Does EEG still play a role in 2019? What a question 7T-MRI, fmri, DTI, MEG, SISCOM, Of ieeg course! /HFO, Genetics EEG

What makes the surface EEG unique? Spatial resolution (mm) Invasiveness EEG SPECT MEG ieeg fmri PET Time resolution (sec)

Surface EEG in clinical epilepsy research Electric source analysis in presurgical evaluation Functional connectivity studies

High-resolution EEG with up to 256 surface electrodes 14 Patients with FLE, 7 resective surgery 76 channel surface EEG vs. ieeg In comparison to conventional EEG, SISCOM Correct lateralization heeg 14/14, ceeg 9/14 Concordant to ieeg heeg 12/14 ceeg 3/14 SISCOM 3/12 Feyissa et al. 7 What is EEG source analysis? or Electric Source Imaging (ESI)? (Geneva) Visual inspection Pattern recognition Spikes are similar Maximum negativity FC5/F3/F7

What is EEG source analysis? Every EEG reviewer builds a hypothesis on the localization of the underlying cortical generator of interictal and/or ictal epileptic activity Quality of this hypothesis depends on experience Source analysis leads to more stable hypotheses and allows for better visualization by MRI co-registration Dipole model L2 minimum norm 8

Success of EEG source localization depends on Optimized individual head model

Success of EEG source localization depends on Optimized individual head model Number of electrodes High-resolution EEG (HR-EEG) or High-density EEG (HD-EEG) Seeck et al. 11 Success of EEG source localization depends on Optimized individual head model Number of electrodes High-resolution EEG (HR-EEG) or High-density EEG (HD-EEG) Analysis of spike onset instead of (potentially propagated) peak activity Averaging of similar spikes 11

Summary EEG is THE important diagnostic tool in the management of epilepsy Availability, repeatability High temporal resolution Satisfying spatial resolution Long-term recordings Source analysis of interictal and ictal epileptiform activity is an additional diagnostic tool in presurgical evaluation. The combination with functional connectivity analysis may improve the accuracy. These functional studies can improve our understanding of the pathophysiology of different epileptic syndromes.
