1 TREATING PROBLEM SINUSES Breathing Freely Again

2 When You Have Sinus Problems Do you suffer from sinus problems? If so, you re not alone. Sinus problems are very common. They can cause uncomfortable symptoms, such as pain and pressure in the head or face. They can also make it hard to breathe freely. Fortunately, sinus problems often can be treated. Talk to your doctor about your options. Also read this booklet to learn more. What Causes Sinus Problems? Sinuses are hollow air-filled chambers within the bones of the face and head. Problems develop when these chambers are blocked and can t drain properly. Causes of blockage include colds or other infections, allergies, or obstructions in the nose. Symptoms of Sinus Problems Sinus problems can cause a number of symptoms. These can include: Facial pain, pressure, tenderness, or swelling A stuffy nose (nasal congestion) Thick drainage from the nose Postnasal drip Reduced sense of smell and taste Headache Cough Tiredness (fatigue) Toothache Bad breath Fever 2

3 Working with Your Doctor Your doctor will examine you to help determine the cause of your problem. Based on the diagnosis, you and your doctor can work together to form a treatment plan. In some cases, self-care and medications may be all that are needed. In others, surgery may be recommended to treat underlying problems. Your doctor will talk with you about your options as well as their benefits and risks. Be sure to ask any questions you have and get the answers you need. 3

4 The Sinuses: A Drainage System Normally, the sinuses make a thin layer of fluid called mucus, which drains into the nose. This mucus keeps the lining of the sinuses and nose moist. It also helps to trap and remove dust, bacteria, and viruses from the sinuses and nasal passages. If mucus becomes trapped in the sinuses, problems can develop. Healthy Sinuses There are four pairs of sinuses. Mucus drains from the sinuses into the nasal cavity through small openings. In healthy sinuses, the drainage paths for mucus are clear and open. The septum is the wall of cartilage and bone in the center of the nasal cavity. Turbinates are ridges in the nose. Normal Cilia Damaged Cilia Frontal sinus Ethmoid sinus Sphenoid sinus Maxillary sinus The sinuses are lined with tiny hairlike cilia. They help move mucus through the sinuses to be drained into the nose. Due to inflammation or infection, cilia can become unable to work correctly. This allows mucus to build up. 4

5 Blocked Sinuses Problems such as colds and allergies may cause the drainage passages in the nose or sinuses to become blocked. As a result, mucus cannot drain properly. The sinuses may become inflamed (irritated and swollen) or infected, causing a condition called sinusitis or rhinosinusitis. Common causes of sinus blockage include those listed below. Colds and other respiratory infections cause the nose and sinus lining to make more and thicker mucus. The lining also swells. Mucus becomes trapped, which can lead to pressure and pain. Polyps Polyps are sacs of swollen tissue in the lining of the nose and sinuses. They may form due to irritation and can block drainage. Allergies irritate nasal and sinus tissues, such as turbinates. This causes swelling and blockage. Over time, irritation can cause polyps to form. A deviated (crooked) septum may block nasal passages and make sinus problems worse. The septum may have been injured or have formed with a bend in it. Acute or Chronic? Acute sinus problems are temporary. They usually last less than 4 weeks. Once the problem goes away, the sinuses most often return to normal. Chronic sinus problems last 12 weeks or longer. They may be caused by problems that never go away or that keep coming back. Chronic sinus inflammation may result from damage to sinus and nasal tissues. 5

6 Your Evaluation A thorough evaluation helps your doctor diagnose your sinus problem and determine the best options for treating it. This evaluation will include a medical history and physical exam. Certain tests may also be done. Medical History and Exam Your doctor will ask about your symptoms and how long you ve had them. Be sure to mention if you ve had a recent cold or infection. Also mention if you have allergies or other health problems. Outline any treatments you ve tried and whether they ve worked for you. Your doctor will examine your face for redness, swelling, or tenderness. He or she will look into your nose, ears, and throat. Endoscopic Exam If needed, an endoscopic exam is done to get a closer look at the sinuses and nasal passages. For this, a thin lighted tube called an endoscope is used. First, the nose and sinuses may be numbed. A decongestant may also be given to reduce swelling. Then, the scope is passed into the nose. The scope sends live images of the sinus openings to a monitor. These can show problems such as infection, polyps, or a deviated septum. Endoscope An endoscope helps the doctor check for problems in the nose and sinuses. 6

7 You may be referred to an otolaryngologist. This is a doctor who specializes in treating ear, nose, and throat (ENT) problems. Tests You May Have CT scanning may be done. This test takes detailed images of the nose and sinuses from different angles. CT scans may help show areas of inflammation and blockage. X-rays may be taken. These create images of the nose and sinuses. They may help rule out structural problems. Nose and sinus cultures may be done to check for an infection. Allergy tests may be done to see if you are allergic to substances such as dust mites or pollen. In some people, allergies can contribute to sinus problems. Your Treatment Plan Sinus A CT scan can reveal problems such as sinusitis and a deviated septum. To check if you have allergies, the doctor may order skin or blood tests. Treatment options depend on the cause of your sinus problem and the severity of your symptoms. They also depend on whether your problems are acute or chronic. You and your doctor will work together to decide on your treatment plan. The goals of treatment are to: Reduce inflammation of the nose and sinuses. Improve drainage of mucus. Treat any infection, if needed. Prevent future sinus problems from occurring. 7

8 Self-Care Self-care measures are things you can do yourself to help relieve symptoms. When treating sinus problems, self-care is a good place to start. It s often effective, especially for acute sinusitis. For Symptom Relief Your doctor may recommend one or more of the self-care measures below. He or she may also suggest additional things to try. Drink Fluids Drinking a glass of fluid every hour or two helps thin mucus. This allows it to drain from your sinuses more easily. Fluids can also offset the drying effects of certain medications. Water and clear broth are good choices. Use a Humidifier or Vaporizer Humidifiers and vaporizers are small appliances you can use to increase moisture in a room. They may help to keep the sinuses moist and also relieve congestion. Ask your doctor if you should use one. Be sure to clean these devices regularly. They are prone to mold growth, which can trigger allergies and sinus problems in some people. Use Saline Rinses Saline (saltwater) rinses help remove thick mucus and allow your sinuses to drain. Mix ¾ teaspoon of noniodized salt and ¼ teaspoon of baking soda in 1 cup of warm distilled water. Use a bulb syringe to gently squirt the mixture into your nose as directed by your doctor. You can buy bulb syringes, sinus rinse kits, and prepared saline sprays at most drugstores. Apply Hot or Cold Packs Applying heat to the area surrounding your sinuses may make you feel more comfortable. Use a hot-water bottle or a hand towel dipped in hot water. Some people also find that cold helps relieve sinus pain. Use a soft ice pack or bag of frozen peas wrapped in a thin towel. When you have a sinus problem, drinking plenty of fluids can thin mucus and relieve congestion. 8

9 Medications Medications help relieve symptoms and also treat underlying sinus problems. They may be over the counter or prescription. Work with your doctor to find the best medication plan for you. Possible Options Decongestants help reduce swelling so mucus can drain. They are available in both pill and spray forms. Note that decongestant nasal sprays should only be used for short periods. Long-term use can cause recurrent ( rebound ) Some medications for sinus swelling of the sinuses. problems are given as nasal sprays. However, pills may be prescribed if a stronger dose of medication is needed for a short time. Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help relieve inflammation, swelling, and pain. Antihistamines help relieve symptoms if you have allergies. Antihistamines are available over the counter and by prescription. Allergy immunotherapy may be recommended if you have persistent allergies. If used regularly, this treatment helps make your body less sensitive to the substances that trigger your allergies. To Treat Infection Sinusitis sometimes leads to an infection. If you have an infection caused by bacteria, you may be prescribed antibiotics to treat it. Note that antibiotics will not help infections, colds, or flu caused by viruses. In certain cases, it s best to let a sinus infection clear up on its own, without medication. Your doctor can tell you more and outline your options. 9

10 If Surgery Is Needed If other treatments fail to relieve your sinus problem, your doctor may recommend surgery. Surgery is done through the nose using an endoscope. For this reason, it s called endoscopic surgery. No incisions outside the nose are needed. Your doctor can explain the surgery to you as well as its risks and benefits. How Endoscopic Surgery Is Done The endoscope is inserted into the nostrils through the nasal passages to the sinus openings. The doctor views images from the endoscope on a video monitor. Other instruments are used with the endoscope to perform treatment. Often, more than one area of the nose and sinuses is treated. Some procedures that may be done are shown on the next page. Risks and Complications Endoscopic surgery is very safe, but it does have some risks. These can include: Infection Bruising Excessive bleeding Altered sense of smell or taste Damage to a tear duct Spinal fluid leakage (very rare) Vision loss (very rare) Your doctor will go over the details of your surgery with you. If you have questions or concerns, be sure to discuss them with your doctor. 10

11 Before After Clearing Blockages Sinus openings blocked by swelling and infection prevent proper drainage. Your doctor may remove Blocked bone, tissue, or some of sinus the sinus lining to clear a path. This helps improve drainage. Before After Removing Polyps If one or more polyps block a sinus opening, they can prevent drainage into the nose. Removing Polyp polyps clears the sinus opening so the sinus can drain again. Before After Straightening the Septum If a deviated septum is contributing to sinus problems, the doctor may straighten it. The cartilage and bone that divide the nose are trimmed or reshaped. This creates Deviated more space for drainage septum airflow. and 11

12 Having Surgery If you need surgery, know what to expect before, during, and afterward. Surgery is done in a hospital or surgery center. You will most likely go home the same day. In certain cases, an overnight stay is needed. Before Surgery Follow all instructions for preparing for surgery. These include: Stop smoking. Smoking increases surgery risks and slows healing. Have any tests requested by your doctor. These can include blood tests as well as a CT scan. Tell your doctor about all of the medications you take. This includes over-the-counter medications, herbal remedies, vitamins, and supplements. Also mention if you take aspirin or medications to prevent blood clots. You may need to stop taking some or all of these before surgery. Stop eating and drinking before surgery as directed. Arrange for an adult family member or friend to give you a ride home after surgery. Before surgery, an IV (intravenous) line is placed into your arm or hand. It supplies fluid and medications. Preventing Pain Just before the procedure begins, you ll be given anesthesia to prevent pain. Depending on the type, this may make you drowsy and relaxed, lightly asleep, or put you in a state like deep sleep. Right After Surgery When your surgery is completed, you ll move to recovery. You may feel groggy from the anesthesia and will likely have some discomfort. Pain medication will be given as needed. You may have ice on your face to help control swelling. A dressing of gauze or other material may be placed inside your nose to absorb drainage. This is called packing. It may need to remain in place for several days. 12

13 At Home You may be asked to recover at home for about a week after surgery. It s normal for your nose to feel stuffy. You may also have pinkish or dark red drainage and some nasal crusting. As you heal, be sure to: Apply a cold pack to the nasal area as directed for the first day or two after surgery. This helps reduce swelling. Take medications as directed. Don t take aspirin or ibuprofen until your doctor says it s okay. Drink plenty of water. Change your nasal dressing as needed. Do saline sinus rinses as instructed. Avoid strenuous exercise, including straining or lifting. Avoid blowing your nose. If you sneeze, do so with your mouth open. Follow-Up Visits Schedule a follow-up visit with the doctor. He or she will check your nose and sinuses to see how well they are healing. Any packing is also removed at this time. You may need to schedule additional visits. Ask about these and about any long-term follow-up. When to Call the Doctor Once home, call your doctor right away if you have any of these: A large amount of bright red bleeding Drainage that is yellowish or greenish Fever of F (38 C) or higher Changes in vision or swelling around the eye A constant headache or increasing pain Drainage of a large amount of clear fluid Extreme tiredness (fatigue) or a stiff neck Other signs or symptoms indicated by your doctor 13

14 Ongoing Prevention Even after treatment, sinus problems can come back. So be sure to follow your doctor s advice to help prevent this. The tips on these pages can help get you started. Moisten Nasal Tissues Keep your nose and sinuses from drying out. This thins mucus, making it easier to drain. It also helps prevent irritation of the sinus and nasal lining. To help maximize moisture: Drink plenty of water and other clear fluids. Limit alcoholic beverages. These can have a drying effect and may worsen symptoms. Ask your doctor about using saline rinses or a humidifier or vaporizer (see page 8). Prevent Colds and Other Infections Do what you can to avoid exposure to colds, flu, and other infections. Wash your hands often during cold and flu season. Get a flu shot every year. Stay away from people who are infected, when possible. Get plenty of rest daily. When washing your hands, use warm water and soap. Be sure to wash for at least 20 seconds. 14

15 Avoid Allergens Allergens are the things that trigger allergies. Get tested to find out what you re allergic to. Then take steps to limit your exposure to them. Some tips: House dust mites are a common allergen. Wear a filter mask when you clean. Remove items that collect dust, such as books and stuffed toys, from the bedroom. If you are allergic to animal dander, avoid having a cat or dog. If you do have one, keep it outside or at least out of the bedroom. If you are allergic to pollen, limit time outdoors on windy days. Avoid Irritants Irritants are things that can irritate the lining of the nose and sinuses, such as smoke or strong odors. To help avoid them: Don t smoke. Also, stay away from places where other people smoke. Avoid perfumes or strong scents if these trigger or worsen your sinus symptoms. Ask your doctor before swimming in chlorinated pools. Chlorine triggers sinus symptoms in some people. Use scent-free products in your home to avoid exposure to irritants such as strong odors or fumes. Sinuses and Air Travel Air travel can be difficult when you re prone to sinus problems. Airplane air dries out your sinuses. Pressure changes can cause sinus pain. To keep your sinuses moist when flying, carry a saline nasal spray. Drink plenty of fluids. You may also want to try using a decongestant beforehand. Ask your doctor for more recommendations. 15

16 Work with Your Doctor Don t let sinus problems get you down. Work with your doctor to form the best treatment plan for your needs. Then follow your plan as directed to keep sinus problems under control. This can help you breathe better and feel more comfortable again.
