is required. The movement may be gentle such as a nice, leisurely walk. You may also divide the movements into segments of 10 or 15 minutes: It does not have to be continuous. Running, aerobics, kickboxing, and heavy sweating at the health club are good for your psyche (and acceptable alternatives) but are not required. A good idea is to stand in front of your TV set and march in place during a 30-minute show, like the local news. During commercials sit down and do some upper body weight lifting using light weights such as soup cans. Weight lifting creates new muscle cells. These have brand new, empty parking spaces: More open spaces to park the insulin stuck in the blood. 30 minutes of daily body movement will empty out your muscle parking spaces and make room for blood insulins to move in. Combined with a strict avoidance of the insulin producing foods this will nicely lower the insulin level in the arteries.

8 Making a HRa If you have an IRA you should think about having HRA. This is a Health Retirement Account. Before going into the reasons why, let me tell you how to invest in a HRA: 1. No money is required. Forget about money: You ll never have enough. 2. There are a number of ideas listed under the checklist for preventing heart attacks and strokes. Each time you take a walk, or lift soup cans (weights), or take the proper vitamins or fish oil, etc., you re making an investment in your HRA. Read this handout! 3. HRAs pay off by helping you to be disease free, or if you do become sick they help to lessen the illnesses severity or shorten the recovery. 4. HRAs are tax-free (at the time of this writing) Note: Since first writing about HRAs 14 years ago much has happened in the world of healthcare. More than ever before you need to read this section. Why Make an HRA The majority of Americans take very poor care of themselves. They are obese, have huge bellies (high insulin levels) and are sedentary. Nearly 70% of Americans, in fact, get no regular exercise. They seem to have forgotten that having good health does require some personal effort. Over 20% of Americans have developed Type II (adult) Diabetes Mellitus. Diabetes is the most expensive of all diseases. Patients don't actually die of diabetes. They develop circulation disorders of the eyes, heart, kidneys and limbs. We use advanced pharmaceuticals, incredible surgical techniques, and fantastic, imaginative technologies to keep these patients alive. This is very expensive. At present most of the expense of taking care of diabetic expenses is covered by insurance programs. How long can this go on? With the incidence of diabetes growing the cost of care is now prohibitive. The health care system is going broke. This year insurance costs are up nearly 30%! The government has passed healthcare that will cut one-half a trillion dollars out of Medicare and leave the elderly defenseless! Get ready for large clinics to replace your doctor s office. This is already happening. The prescription co-pays are more expensive. Some programs are covering less and less in the way of procedures and surgeries. If you re in an HMO you really know what I m talking about. The changes are subtle, but it s going to get a lot worse. Each year your health insurance card will cover less and less (especially Medicare). Eventually that card will barely be worth the plastic. What to do? Make an HRA. Invest daily. Prevent illness. If you re not getting sick the rest just becomes a political debate. Makes sense, no?
