1 Madonna University Athletic Training Room Policies and Procedures Revised June 2016

2 Mission Statement The sports medicine staff strives to maintain the health and wellness of MU student-athletes through the prevention, recognition, management, and rehabilitation of athletic injuries. Team physicians, certified athletic trainers, and student assistants work in collaboration to provide superior athletic health care services for the student-athletes of Madonna University. Overview of Services The certified athletic trainers and sports medicine staff at Madonna University work under the direct supervision of the physicians from Providence Athletic Medicine-Novi. Our team physicians are board certified in family practice with a specialty in sports medicine. Our team physicians are: Dr. Michael Montico, MD, Dr. Scott Eathorn, MD, Dr. David Peck, MD, and Dr. Gregory Cibor, DO. Certified athletic trainers (ATCs) are professionals who specialize in athletic healthcare. They work under the direction of a physician and have extensive education and experience in injury prevention, assessment, care, and rehabilitation. Certified athletic trainers must complete a Bachelors degree program from an accredited institution and pass a national certifying exam to be eligible to practice. Like all healthcare professionals, they are required to keep current in their field by completing continuing education units (CEUs) on a regular basis. The certified athletic trainers and team physicians work together to provide superior athletic healthcare services to the student-athletes of Madonna University. The sports medicine team will provide medical coverage, with its staff and/or students, for all official inseason practices and competitions. Every effort will be made to provide coverage for non-traditional seasons during the academic school year. Summer workouts or non-approved, off-season athletic events will not receive coverage. If a student-athlete is injured while participating in an official intercollegiate sport activity they will be evaluated by a certified athletic trainer, provided immediate care, and referred for follow-up care, when necessary. Appropriate documentation of the injury/illness, including immediate management and followup plan, will be maintained in the student-athlete s personal athletic medical file. Student-Athlete Privacy & Confidentiality The content of a student-athlete s athletic medical file is considered Personal Health Information and is therefore protected by law. Personal Health Information (PHI) includes, but is not limited to: documentation regarding the nature and treatment of an injury/illness, medical history, pre-participation screening exam, participation status, insurance coverage and copies of any other medical records. Information obtained from any of these documents is considered confidential. The sports medicine staff of Madonna University will not disclose (written and/or verbally) any of the information contained in these records without the express written consent of the student-athlete. The signed consent form will be kept on file in the athletic training room. Medical records are kept for a period of 7 years after the date of graduation.

3 Medical Policies 1. All students wishing to participate in intercollegiate athletes at Madonna University must pass a preparticipation health appraisal conducted by the team physicians from Providence Athletic Medicine. No exams from outside physicians (personal physician, walk-in clinic, etc ) will be accepted. 2. No student-athlete is allowed to participate in any sport activity until they are medically cleared by the team physician and the appropriate preseason paperwork is on-file in the training room. 3. Madonna University s team physician is the final authority with regard to all medical disqualifications, treatment, and return of the student-athlete to full participation. If an athlete is referred to a physician for care, a written release from that physician must be on file in the training room before the athlete is allowed to return to participation. 4. Student-athletes must have insurance information on file with the athletic department prior to any athletic participation at MU. All student-athletes are required to have a valid primary insurance policy that covers athletic injuries. 5. The athletic department is responsible only for those injuries incurred in athletic practices, scrimmages, games or conditioning sessions, provided the activity is directly supervised by an authorized Madonna University official. 6. Any referrals and/or appointments, for health care services pertaining to athletic injuries or illnesses, must be coordinated through MU s sports medicine staff. The University assumes no financial responsibility if a student-athlete fails to follow the proper procedures for obtaining treatment. 7. The athletic department is not responsible for dental work or replacing glasses or contacts other than what might occur as the result of participation in supervised athletics practices, scrimmages, games or conditioning. 8. Upon completion of eligibility and/or termination as an athlete at Madonna University, the athletic department will no longer be responsible for the individual s medical or dental injuries. 9. Directions by the team physician or certified athletic trainer will be implemented without alteration by the coach, parent, or student-athlete. Coaches will neither require nor permit an injured and/or ill student-athlete to participate in an event without medical clearance from the medical staff. 10. Failure to comply with any of these regulations relieves Madonna University, its department of athletics and medical staff from all liability and responsibility.

4 Athletic Training Room Rules 1. Use of the athletic training room is reserved strictly for student-athletes involved in the intercollegiate athletics program. 2. Student-athletes are not permitted to utilize the athletic training room without proper supervision. 3. Student-athletes must sign-in EVERY time they receive service in the athletic training room. 4. Student-athletes should report to the athletic training room for taping, treatment, and rehabilitation in proper attire (generally shorts and t-shirts). We reserve the right to refuse treatment to athletes who are not dressed appropriately. 5. Please shower before receiving treatment after activity. 6. Bags and sport equipment should not be brought into the athletic training room. These items should be left in the hall or locker room. 7. No food or drink is allowed in the taping or treatment areas. 8. Abusive and/or foul language, horseplay, loud talking, and/or loitering will not be tolerated. You will be asked to leave. 9. No supplies are to be taken from the athletic training room without the permission of a staff athletic trainer. 10. Student-athletes are not to operate equipment and/or perform self-treatments. 11. The phone, computer, and copy machine in the athletic training room are off limits to all student-athletes. Student-athletes are expected to adhere to the rules and regulations at all times. Failure to comply will result in loss of training room privileges.

6 Purpose This emergency plan is intended to act as a guide for athletic department personnel handling accidents and emergencies that occur at Madonna University athletic events. Although every attempt will be made to ensure a safe environment at all practices and games, unfortunately accidents and injuries sometimes occur. The following document outlines specific steps that should be taken whenever an accident or emergency occurs at a Madonna University athletic venue Personnel The following people have responsibilities under this plan: Team Physician Develop medical policies and procedures relating to all aspects of health care for student-athletes. Evaluate and treat injured/ill student-athletes in primary care office or during training room clinics. Provide medical care for student-athletes, and spectators and coaches, if necessary, during athletic events. Coordinate emergency care and make transport decision, if necessary. Communicate with certified athletic trainer, medical personnel, coaches and parents, when appropriate. Certified Athletic Trainers Provide athletic training services under the direction of the team physician. These services include injury prevention, management, rehabilitation, and education. Work with the team physician to develop medical policies and procedures governing all aspects of athletic health care. Coordinate emergency care and makes transport decision, in absence of team physician. Refer injured student athletes to physician as necessary Supervise athletic training student aides. Maintain documentation of injuries and illness in the student-athlete s medical record. Supervise Athletic Training Student Aides. Communicate with team physician, medical personnel, coaches and parents, when appropriate. Schedule and organize annual Pre-Participation physical exams done on campus. Coaches Assist medical personnel in managing the scene of an injury including crowd and game control. Assist in implementing the emergency plan by activating, directing, and assisting EMS personnel. Notify the certified athletic trainer of all injuries. When traveling, make participation decisions regarding injured athletes after consulting with the host school s certified athletic trainer and medical staff. Current CPR and First Aid certification is strongly recommended. Public Safety Direct emergency service personnel to the location of the accident or injury. Provide first aid for injured or ill spectators.

7 Community EMS Personnel Transport injured or ill student-athletes or spectators to closest hospital or hospital of patient s choice. Provide first aid and emergency care for injured student-athletes or spectators. Athletic Director Assist medical personnel in managing the scene of an injury, including crowd and game control. Assist in implementing the emergency plan by activating, directing, and assisting EMS personnel. Training Room Student Aides (work study) Provide first aid designated by and under the supervision of the certified athletic trainer. When traveling with a team, provide the same services under the direction of the host school s certified athletic trainer. Assist in implementing the emergency plan by activating, directing, and assisting EMS personnel. Maintain current CPR certification. Injury and Accident Prevention 1. All student-athletes participating in intercollegiate athletics must pass an annual pre-participation physical exam prior to their first practice. This exam must be conducted by the team physicians from Providence Athletic Medicine and a copy of the exam on file in the athletic training room. Studentathletes participating in non-traditional seasons are required to have a physical prior to the start of the non-traditional season. 2. A certified athletic trainer will be on site for most traditional season practices and games. The certified athletic trainer will be available by cell phone when not onsite. 3. First-aid supplies will be available on site for every practice and game. Each team is assigned a first aid kit and is responsible for making sure the kit is at all practices and games. Kits should be brought to the training room at least once a week to restock supplies. 4. Coaches should refer all injured or ill student-athletes to the certified athletic trainer as soon as they become aware of an injury. 5. Coaches must notify the athletic training staff of changes in their practice and/or game schedules at least 48 hours in advance. Failure to notify the AT staff in a timely manner may result in no athletic trainer coverage. 6. All coaches should modify their practices under the conditions extreme temperature or inclement weather, in accordance with the WHAC weather policy. 7. All athletic department personnel should be familiar with the guidelines and instructions outlined in this emergency plan and review it annually. 8. All head coaches and assistant coaches are strongly encouraged to obtain and maintain current CPR/AED and First Aid certification through the American Red Cross or American Heart Association.

8 Injury and Accident Management The following steps should be taken each time an injury or accident occurs at a Madonna University athletic practice or game: 1. Injured student-athletes should receive first aid care from the sports medicine staff before leaving the game or practice site. In the absence of the certified athletic trainer, the coach will act as first responder. The certified athletic trainer should be notified of the injury as soon as possible. 2. If the injury or illness appears life threatening, contact EMS immediately by dialing 911 (dial x9-911 from campus phones). If the injury or illness occurs at an on-campus venue, Public Safety must also be contacted by dialing x5442 from any campus phone ( from cell phone). Public Safety will be responsible for directing EMS to the location needing assistance. The certified athletic trainer, coach, or a designee will be responsible for activating EMS. 3. A plan for medical follow-up will be developed by the sports medicine staff and communicated to the injured student athlete prior to leaving the practice or game site. 4. If an injury or illness requires a referral to the team physician or other health care provider, the certified athletic trainer will aid in facilitating this process. All referrals will be documented using an appropriate form. 5. If a spectator becomes injured or ill, Public Safety should be contacted immediately. First aid for all injured/ill spectators should be provided by Public Safety or EMS personnel. The athletic training staff may assist in the provision of care when doing so does not impair their ability to attend to the needs of the studentathletes. 6. When an injury or illness occurs at an athletic event hosted by another school, the host school s emergency plan should be followed. Coaches traveling with a Madonna athletic team should seek out the certified athletic trainer or team physician for assistance. 7. When determining a student-athlete s participation status, adhere to the medical chain of command: Team Physician Certified Athletic Trainer Coach
