Once again, we are proud to publish the proceedings of the meeting in this supplement. This is a very special conference in the history of both the Society and the Journal because we can confirm that, beginning in 2018, all EUNOS members will receive a print and online subscription to the Journal through their membership fee. This will enable us to establish ever stronger mutual ties and cooperation has also been a very special year for the Journal because we are now listed on PubMed Central allowing free access to previously published content. This means that the HTML full-text version of articles published in the Journal are deposited into PubMed Central and made searchable on PubMed/ MEDLINE 12 months after publication. 5 S2 EUNOS 2017 EUNOS President s Introduction Dear Colleagues, The 13th biennial meeting of EUNOS is being held in Budapest, the capital of Hungary, on September 10 13, A huge amount of preparation by the organizing committee under the leadership of Gabriella Szatmáry has resulted in a very appealing program, as presented in this special issue of Neuro-Ophthalmology. As the current President of EUNOS I d like to welcome you in central European Budapest and I am confident that our professional society will continue to grow and bring together old and new members from Portugal in the West to Russia in the East, from Norway in the North to Italy in the South, as well as from many more countries outside of Europe. We hope that you will spend memorable days with us, recharging your batteries with new knowledge, and getting together with old friends and colleagues and meeting new ones. 6 NEURO-OPHTHALMOLOGY S3 Chair of the Organizing Commission s Introduction: Welcome to Budapest Dear Colleagues, It is my great pleasure to welcome you to Budapest for the 13th European Neuro-Ophthalmological Society conference. After the very successful 1st EUNOS Update Course in Budapest in 2012, we are honoured to host the main Congress in the serene setting of the Danubius Health Spa Resort Hélia, which is located on the East Bank of the Danube River across from Saint Margaret Island. Within short distances of the hotel are historic sites such as the Hungarian Parliament, the Heroes Square, the Széchenyi Chain Bridge, and the famous Széchenyi Thermal Bath, the largest outdoor thermal bath in all of Europe. The members of the organizing committee are very proud to present to you a comprehensive program covering the latest clinically relevant brain and eye research, with a focus on emerging imaging tools, and their role in untangling the pathophysiology of complex neuro-ophthalmic disorders. The presenters include fifty plus internationally renowned speakers and four keynote lecturers. The first day of the meeting includes two teaching courses, Emergency Neuro-Ophthalmology and Practical Tools and Tricks, to enhance the differential diagnostic and clinical skills of physicians encountering patients afflicted by neuro-ophthalmic disorders. In addition, for the first time in the history of EUNOS, we will have a half-day Research Symposium dedicated to optic nerve disorders. Also, the present state and future directions of European research initiatives will be discussed. Interaction between the clinician, the scientist, and the afflicted population we serve will be enhanced by having Leber hereditary optic neuropathy patient representatives from three European nations at the meeting. A wide range of topics will be presented during the meeting, such as Genetics and Gene Therapy in Neuro-Ophthalmology, neurodegenerative disorders causing neuro-ophthalmic abnormalities in The Eye and The Brain Symposium, Vascular Disorders with Neuro-Ophthalmic Manifestations, Neuro- Ophthalmic Complications of Neoplastic Disease, Eye Pain and Alikes, Beyond IIH: High and Low Cerebrospinal Fluid Pressure Syndromes, Neuro-Ophthalmic Applications of Eye and Brain Imaging, Future Directions in Neuro-Ophthalmology, and Unexplained Visual Loss as introduced by interesting case presentations. The last day of the meeting will include a lecture by Prof. Neil Miller on Controversies in Neuro-Ophthalmology and the Scientific Symposium with abstracts selected for oral presentations. Beside the scientific program and exhibition, there will be a number of exciting social activities to make your stay in Budapest pleasurable and memorable, including the gala dinner served while cruising on the Danube River and enjoying the spectacular panorama of both Buda and Pest. Mollan (UNITED KINGDOM) INVESTIGATION AND MANAGEMENT OF ACUTE SINUS AND SELLAR PATHOLOGY CAUSING OPTIC NEUROPATHY Anthony C. Arnold UCLA Stein Eye Institute, Los Angeles, California, USA The sella turcica and the adjacent sinuses may be the sites for disease processes affecting the optic nerves, but they are most often slowly progressive disorders. Less commonly, acute lesions may produce rapid and severe loss of vision due to optic neuropathy. This presentation focuses on the clinical features, appropriate diagnostic testing and radiologic features, and management of the following conditions: (1) Pituitary apoplexy (2) Sinusitis Acute bacterial sinusitis with ethmoid sinus involvement; Fungal sinusitis secondary to mucormycosis or aspergillosis; Non-infectious sinusitis from granulomatosis with polyangiitis (GPA). It is the commonest immune-mediated primary systemic vasculitis affecting medium to large arteries that almost exclusively affects patients over 50 years of age with an increased incidence with increasing age and a striking female predominance. For decades the American College of Rheumatology Criteria for GCA have been the corner stone guiding clinicians. However, it is typically the serological markers of inflammation that can be difficult to interpret. Until recently temporal artery biopsy (TAB) was the definitive answer, when positive. Results: Ultrasound has now contested TAB as the gold standard for diagnosis with the results of the TABUL study [1] showing that clinical judgement at two weeks, combined with the tests showed sensitivity and specificity of 91% and 81%, respectively, for TAB and 93% and 77%, respectively, for US. The limitations of both TAB and cranial ultrasound will be compared. In the field of research other modalities, such as MRI, are being investigated but due to limited access and high cost have yet to be taken up widely.

20 NEURO-OPHTHALMOLOGY S17 Conclusions: The diagnosis is often challenging due to the heterogenesis nature of the presentation. It requires a full clinical history, thorough examination and a combination of investigations to diagnose the disease. The key features in the history and initial examination, which indicate the need for urgent investigation will be identified. The key investigation modalities and the importance of the order of these investigations will be highlighted. PRESENTATION AND INVESTIGATION OF THIRD NERVE PALSIES Dániel Bereczki Department of Neurology, Semmelweis University, Budapest, Hungary The nucleus of the 3rd cranial nerve (CN3, oculomotor nerve) is located in the tegmentum of the mesencephalon at the level of the superior colliculus. The root fibers cross the superior cerebellar peduncle and the red nucleus and exit the brainstem into the interpeduncular fossa. CN3 then runs on the skull base, enters the cavernous sinus, then the orbita via the superior orbital fissure. Lesions of both the intraparenchymal and the extraparenchymal course of CN3 should be considered if CN3 dysfunction is found at physical examination. Small ischemic lesion in the mesencephalon may involve the nucleus or the intraparenchymal part of CN3. If the ischemic lesion also involves the pyramidal tract, Weber-syndrome develops: ipsilateral CN3 lesion with contralateral hemiparesis. In rare cases mesencephalic hemorrhage may result in the same clinical syndrome. Mesencephalic lesions in multiple sclerosis, in brainstem encephalitis or in malignancies may result in CN3 damage. Distal to exiting to the interpeduncular fossa and before entering the cavernous sinus, brain herniation, meningeal carcinomatosis, basilar meningitis and aneurysm of the posterior communicating artery may result in clinical signs of CN3 lesion. In the cavernous sinus thrombosis, granulomatosis, and carotid-cavernous fistule; whereas in the supraorbital fissure and the orbit injuries, tumors and local inflammation may affect CN3. Systemic diseases may also result in CN3 damage. Cranial neuropathy of CN3 is frequently a complication of diabetes mellitus, presenting with usually unilateral, pupil-sparing CN3 lesion. Microvascular disorder is assumed in the background. Miller Fisher syndrome a manifestation of the anti-gq1b IgG antibody syndrome presents with ataxia, hyporeflexia and ophthalmoplegia. Space occupying lesions leading to uncal herniation results in compression of CN3. Risk factors and associating clinical signs might help to approach the diagnosis. Neuroimaging preferably multimodal MRI including DWI is needed. If MRI reveals normal conditions, examination of
